Therefore, thepreservation and maintenance of these roads are critical to attaining a pavement network in good conditionthroughout its service life. Various performance indicators like the international roughness index (IRI), pavementcondition index (PCI), and present serviceability rating (PSR) have been used by the state department of transportation (DOT) and highway agencies for evaluating pavement surface conditions and planning future maintenance strategies. Limited data availability, multiple distresses depending on region, lack of correlation of thesecondition indices to maintenance strategies, and data collection limitations pose a challenge for applying theseindices to local conditions. This paper compares condition indices of different states for rigid pavements. Further,using a specific condition index for local conditions is also highlighted. For this purpose, five states and theircorresponding condition indices were evaluated and compared to the Michigan DOT distress index (DI). Thesestates include Virginia, Minnesota, North Dakota, Louisiana, and Oregon. The corresponding distresses of eachcondition index were converted to make them compatible with the MDOT DI. This study used the MDOT'spavement management system (PMS) database to evaluate each condition index for 433 rigid pavement sections.Each distress index was plotted against MDOT DI and compared using a paired t-test. Results show that thecondition indices of Virginia and Minnesota are comparable to DI in terms of the Spearman correlation value. Thet-test results show that except for Virgina, condition indices from other states statistically differ from DI.Therefore, one can't use those directly for local conditions in Michigan. This paper presents the evaluation anddata requirements for each condition index and its impact on selecting a maintenance treatment.展开更多

A possible contributor to this dieback is sapstreak disease caused by Ceratocystis virescens (Davidson) Moreau. Unhealthy trees with considerable crown dieback were evaluated across the western Upper Peninsula, MI to determine the prevalence of the sapstreak fungus using a minimally destructive sampling technique. Approximately 8% of 90 trees sampled were sapstreak positive and approximately 10% of trees were positive at one site that had recently been harvested. While the high levels of maple dieback present in these forests appear not to be directly caused by widespread sapstreak disease, the occurrence of sapstreak may be significantly impacting trees at some locations. However, even when present on a low number of trees, the biointeraction of sapstreak and decay rates from other fungi could be important for future tree mortality and value to the forest industry. Therefore, the effect of two sapstreak fungal isolates on the amount of decay caused by two common maple white rot fungi, Trametes versicolor (L.:Fr.) Pilat. And Irpex lacteus (Fr.:Fr.) Fr. was tested in the laboratory. Sugar maple wood blocks were precolonized by two native isolates of C. virescens followed by inoculation and incubation with decay fungi. Mean percent weight loss of blocks by white rot decay fungi ranged from 39% to 55%, but decay rates were not significantly affected by the presence of the sapstreak fungus.展开更多
Understanding how non-industrial private forest (NIPF) owners gain and share information regarding the management of their property is very important to policy makers, yet our knowledge regarding how and to what degre...Understanding how non-industrial private forest (NIPF) owners gain and share information regarding the management of their property is very important to policy makers, yet our knowledge regarding how and to what degree this information flows over privately owned landscapes is limited. The work described here seeks to address this shortfall. Widely administered surveys with close-ended questions may not adequately capture this information flow within NIPF owner communities. This study used open-ended questions in interviews of clusters of NIPF owners to determine whether and to what extent owners in-fluence each other directly (through conversations or referrals to sources of advice) or indirectly (through observation of management). We obtained data from thirty-four telephone interviews with owners of NIPF properties in the Western Upper Peninsula of Michigan, and analyzed the data using open coding. Roughly half of the forest owners we interviewed were influenced either directly or indirectly by other members of their NIPF communities. Reasons for owning forests (such as privacy, hunting and nature recreation, and economics) also influenced owners’ management behaviors and goals. This peer-to-peer flow of information (whether direct or indirect) has significant implications for how to distribute management and programmatic information throughout NIPF owner communities, and how amenable these communities may be to cooperative or cross-boundary programs to achieve ecosystem and landscapescale goals.展开更多

