Acupuncture originated in China and is an important component of traditional Chinese medicine.The elucidation of its anesthetic mechanisms,the medical needs of Western societies,as well as the proven therapeutic effects of this technique have made it the most widely used medical practice in the world.The concept of evidence-based medicine has been introduced in acupuncture research for technical improvement and the production of high-quality clinical evidence.Upon reviewing the acupuncture clinical research over the past 10years,we found the gradually increasing range of diseases that it benefits,from pain management to neoplasms and genitourinary,digestive,and mental disorders.Acupuncture is included in a number of international guidelines for clinical practice.However,high-quality clinical evidence to support the efficacy and effectiveness of acupuncture is lacking as a result of multiple exceptional clinical trials conducted by Western researchers that have yielded antagonistic results.Future clinical research should focus on exploring objective evaluation methods for studying the therapeutic effects of acupuncture. 展开更多

The editorial board of Acupuncture and Herbal Medicine recently convened an academic seminar with a focus on studies regarding the mechanisms mediating acupuncture efficacy and moxibustion action inspired by the 2021 Nobel Prize in Physiology or Medicine.Specifically,Professor Bailong Xiao introduced the Nobel Prize for research on the mechanically activated Piezo ion channel,evaluating the structure of the Piezo channel and its physiological and pathological functions,and proposed a possible role for the Piezo channel in acupuncture mechanical stimulation.Professor Michael Xi Zhu introduced the discovery of the transient receptor potential(TRP)family,reporting that the therapeutic effects of Chinese medicine and acupuncture may be achieved via the TRP family,and that information regarding associations between the meridian and lymphatic systems may have important research and medical value.In addition,Professor Tianle Xu reviewed the history of ion channel research,particularly the physiological and pharmacological effects of non-classical ion channels(eg,the acid sensing ion channel family)and pointed out that the characterization and neural circuits of acupuncture deqi manipulation are important for elucidating the mechanisms of acupuncture actions.Professor Yongming Li similarly proposed that the 2021 Nobel Prize may open the door to disclosing the histological basis of acupuncture and moxibustion and analyzing the main scientific concerns regarding the clinical translation of acupuncture and moxibustion from basic to translational research.Finally,Professor Yi Guo summarized the study progress of the acupoint microenvironment induced by acupuncture over the course of nearly 30 years and put forward the hypothesis that acupuncture may initiate the physical-chemical coupling network by activating ion channel receptors in acupoints via physical and mechanical stimulation.Therefore,we conclude that a primary achievement of the 2021 Nobel Prize in Physiology or Medicine is in helping interpret how acupuncture and moxibustion adjust homeostasis(ie,by activating mechanical and thermal sensation),which is conducive to validating and promoting the clinical efficacy of acupuncture modalities. 展开更多

BACKGROUND Few studies have investigated low-frequency electrical stimulation combined with tri-tongue acupuncture for the treatment of post-stroke dysarthria.This randomized clinical study assessed the correlation between the clinical efficacy of low-frequency electrical stimulation combined with tri-tongue acupuncture in patients with post-stroke dysarthria.AIM To investigate the clinical effects of tri-tongue acupuncture combined with lowfrequency electrical stimulation for treating post-stroke dysarthria.METHODS Ninety patients with post-stroke dysarthria,who were admitted to our hospital from December 2019 to June 2021,were selected and equally divided into two groups(n=45/group)according to the random number table method.Tri-tongue acupuncture was administered in the control group.The treatment group received both tri-tongue acupuncture and low-frequency electrical stimulation.The clinical efficacy,Western Aphasia Battery(WAB)score,general quality of life inventory(GQOLI-74)score,Frenchay Dysarthria Assessment score,and speech function grades were compared and analyzed between both groups.RESULTS The overall efficacy in the treatment group was better than that in the control group(P<0.05).Before treatment,the WAB,Frenchay Dysarthria Assessment,or GQOLI-74 scores(P>0.05)did not differ between the groups.After therapy,the WAB,Frenchay Dysarthria Assessment,and GQOLI-74 scores in both groups increased significantly(P<0.05),and the treatment group exhibited a significantly greater increase than that of the controls(P<0.05).Moreover,the classification of speech function did not differ between the two groups before treatment(P>0.05),whereas significant improvements were observed in both groups after treatment(P<0.05).The degree of improvement in the treatment group was greater than that in the control group(P<0.05).CONCLUSION Low-frequency electrical stimulation,in conjunction with tri-tongue acupuncture,exhibits a good clinical effect on post-stroke dysarthria. 展开更多

Acupuncture is an effective treatment for ischemic stroke(IS)and plays a key role in neurological rehabilitation after IS.Acupuncture can improve the clinical symptoms of various complications after IS,including motor dysfunction,swallowing disorders,speech disorders,cognitive impairment,depression,insomnia,and fatigue.However,the mechanisms underlying the effects of acupuncture in IS remain unclear.Available evidence suggests that acupuncture may exert neuroprotective effects through neuroplasticity(neurogenesis and synaptogenesis),angiogenesis,cell proliferation and apoptosis,and regulation of oxidative stress,inflammation,and immunity.Further studies should be conducted to improve the high-quality evidence-based system of acupuncture intervention for IS,by focusing on the clinical and basic research design,increasing the sample size,standardizing and quantifying the standards of acupuncture operations,using multidisciplinary techniques and methods to systematically explore the key targets of acupuncture intervention for IS,and reveal the efficacy and mechanism of acupuncture in the treatment of IS. 展开更多

