Marine traffic simulations are carried out in a virtual port area with various arrangements and conditions. Calculations of stress values and traffic volume criteria are illustrated. The simulation results provide a valuable hint for safety management of vessel traffic. The model provides quantitative information and helps administrators in decision making to achieve desired safety level and improve the efficiency of vessel traffic in ports and waterways.展开更多
In order to clear the activated area in electrostatic accelerator facilities,four accelerator facilities were selected and typical neutron emission experiments were performed.Neutron flux during operation and induced ...In order to clear the activated area in electrostatic accelerator facilities,four accelerator facilities were selected and typical neutron emission experiments were performed.Neutron flux during operation and induced activity caused by charged particles on the accelerator and its surrounding area after irradiation were measured.Also the monitored neutron flux and calculated value by Monte Carlo calculation using PHITS code were compared.It was confirmed that the results between calculated data and measured data showed the good agreement with each other.Finally,it was concluded that we have to take care the activation of beam line and target.But,it is not necessary to treat accelerator tank,surrounding materials,and building concrete as radioactive materials in case of decommissioning.展开更多

Design Prospecti ve interventional case series. Methods Thirty five eyes of 32 patients with idi opathic stage 2 or 3 macular hole were enrolled in this study. The best correct ed visual acuity (BCVA), cross sectional image of the macular hole by optical c oherence tomography (OCT), and retinal thickness in the central ( < 1000 μm), i nner (1000 to 2220 μm), and outer ring areas (2220 to 3450 μm) as defined by t he OCT retinal mapping program were evaluated preoperatively and at 1, 3, 6, and 12 months postoperatively. The change in retinal thickness of the inner ring ar ea at the 6month postoperative period was used to evaluate the degree of preoper ative retinal deformation. The macular hole index (MHI) (ratio of hole height to base diameter of hole) was calculated and correlated with minimum diameter of h ole, base diameter of hole, the postoperative decrease in macular thickness, and the postoperative BCVA. The postoperative BCVA was further evaluated in two pat ient matched groups. Results Retinal thickness values in the inner ring area we re decreased at the 1 month postoperative period. MHI significantly correlated with the postoperative decrease in macular thickness in the inner ring area at 6 months (correlation coefficient=-0.632, P=.030, Spearman analysis) and with th e postoperative BCVA (P=.013, multiple regression analysis). Postoperative BCVA in the MHI ≥0.5 group was better than that in the MHI < 0.5 group (P= .032, Man n Whitney test). Conclusions The MHI is a ratio easily calculated from OCT tran sverse images of the macular area. The authors present an infant with critical esophageal stenosis owing to tracheo bronchial remnant. This lesion was managed successfully by circular myectomy of the esophageal wall without resection. An 1-year-old girl was referred for rec urrent vomiting and dysphagia. An esophagogram showed an abrupt stenosis, and en doscopic ultrasonography showed cartilaginous tracheo bronchial remnants within the esophageal wall. The extirpation of muscular lay er, which contained cartilage, was attempted. Circular muscular layer was resect ed in 1 cm width leaving mucosal layer intact. Muscular layer was closed horizon tally with absorbable sutures. The patient is free from the symptoms and eats no rmally 1 year after surgery. In case of short segmental stenosis owing to trache obronchial remnant, this may be the preferred approach.展开更多

RESULTS: A 53-year-old woman with POEMS syndrome was referred complaining of long-standing impaired vision. Typical CME demonstrating petal-like dye pooling was observed in both eyes. Aft er subtenon administrations of triamcinolone acetonide (TA), pars plana vitrecto my, and intraocular TA injection in the right eye, macular thickness decreased. Intraocular VEGF levels were < 31 pg/ml at the vitrectomy and intraocular TA inj ection, whereas serum the VEGF level was >2000 pg/ml throughout the follow-up periods. In the untreated left eye, macular th ickness altered related to serum VEGF level. CONCLUSIONS: Based on the low intra ocular VEGF level and serum VEGF-related change of macular thickness, elevated serum VEGF might be a cause of CME in POEMS syndrome.展开更多

A large erosive nodule developed on the scr...We have previously cloned human papillomavirus type 82 (HPV- 82) from a vaginal intraepithelial neoplasia, but it is not known whether HPV- 82 can induce a cutaneous lesion. A large erosive nodule developed on the scrotum of a 50- year- old Japanese patient. Histopathologically, the lesion was composed of two distinct parts; one part showing changes characteristic of Bowen’ s disease in the epidermis, and the other showing elongated rete ridges and proliferation of atypical basaloid cells in the dermis. These parts were partially connected, giving the diagnosis of Bowen’ s carcinoma. Immunohistochemically, HPV capsid antigen was detected only in the nuclei of a few cells on the upper part of the epidermis. HPV- 82 was identified in the lesion by blot hybridization and viral DNA was demonstrated in the lesion by in situ hybridization. HPV- 82 has tropism for both the skin and the genital region.展开更多