BACKGROUND Non-alcoholic fatty liver disease(NAFLD)increases cardiovascular disease(CVD)risk irrespective of other risk factors.However,large-scale cardiovascular sex and race differences are poorly understood.AIM To ...BACKGROUND Non-alcoholic fatty liver disease(NAFLD)increases cardiovascular disease(CVD)risk irrespective of other risk factors.However,large-scale cardiovascular sex and race differences are poorly understood.AIM To investigate the relationship between NAFLD and major cardiovascular and cerebrovascular events(MACCE)in subgroups using a nationally representative United States inpatient sample.METHODS We examined National Inpatient Sample(2019)to identify adult hospitalizations with NAFLD by age,sex,and race using ICD-10-CM codes.Clinical and demographic characteristics,comorbidities,and MACCE-related mortality,acute myocardial infarction(AMI),cardiac arrest,and stroke were compared in NAFLD cohorts by sex and race.Multivariable regression analyses were adjusted for sociodemographic characteristics,hospitalization features,and comorbidities.RESULTS We examined 409130 hospitalizations[median 55(IQR 43-66)years]with NFALD.NAFLD was more common in females(1.2%),Hispanics(2%),and Native Americans(1.9%)than whites.Females often reported non-elective admissions,Medicare enrolment,the median age of 55(IQR 42-67),and poor income.Females had higher obesity and uncomplicated diabetes but lower hypertension,hyperlipidemia,and complicated diabetes than males.Hispanics had a median age of 48(IQR 37-60),were Medicaid enrollees,and had non-elective admissions.Hispanics had greater diabetes and obesity rates than whites but lower hypertension and hyperlipidemia.MACCE,all-cause mortality,AMI,cardiac arrest,and stroke were all greater in elderly individuals(P<0.001).MACCE,AMI,and cardiac arrest were more common in men(P<0.001).Native Americans(aOR 1.64)and Asian Pacific Islanders(aOR 1.18)had higher all-cause death risks than whites.CONCLUSION Increasing age and male sex link NAFLD with adverse MACCE outcomes;Native Americans and Asian Pacific Islanders face higher mortality,highlighting a need for tailored interventions and care.展开更多

BACKGROUND Fecal incontinence(FI)is an involuntary passage of fecal matter which can have a significant impact on a patient’s quality of life.Many modalities of treatment exist for FI.Sacral nerve stimulation is a we...BACKGROUND Fecal incontinence(FI)is an involuntary passage of fecal matter which can have a significant impact on a patient’s quality of life.Many modalities of treatment exist for FI.Sacral nerve stimulation is a well-established treatment for FI.Given the increased need of magnetic resonance imaging(MRI)for diagnostics,the In-terStim which was previously used in sacral nerve stimulation was limited by MRI incompatibility.Medtronic MRI-compatible InterStim was approved by the United States Food and Drug Administration in August 2020 and has been widely used.AIM To evaluate the efficacy,outcomes and complications of the MRI-compatible InterStim.METHODS Data of patients who underwent MRI-compatible Medtronic InterStim placement at UPMC Williamsport,University of Minnesota,Advocate Lutheran General Hospital,and University of Wisconsin-Madison was pooled and analyzed.Patient demographics,clinical features,surgical techniques,complications,and outcomes were analyzed.Strengthening the Reporting of Observational studies in Epidemiology(STROBE)cross-sectional reporting guidelines were used.RESULTS Seventy-three patients had the InterStim implanted.The mean age was 63.29±12.2 years.Fifty-seven(78.1%)patients were females and forty-two(57.5%)patients had diabetes.In addition to incontinence,overlapping symptoms included diarrhea(23.3%),fecal urgency(58.9%),and urinary incontinence(28.8%).Fifteen(20.5%)patients underwent Peripheral Nerve Evaluation before proceeding to definite implant placement.Thirty-two(43.8%)patients underwent rechargeable InterStim placement.Three(4.1%)patients needed removal of the implant.Migration of the external lead connection was observed in 7(9.6%)patients after the stage I procedure.The explanation for one patient was due to infection.Seven(9.6%)patients had other complications like nerve pain,hematoma,infection,lead fracture,and bleeding.The mean follow-up was 6.62±3.5 mo.Sixty-eight(93.2%)patients reported significant improvement of symptoms on follow-up evaluation.CONCLUSION This study shows promising results with significant symptom improvement,good efficacy and good patient outcomes with low complication rates while using MRI compatible InterStim for FI.Further long-term follow-up and future studies with a larger patient population is recommended.展开更多

This paper presents a risk-informed data-driven safe control design approach for a class of stochastic uncertain nonlinear discrete-time systems.The nonlinear system is modeled using linear parameter-varying(LPV)syste...This paper presents a risk-informed data-driven safe control design approach for a class of stochastic uncertain nonlinear discrete-time systems.The nonlinear system is modeled using linear parameter-varying(LPV)systems.A model-based probabilistic safe controller is first designed to guarantee probabilisticλ-contractivity(i.e.,stability and invariance)of the LPV system with respect to a given polyhedral safe set.To obviate the requirement of knowing the LPV system model and to bypass identifying its open-loop model,its closed-loop data-based representation is provided in terms of state and scheduling data as well as a decision variable.It is shown that the variance of the closedloop system,as well as the probability of safety satisfaction,depends on the decision variable and the noise covariance.A minimum-variance direct data-driven gain-scheduling safe control design approach is presented next by designing the decision variable such that all possible closed-loop system realizations satisfy safety with the highest confidence level.This minimum-variance approach is a control-oriented learning method since it minimizes the variance of the state of the closed-loop system with respect to the safe set,and thus minimizes the risk of safety violation.Unlike the certainty-equivalent approach that results in a risk-neutral control design,the minimum-variance method leads to a risk-averse control design.It is shown that the presented direct risk-averse learning approach requires weaker data richness conditions than existing indirect learning methods based on system identification and can lead to a lower risk of safety violation.Two simulation examples along with an experimental validation on an autonomous vehicle are provided to show the effectiveness of the presented approach.展开更多