Acupuncture and moxibustion treating lower urinary tract symptoms due to benign prostatic hyperplasia:a systematic review and network meta-analysis 被引量:3

Objective: Lower urinary tract symptoms(LUTS) caused by benign prostatic hyperplasia(BPH) affect the quality of life of elderly individuals. Acupuncture and moxibustion are used in the clinic in China for improving LUTS symptoms due to BPH. However,there is no evidence to suggest which is the best option. We compared the efficacy of acupuncture and moxibustion to provide evidence for clinical decision-making.Methods: PubMed, Embase, Cochrane Library, Web of Science, China National Knowledge Infrastructure, Wan Fang Data, and VIP databases were searched from inception to July 2020 to identify the randomized controlled trials(RCTs) of acupuncture and moxibustion for LUTS due to BPH. Two researchers filtered studies and extracted the information independently. This study conducted a network meta-analysis using the Bayesian random method. The interventions ranking was evaluated using the surface under the cumulative ranking curve(SUCRA).Results: We finally included 40 studies comprising 10 treating therapies and 3,655 patients with LUTS caused by BPH. In terms of the International Prostate Symptom Score, maximum urinary flow rate, and quality of life, electroacupuncture(EA) [MD =-3.6,95% credible interval(CrI)(-5.5,-1.8), very low certainty of evidence;MD = 2.2, 95% CrI(1.1, 3.3), low certainty of evidence;MD =-1.3, 95% CrI(-2.2,-0.43), very low certainty of the evidence] may be consistently the optimal treatment compared with other interventions, with SUCRA values of 84%, 81%, and 89%, respectively.Conclusions: Of all treatments, EA may have the best efficacy with fewer adverse events for LUTS due to BPH. The quality of evidence supporting this result is low to very low certainty of the evidence due to the limitations of primary studies;thus, more highquality RCTs are needed for further evidence. 展开更多

Efficacy of activating blood and resolving stasis therapy for IgA nephropathy:a systematic review and meta-analysis

Background:To systematically evaluate the efficacy and safety of activating blood and resolving stasis in patients with IgA nephropathy.Methods:From inception to May 2022,databases including PubMed,Embase,the Cochrane Library,Web of Science,WanFang database,Chinese Biomedical Database,VIP,and China National Knowledge Infrastructure were searched for randomized controlled trials about enhancing blood circulation and removing stasis for IgA nephropathy.For the articles that satisfied the requirements,quality assessment and meta-analysis were done.Results:Seventeen randomized controlled trials with a total of 1653 patients were included.Meta-analysis showed that activating blood and resolving stasis could increase therapeutic effectiveness(risk ratio(RR)=-0.47,95%confidence interval(CI)(-0.37,-0.2),P=0.0006)and decrease levels of serum creatinine(RR=-0.47,95%CI(-0.37,-0.2),P=0.0006),urea nitrogen(RR=0.85,95%CI(1.44,0.26),P=0.005),24-hour urinary protein quantification(RR=1.6,95%CI(2.44,0.95).P=0.00001),and urine red blood cell count(RR=1.7,95%CI(2.57,0.82),P=0.0001).There was no significant difference between the two groups in terms of security(RR=0.6,95%CI(0.36,1.01),P=0.05).Conclusion:Western medicine combined activating blood and resolving stasis is more efficient than Western medicine therapy alone in treating IgA nephropathy,but it still needs to be supported by additional large-scale,multi-center randomized controlled clinical trials due to the poor quality of the included trials. 展开更多

Background:Aphasia is one of the common complications of stroke.Speech and language therapy is a conventional treatment for aphasia following stroke.However,rehabilitation often remains unsatisfactory after speech and language therapy alone.Acupuncture may be effective for aphasia after stroke.We designed this systematic review to assess the efficacy and safety of acupuncture as an adjunct to speech and language therapy for aphasia following ischemic stroke.Methods:An extensive search will be performed in databases including Cochrane Central Register of Controlled Trials,PubMed,Web of Science,EMBASE,China National Knowledge Infrastructure,Sinomed,Wei Pu Date and Wan Fang Data.No limitation will be set in language,publication date or status of the study.The primary outcomes include scores of Western Aphasia Battery and Aphasia Quotient.The secondary outcomes include Scales of Clinical Rehabilitation Research Center Aphasia Examination,Communicative Abilities in Daily Living,Aphasia Battery of Chinese,Chinese Functional Communication Profile,adverse reactions,etc.Results:All of the data will be processed with Review Manager software.Cochrane‘risk of bias’tool will be used to assess the risk of bias.Conclusion:This study will contribute to the selection of appropriate therapies for aphasia after ischemic stroke and the development of clinical guidelines on this topic. 展开更多