The delicate equilibrium between energy availability and expendi...During the periparturient period, dairy cows exhibit negative energy balance due to limited appetite and increased energy requirements for lactogenesis. The delicate equilibrium between energy availability and expenditure puts cows in a state of metabolic stress characterized by excessive lipolysis in white adipose tissues(AT), increased production of reactive oxygen species, and immune cell dysfunction. Metabolic stress, especially in AT, increases the risk for metabolic and inflammatory diseases. Around parturition, cows are also susceptible to endotoxemia. Bacterial-derived toxins cause endotoxemia by promoting inflammatory processes and immune cell infiltration in different organs and systems while impacting metabolic function by altering lipolysis, mitochondrial activity, and insulin sensitivity. In dairy cows, endotoxins enter the bloodstream after overcoming the defense mechanisms of the epithelial barriers, particularly during common periparturient conditions such as mastitis, metritis, and pneumonia, or after abrupt changes in the gut microbiome. In the bovine AT, endotoxins induce a pro-inflammatory response and stimulate lipolysis in AT, leading to the release of free fatty acids into the bloodstream. When excessive and protracted, endotoxin-induced lipolysis can impair adipocyte's insulin signaling pathways and lipid synthesis. Endotoxin exposure can also induce oxidative stress in AT through the production of reactive oxygen species by inflammatory cells and other cellular components. This review provides insights into endotoxins' impact on AT function, highlighting the gaps in our knowledge of the mechanisms underlying AT dysfunction, its connection with periparturient cows' disease risk, and the need to develop effective interventions to prevent and treat endotoxemia-related inflammatory conditions in dairy cattle.展开更多

Background Dairy cows are at high risk of fatty liver disease in early lactation,but current preventative measures are not always effective.Cows with fatty liver have lower circulating branched-chain amino acid(BCAA)c...Background Dairy cows are at high risk of fatty liver disease in early lactation,but current preventative measures are not always effective.Cows with fatty liver have lower circulating branched-chain amino acid(BCAA)concentra-tions whereas cows with high circulating BCAA levels have low liver triglyceride(TG).Our objective was to determine the impact of BCAA and their corresponding ketoacids(branched-chain ketoacids,BCKA)on production performance and liver TG accumulation in Holstein cows in the first 3 weeks postpartum.Methods Thirty-six multiparous Holstein cows were used in a randomized block design experiment.Cows were abomasally infused for the first 21 d postpartum with solutions of 1)saline(CON,n=12);2)BCA(67 g valine,50 g leu-cine,and 34 g isoleucine,n=12);and 3)BCK(77 g 2-ketovaline calcium salt,57 g 2-ketoleucine calcium salt,and 39 g 2-ketoisoleucine calcium salt,n=12).All cows received the same diet.Treatment effects were determined using PROC GLIMMIX in SAS.Results No differences were detected for body weight,body condition score,or dry matter intake averaged over the first 21 d postpartum.Cows receiving BCK had significantly lower liver TG concentrations compared to CON(6.60%vs.4.77%,standard error of the mean(SEM)0.49)during the first 3 weeks of lactation.Infusion of BCA increased milk yield(39.5 vs.35.3 kg/d,SEM 1.8),milk fat yield(2.10 vs.1.69 kg/d,SEM 0.08),and lactose yield(2.11 vs.1.67 kg/d,SEM 0.07)compared with CON.Compared to CON,cows receiving BCA had lower plasma glucose(55.0 vs.59.2 mg/dL,SEM 0.86)but higherβ-hydroxybutyrate(9.17 vs.6.00 mg/dL,SEM 0.80).Conclusions Overall,BCAA supplementation in this study improved milk production,whereas BCKA supplementa-tion reduced TG accumulation in the liver of fresh cows.展开更多

In this paper,we develop bound-preserving discontinuous Galerkin(DG)methods for chemical reactive flows.There are several difficulties in constructing suitable numerical schemes.First of all,the density and internal e...In this paper,we develop bound-preserving discontinuous Galerkin(DG)methods for chemical reactive flows.There are several difficulties in constructing suitable numerical schemes.First of all,the density and internal energy are positive,and the mass fraction of each species is between 0 and 1.Second,due to the rapid reaction rate,the system may contain stiff sources,and the strong-stability-preserving explicit Runge-Kutta method may result in limited time-step sizes.To obtain physically relevant numerical approximations,we apply the bound-preserving technique to the DG methods.Though traditional positivity-preserving techniques can successfully yield positive density,internal energy,and mass fractions,they may not enforce the upper bound 1 of the mass fractions.To solve this problem,we need to(i)make sure the numerical fluxes in the equations of the mass fractions are consistent with that in the equation of the density;(ii)choose conservative time integrations,such that the summation of the mass fractions is preserved.With the above two conditions,the positive mass fractions have summation 1,and then,they are all between 0 and 1.For time discretization,we apply the modified Runge-Kutta/multi-step Patankar methods,which are explicit for the flux while implicit for the source.Such methods can handle stiff sources with relatively large time steps,preserve the positivity of the target variables,and keep the summation of the mass fractions to be 1.Finally,it is not straightforward to combine the bound-preserving DG methods and the Patankar time integrations.The positivity-preserving technique for DG methods requires positive numerical approximations at the cell interfaces,while Patankar methods can keep the positivity of the pre-selected point values of the target variables.To match the degree of freedom,we use polynomials on rectangular meshes for problems in two space dimensions.To evolve in time,we first read the polynomials at the Gaussian points.Then,suitable slope limiters can be applied to enforce the positivity of the solutions at those points,which can be preserved by the Patankar methods,leading to positive updated numerical cell averages.In addition,we use another slope limiter to get positive solutions used for the bound-preserving technique for the flux.Numerical examples are given to demonstrate the good performance of the proposed schemes.展开更多

Many people with hypertension have poorly controlled blood pressure and up to half of...Background: Cardiovascular diseases are the leading cause of death globally and hypertension is a major contributor to this burden. Many people with hypertension have poorly controlled blood pressure and up to half of the adults with hypertension are unaware of their hypertensive status due to factors that bother on poor management and poor screening approaches. The implication is that people who have poor access to healthcare especially those in the rural communities are at increased risk of cardiovascular complications and all-cause mortality. Unfortunately, not much has been done to ascertain the burden of undiagnosed hypertension and associated risk factors in rural communities in Nigeria. Methods: We conducted a community-based cross-sectional study in a rural community in Imo State, Nigeria, on burden of undiagnosed hypertension with participants recruited via a multi-stage sampling method. An interviewer-administered questionnaire was used, and standardized instruments were applied to obtain, process and analyze the data. Tests of association between the independent variables and outcome were conducted using logistic regression. P-value of Results: A total of 380 adults participated in the study. The mean age was 44.2 years. The prevalence of undiagnosed hypertension was 35.8%. Logistic regression revealed that age, with the respondents in the age groups 26 - 35 years (OR = 10.647, 1.910 - 59.345, p-value = 0.007), 36 - 45 (OR = 3.680, 1.263 - 10.723, p-value = 0.017), 46 - 55 years (OR = 2.737, 1.114 - 6.727, p-value = 0.039), 56 - 65 years old (OR = 3.384, 1.610 - 7.115, p-value = 0.001);and being married (OR = 3.846, 1.118 - 13.233, p-value = 0.033), were independent risk factors for undiagnosed hypertension. Conclusion: The prevalence of undiagnosed hypertension in the rural population of South-East Nigeria is high. Younger age (26 - 35 years) had the highest odds of risk for occurrence of hypertension. 文摘Pavement infrastructure is vital in providing services and links between various sectors of society. Therefore, thepreservation and maintenance of these roads are critical to attaining a pavement network in good conditionthroughout its service life. Various performance indicators like the international roughness index (IRI), pavementcondition index (PCI), and present serviceability rating (PSR) have been used by the state department of transportation (DOT) and highway agencies for evaluating pavement surface conditions and planning future maintenance strategies. Limited data availability, multiple distresses depending on region, lack of correlation of thesecondition indices to maintenance strategies, and data collection limitations pose a challenge for applying theseindices to local conditions. This paper compares condition indices of different states for rigid pavements. Further,using a specific condition index for local conditions is also highlighted. For this purpose, five states and theircorresponding condition indices were evaluated and compared to the Michigan DOT distress index (DI). Thesestates include Virginia, Minnesota, North Dakota, Louisiana, and Oregon. The corresponding distresses of eachcondition index were converted to make them compatible with the MDOT DI. This study used the MDOT'spavement management system (PMS) database to evaluate each condition index for 433 rigid pavement sections.Each distress index was plotted against MDOT DI and compared using a paired t-test. Results show that thecondition indices of Virginia and Minnesota are comparable to DI in terms of the Spearman correlation value. Thet-test results show that except for Virgina, condition indices from other states statistically differ from DI.Therefore, one can't use those directly for local conditions in Michigan. This paper presents the evaluation anddata requirements for each condition index and its impact on selecting a maintenance treatment.
文摘The stability analysis of an abandoned underground gypsum mine requires the determination of the mine pillar’s strength.This is especially important for flooded abandoned mines where the gypsum pillars become saturated and are subjected to dissolution after flooding.Further,mine pillars are subjected to blast vibrations that generate some level of macro- and micro-fracturing.Testing samples of gypsum must,therefore,simulate these conditions as close as possible.In this research,the strength of gypsum is investigated in an as-received saturated condition using uniaxial compressive strength (UCS),Brazilian tensile strength (BTS) and point load index (PLI) tests.The scale effect was investigated and new correlations were derived to describe the effect of sample size on both UCS and BTS under dry and saturated conditions.Effects of blasting on these parameters were observed and the importance of choosing the proper samples was discussed.Finally,correlations were derived for both compressive and tensile strengths under dry and saturated conditions from the PLI test results,which are commonly used as a simple substitute for the indirect determination of UCS and BTS.
文摘Unusually high levels of dieback have recently been reported in sugar maple, Acer saccharum Marsh., in Upper Michigan, and a network of plots was established to determine the extent and factors associated with the dieback. A possible contributor to this dieback is sapstreak disease caused by Ceratocystis virescens (Davidson) Moreau. Unhealthy trees with considerable crown dieback were evaluated across the western Upper Peninsula, MI to determine the prevalence of the sapstreak fungus using a minimally destructive sampling technique. Approximately 8% of 90 trees sampled were sapstreak positive and approximately 10% of trees were positive at one site that had recently been harvested. While the high levels of maple dieback present in these forests appear not to be directly caused by widespread sapstreak disease, the occurrence of sapstreak may be significantly impacting trees at some locations. However, even when present on a low number of trees, the biointeraction of sapstreak and decay rates from other fungi could be important for future tree mortality and value to the forest industry. Therefore, the effect of two sapstreak fungal isolates on the amount of decay caused by two common maple white rot fungi, Trametes versicolor (L.:Fr.) Pilat. And Irpex lacteus (Fr.:Fr.) Fr. was tested in the laboratory. Sugar maple wood blocks were precolonized by two native isolates of C. virescens followed by inoculation and incubation with decay fungi. Mean percent weight loss of blocks by white rot decay fungi ranged from 39% to 55%, but decay rates were not significantly affected by the presence of the sapstreak fungus.
文摘Understanding how non-industrial private forest (NIPF) owners gain and share information regarding the management of their property is very important to policy makers, yet our knowledge regarding how and to what degree this information flows over privately owned landscapes is limited. The work described here seeks to address this shortfall. Widely administered surveys with close-ended questions may not adequately capture this information flow within NIPF owner communities. This study used open-ended questions in interviews of clusters of NIPF owners to determine whether and to what extent owners in-fluence each other directly (through conversations or referrals to sources of advice) or indirectly (through observation of management). We obtained data from thirty-four telephone interviews with owners of NIPF properties in the Western Upper Peninsula of Michigan, and analyzed the data using open coding. Roughly half of the forest owners we interviewed were influenced either directly or indirectly by other members of their NIPF communities. Reasons for owning forests (such as privacy, hunting and nature recreation, and economics) also influenced owners’ management behaviors and goals. This peer-to-peer flow of information (whether direct or indirect) has significant implications for how to distribute management and programmatic information throughout NIPF owner communities, and how amenable these communities may be to cooperative or cross-boundary programs to achieve ecosystem and landscapescale goals.

文摘BACKGROUND Non-alcoholic fatty liver disease(NAFLD)increases cardiovascular disease(CVD)risk irrespective of other risk factors.However,large-scale cardiovascular sex and race differences are poorly understood.AIM To investigate the relationship between NAFLD and major cardiovascular and cerebrovascular events(MACCE)in subgroups using a nationally representative United States inpatient sample.METHODS We examined National Inpatient Sample(2019)to identify adult hospitalizations with NAFLD by age,sex,and race using ICD-10-CM codes.Clinical and demographic characteristics,comorbidities,and MACCE-related mortality,acute myocardial infarction(AMI),cardiac arrest,and stroke were compared in NAFLD cohorts by sex and race.Multivariable regression analyses were adjusted for sociodemographic characteristics,hospitalization features,and comorbidities.RESULTS We examined 409130 hospitalizations[median 55(IQR 43-66)years]with NFALD.NAFLD was more common in females(1.2%),Hispanics(2%),and Native Americans(1.9%)than whites.Females often reported non-elective admissions,Medicare enrolment,the median age of 55(IQR 42-67),and poor income.Females had higher obesity and uncomplicated diabetes but lower hypertension,hyperlipidemia,and complicated diabetes than males.Hispanics had a median age of 48(IQR 37-60),were Medicaid enrollees,and had non-elective admissions.Hispanics had greater diabetes and obesity rates than whites but lower hypertension and hyperlipidemia.MACCE,all-cause mortality,AMI,cardiac arrest,and stroke were all greater in elderly individuals(P<0.001).MACCE,AMI,and cardiac arrest were more common in men(P<0.001).Native Americans(aOR 1.64)and Asian Pacific Islanders(aOR 1.18)had higher all-cause death risks than whites.CONCLUSION Increasing age and male sex link NAFLD with adverse MACCE outcomes;Native Americans and Asian Pacific Islanders face higher mortality,highlighting a need for tailored interventions and care.

文摘Rough set theory places great importance on approximation accuracy,which is used to gauge how well a rough set model describes a target concept.However,traditional approximation accuracy has limitations since it varies with changes in the target concept and cannot evaluate the overall descriptive ability of a rough set model.To overcome this,two types of average approximation accuracy that objectively assess a rough set model’s ability to approximate all information granules is proposed.The first is the relative average approximation accuracy,which is based on all sets in the universe and has several basic properties.The second is the absolute average approximation accuracy,which is based on undefinable sets and has yielded significant conclusions.We also explore the relationship between these two types of average approximation accuracy.Finally,the average approximation accuracy has practical applications in addressing missing attribute values in incomplete information tables.
文摘BACKGROUND Fecal incontinence(FI)is an involuntary passage of fecal matter which can have a significant impact on a patient’s quality of life.Many modalities of treatment exist for FI.Sacral nerve stimulation is a well-established treatment for FI.Given the increased need of magnetic resonance imaging(MRI)for diagnostics,the In-terStim which was previously used in sacral nerve stimulation was limited by MRI incompatibility.Medtronic MRI-compatible InterStim was approved by the United States Food and Drug Administration in August 2020 and has been widely used.AIM To evaluate the efficacy,outcomes and complications of the MRI-compatible InterStim.METHODS Data of patients who underwent MRI-compatible Medtronic InterStim placement at UPMC Williamsport,University of Minnesota,Advocate Lutheran General Hospital,and University of Wisconsin-Madison was pooled and analyzed.Patient demographics,clinical features,surgical techniques,complications,and outcomes were analyzed.Strengthening the Reporting of Observational studies in Epidemiology(STROBE)cross-sectional reporting guidelines were used.RESULTS Seventy-three patients had the InterStim implanted.The mean age was 63.29±12.2 years.Fifty-seven(78.1%)patients were females and forty-two(57.5%)patients had diabetes.In addition to incontinence,overlapping symptoms included diarrhea(23.3%),fecal urgency(58.9%),and urinary incontinence(28.8%).Fifteen(20.5%)patients underwent Peripheral Nerve Evaluation before proceeding to definite implant placement.Thirty-two(43.8%)patients underwent rechargeable InterStim placement.Three(4.1%)patients needed removal of the implant.Migration of the external lead connection was observed in 7(9.6%)patients after the stage I procedure.The explanation for one patient was due to infection.Seven(9.6%)patients had other complications like nerve pain,hematoma,infection,lead fracture,and bleeding.The mean follow-up was 6.62±3.5 mo.Sixty-eight(93.2%)patients reported significant improvement of symptoms on follow-up evaluation.CONCLUSION This study shows promising results with significant symptom improvement,good efficacy and good patient outcomes with low complication rates while using MRI compatible InterStim for FI.Further long-term follow-up and future studies with a larger patient population is recommended.
文摘The deficiencies of popular phthalate plasticizers(ready migration from a polymer matrix into which they have been incorporated,flammability,environmental pollution,human health risks)have stimulated efforts to develop new effective,nonmigrating,low-cost,nontoxic replacements.In the main,these have been based on readilyavailable,nontoxic biobased precursors.Some,including those prepared from plant oils,have been generated from biomaterials themselves.However,the more numerous and generally more effective have been generated from discrete compounds produced from various biomaterials.Several structural features of effective plasticizers have been recognized.Polar functionality is required to assure compatibility with a wide range of polymeric materials,including poly(vinyl chloride)(PVC),the most heavily plasticized polymer.A branched structure greatly enhances the effectiveness of compounds used as plasticizers.An oligomeric structure may strongly limit or prevent migration from a polymer matrix.Hyperbranched oligomers of defined structure derived from the readilyavailable,inexpensive,nontoxic biomonomers,glycerol and adipic acid contain all these features and are excellent plasticizers.They contain ester functionality,are highly branched,and display a large number of end groups,all of which contribute to their effectiveness as plasticizers.

文摘This paper presents a risk-informed data-driven safe control design approach for a class of stochastic uncertain nonlinear discrete-time systems.The nonlinear system is modeled using linear parameter-varying(LPV)systems.A model-based probabilistic safe controller is first designed to guarantee probabilisticλ-contractivity(i.e.,stability and invariance)of the LPV system with respect to a given polyhedral safe set.To obviate the requirement of knowing the LPV system model and to bypass identifying its open-loop model,its closed-loop data-based representation is provided in terms of state and scheduling data as well as a decision variable.It is shown that the variance of the closedloop system,as well as the probability of safety satisfaction,depends on the decision variable and the noise covariance.A minimum-variance direct data-driven gain-scheduling safe control design approach is presented next by designing the decision variable such that all possible closed-loop system realizations satisfy safety with the highest confidence level.This minimum-variance approach is a control-oriented learning method since it minimizes the variance of the state of the closed-loop system with respect to the safe set,and thus minimizes the risk of safety violation.Unlike the certainty-equivalent approach that results in a risk-neutral control design,the minimum-variance method leads to a risk-averse control design.It is shown that the presented direct risk-averse learning approach requires weaker data richness conditions than existing indirect learning methods based on system identification and can lead to a lower risk of safety violation.Two simulation examples along with an experimental validation on an autonomous vehicle are provided to show the effectiveness of the presented approach.

文摘During the periparturient period, dairy cows exhibit negative energy balance due to limited appetite and increased energy requirements for lactogenesis. The delicate equilibrium between energy availability and expenditure puts cows in a state of metabolic stress characterized by excessive lipolysis in white adipose tissues(AT), increased production of reactive oxygen species, and immune cell dysfunction. Metabolic stress, especially in AT, increases the risk for metabolic and inflammatory diseases. Around parturition, cows are also susceptible to endotoxemia. Bacterial-derived toxins cause endotoxemia by promoting inflammatory processes and immune cell infiltration in different organs and systems while impacting metabolic function by altering lipolysis, mitochondrial activity, and insulin sensitivity. In dairy cows, endotoxins enter the bloodstream after overcoming the defense mechanisms of the epithelial barriers, particularly during common periparturient conditions such as mastitis, metritis, and pneumonia, or after abrupt changes in the gut microbiome. In the bovine AT, endotoxins induce a pro-inflammatory response and stimulate lipolysis in AT, leading to the release of free fatty acids into the bloodstream. When excessive and protracted, endotoxin-induced lipolysis can impair adipocyte's insulin signaling pathways and lipid synthesis. Endotoxin exposure can also induce oxidative stress in AT through the production of reactive oxygen species by inflammatory cells and other cellular components. This review provides insights into endotoxins' impact on AT function, highlighting the gaps in our knowledge of the mechanisms underlying AT dysfunction, its connection with periparturient cows' disease risk, and the need to develop effective interventions to prevent and treat endotoxemia-related inflammatory conditions in dairy cattle.

文摘We investigate the following inverse problem:starting from the acoustic wave equation,reconstruct a piecewise constant passive acoustic source from a single boundary temporal measurement without knowing the speed of sound.When the amplitudes of the source are known a priori,we prove a unique determination result of the shape and propose a level set algorithm to reconstruct the singularities.When the singularities of the source are known a priori,we show unique determination of the source amplitudes and propose a least-squares fitting algorithm to recover the source amplitudes.The analysis bridges the low-frequency source inversion problem and the inverse problem of gravimetry.The proposed algorithms are validated and quantitatively evaluated with numerical experiments in 2D and 3D.

文摘This paper reviews the adaptive sparse grid discontinuous Galerkin(aSG-DG)method for computing high dimensional partial differential equations(PDEs)and its software implementation.The C++software package called AdaM-DG,implementing the aSG-DG method,is available on GitHub at https://github.com/JuntaoHuang/adaptive-multiresolution-DG.The package is capable of treating a large class of high dimensional linear and nonlinear PDEs.We review the essential components of the algorithm and the functionality of the software,including the multiwavelets used,assembling of bilinear operators,fast matrix-vector product for data with hierarchical structures.We further demonstrate the performance of the package by reporting the numerical error and the CPU cost for several benchmark tests,including linear transport equations,wave equations,and Hamilton-Jacobi(HJ)equations.

文摘Background Dairy cows are at high risk of fatty liver disease in early lactation,but current preventative measures are not always effective.Cows with fatty liver have lower circulating branched-chain amino acid(BCAA)concentra-tions whereas cows with high circulating BCAA levels have low liver triglyceride(TG).Our objective was to determine the impact of BCAA and their corresponding ketoacids(branched-chain ketoacids,BCKA)on production performance and liver TG accumulation in Holstein cows in the first 3 weeks postpartum.Methods Thirty-six multiparous Holstein cows were used in a randomized block design experiment.Cows were abomasally infused for the first 21 d postpartum with solutions of 1)saline(CON,n=12);2)BCA(67 g valine,50 g leu-cine,and 34 g isoleucine,n=12);and 3)BCK(77 g 2-ketovaline calcium salt,57 g 2-ketoleucine calcium salt,and 39 g 2-ketoisoleucine calcium salt,n=12).All cows received the same diet.Treatment effects were determined using PROC GLIMMIX in SAS.Results No differences were detected for body weight,body condition score,or dry matter intake averaged over the first 21 d postpartum.Cows receiving BCK had significantly lower liver TG concentrations compared to CON(6.60%vs.4.77%,standard error of the mean(SEM)0.49)during the first 3 weeks of lactation.Infusion of BCA increased milk yield(39.5 vs.35.3 kg/d,SEM 1.8),milk fat yield(2.10 vs.1.69 kg/d,SEM 0.08),and lactose yield(2.11 vs.1.67 kg/d,SEM 0.07)compared with CON.Compared to CON,cows receiving BCA had lower plasma glucose(55.0 vs.59.2 mg/dL,SEM 0.86)but higherβ-hydroxybutyrate(9.17 vs.6.00 mg/dL,SEM 0.80).Conclusions Overall,BCAA supplementation in this study improved milk production,whereas BCKA supplementa-tion reduced TG accumulation in the liver of fresh cows.
文摘This article discusses the literature review article by Pacheco et al published in July 2024;the authors provided good reviews of perianal Crohn’s disease(CD),and challenges faced by clinicians in the management.CD,characterized by its chronic and relapsing nature,is an idiopathic condition that can involve any segment of the gastrointestinal tract.Perianal disease impacts up to 40%of patients with CD,with perianal fistulas constituting up to 80%of perianal lesions.Perianal CD can be highly incapacitating and profoundly diminish the overall well-being of patients.The management focuses on controlling the perianal sepsis and treating luminal CD.Biologics are crucial to the treatment approach,and results have been encouraging.The surgery focuses on controlling the sepsis,with more definitive treatments being fistula surgery,fecal diversion,and proctectomy as the last resort.This manuscript briefly describes the burden of CD,the challenges posed by perianal CD,and the role of different treatment modalities from colorectal surgeon’s perspective.

文摘Additive Runge-Kutta methods designed for preserving highly accurate solutions in mixed-precision computation were previously proposed and analyzed.These specially designed methods use reduced precision for the implicit computations and full precision for the explicit computations.In this work,we analyze the stability properties of these methods and their sensitivity to the low-precision rounding errors,and demonstrate their performance in terms of accuracy and efficiency.We develop codes in FORTRAN and Julia to solve nonlinear systems of ODEs and PDEs using the mixed-precision additive Runge-Kutta(MP-ARK)methods.The convergence,accuracy,and runtime of these methods are explored.We show that for a given level of accuracy,suitably chosen MP-ARK methods may provide significant reductions in runtime.

文摘In this paper,we develop bound-preserving discontinuous Galerkin(DG)methods for chemical reactive flows.There are several difficulties in constructing suitable numerical schemes.First of all,the density and internal energy are positive,and the mass fraction of each species is between 0 and 1.Second,due to the rapid reaction rate,the system may contain stiff sources,and the strong-stability-preserving explicit Runge-Kutta method may result in limited time-step sizes.To obtain physically relevant numerical approximations,we apply the bound-preserving technique to the DG methods.Though traditional positivity-preserving techniques can successfully yield positive density,internal energy,and mass fractions,they may not enforce the upper bound 1 of the mass fractions.To solve this problem,we need to(i)make sure the numerical fluxes in the equations of the mass fractions are consistent with that in the equation of the density;(ii)choose conservative time integrations,such that the summation of the mass fractions is preserved.With the above two conditions,the positive mass fractions have summation 1,and then,they are all between 0 and 1.For time discretization,we apply the modified Runge-Kutta/multi-step Patankar methods,which are explicit for the flux while implicit for the source.Such methods can handle stiff sources with relatively large time steps,preserve the positivity of the target variables,and keep the summation of the mass fractions to be 1.Finally,it is not straightforward to combine the bound-preserving DG methods and the Patankar time integrations.The positivity-preserving technique for DG methods requires positive numerical approximations at the cell interfaces,while Patankar methods can keep the positivity of the pre-selected point values of the target variables.To match the degree of freedom,we use polynomials on rectangular meshes for problems in two space dimensions.To evolve in time,we first read the polynomials at the Gaussian points.Then,suitable slope limiters can be applied to enforce the positivity of the solutions at those points,which can be preserved by the Patankar methods,leading to positive updated numerical cell averages.In addition,we use another slope limiter to get positive solutions used for the bound-preserving technique for the flux.Numerical examples are given to demonstrate the good performance of the proposed schemes.
文摘Background: Cardiovascular diseases are the leading cause of death globally and hypertension is a major contributor to this burden. Many people with hypertension have poorly controlled blood pressure and up to half of the adults with hypertension are unaware of their hypertensive status due to factors that bother on poor management and poor screening approaches. The implication is that people who have poor access to healthcare especially those in the rural communities are at increased risk of cardiovascular complications and all-cause mortality. Unfortunately, not much has been done to ascertain the burden of undiagnosed hypertension and associated risk factors in rural communities in Nigeria. Methods: We conducted a community-based cross-sectional study in a rural community in Imo State, Nigeria, on burden of undiagnosed hypertension with participants recruited via a multi-stage sampling method. An interviewer-administered questionnaire was used, and standardized instruments were applied to obtain, process and analyze the data. Tests of association between the independent variables and outcome were conducted using logistic regression. P-value of Results: A total of 380 adults participated in the study. The mean age was 44.2 years. The prevalence of undiagnosed hypertension was 35.8%. Logistic regression revealed that age, with the respondents in the age groups 26 - 35 years (OR = 10.647, 1.910 - 59.345, p-value = 0.007), 36 - 45 (OR = 3.680, 1.263 - 10.723, p-value = 0.017), 46 - 55 years (OR = 2.737, 1.114 - 6.727, p-value = 0.039), 56 - 65 years old (OR = 3.384, 1.610 - 7.115, p-value = 0.001);and being married (OR = 3.846, 1.118 - 13.233, p-value = 0.033), were independent risk factors for undiagnosed hypertension. Conclusion: The prevalence of undiagnosed hypertension in the rural population of South-East Nigeria is high. Younger age (26 - 35 years) had the highest odds of risk for occurrence of hypertension. Also being married was identified as a risk factor for undiagnosed hypertension.