Small cell lung cancer(SCLC)has a high degree of malignancy and rapid progression,and most of the patients are in the extensive stage when they are diagnosed.The traditional treatment of SCLC is mainly systematic chemotherapy combined with radiotherapy,but there has been no significant progress for many years.The emergence of immunotherapy represented by immune checkpoint inhibitors(ICIs)has changed the treatment pattern of SCLC.Some studies have confirmed the effect of ICIs monotherapy.However,the disadvantages of ICIs monotherapy such as low responsiveness,drug resistance,and adverse reactions limited the clinical application.The combined treatment of immune checkpoint inhibitors was newly developing treatment models.Clinical studies have found that ICIs combined with standard chemotherapy can significantly improve the survival of patients with extensive small cell lung cancer without increasing adverse reactions.Based on the synergistic effect of immunotherapy combined with chemotherapy,this paper reviews the current progress,deficiency and future exploration direction of first-line immunotherapy for extensive small cell lung cancer. 展开更多

Objective:To explore the main acupoint prescription and mechanism of acupuncture in treating Functional dyspepsia(FD)by analyzing the clinical randomized controlled trial literature on acupoints and targets of acupuncture in the treatment of FD combined with association rule mining and bioinformatics/network pharmacology methods.Methods:Search relevant clinical randomized controlled trial literature on acupoints and therapeutic targets of acupuncture in the treatment of FD from eight databases from their inception to June 18th,2022.We obtained acupuncture selection points,meridian,and therapeutic targets and established acupoints database for acupuncture treatment of FD.FD-related targets were collected from GeneCards,DisGeNET,OMIM,and DrugBank databases.We obtained the potential targets of acupuncture on FD by taking the intersection of acupuncture for FD therapeutic targets and FD-related targets.We performed the protein-protein interaction network,Gene Ontology(GO)analysis,and Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway enrichment analysis.Results:In this study,26 Randomized Controlled Trials related to acupuncture treatment of FD were retrieved.We obtained 29 acupoints,17 acupuncture for FD therapeutic targets,and 10 intersection targets.Tumor necrosis factor(TNF),Interleukin(IL)-1,and Neuropeptide Y(NPY)are key targets of acupuncture in the treatment of FD.Conclusion:ST36-CV12-PC6-LR3 is the main acupoint prescription for FD.Acupuncture may affect Neuroactive ligand-receptor interaction,Gastric acid secretion,and IL-17,TNF signaling pathways by regulating related key targets,and play a synergistic role in the treatment of FD by inhibiting gastric acid secretion,alleviating inflammatory response,regulating the brain-gut axis,improving mood and other aspects. 展开更多

BACKGROUND Synaptophysin plays a key role in synaptic development and plasticity of neurons and is closely related to the cognitive process of Alzheimer’s disease(AD)patients.Exogenous neural stem cells(NSCs)improve the damaged nerve function.The effects of Sanjiao acupuncture on cognitive impairment may be related to the regulation of the NSC microenvironment.AIM To explore the anti-dementia mechanism of acupuncture by regulating the NSC microenvironment.METHODS NSCs were isolated from pregnant senescence-accelerated mouse resistant 1(SAMR1)mice,labeled with BrdU,and injected into the hippocampus of senescence-accelerated mouse prone 8(SAMP8)mice.Eight-month-old senescence-accelerated mice(SAM)were randomly divided into six groups:SAMR1(RC),SAMP8(PC),sham transplantation(PS),NSC transplantation(PT),NSC transplantation with acupuncture(PTA),and NSC transplantation with nonacupoint acupuncture(PTN).Morris water maze test was used to study the learning and memory ability of mice after NSC transplantation.Hematoxylin-eosin staining and immunofluorescence were used to observe the histopathological changes and NSC proliferation in mice.A co-culture model of hippocampal slices and NSCs was established in vitro,and the synaptophysin expression in the hippocampal microenvironment of mice was observed by flow cytometry after acupuncture treatment.RESULTS Morris water maze test showed significant cognitive impairment of learning and memory in 8-mo-old SAMP8,which improved in all the NSC transplantation groups.The behavioral change in the PTA group was stronger than those in the other two groups(P<0.05).Histopathologically,the hippocampal structure was clear,the cell arrangement was dense and orderly,and the necrosis of cells in CA1 and CA3 areas was significantly reduced in the PTA group when compared with the PC group.The BrdU-positive proliferating cells were found in NSC hippocampal transplantation groups,and the number increased significantly in the PTA group than in the PT and PTN groups(P<0.05).Flow cytometry showed that after co-culture of NSCs with hippocampal slices in vitro,the synaptophysin expression in the PC group decreased in comparison to the RC group,that in PT,PTA,and PTN groups increased as compared to the PC group,and that in the PTA group increased significantly as compared to the PTN group with acupointrelated specificity(P<0.05).CONCLUSION Acupuncture may promote nerve regeneration and synaptogenesis in SAMP8 mice by regulating the microenvironment of NSC transplantation to improve the nerve activity and promote the recovery of AD-damaged cells. 展开更多

Recent studies have demonstrated that neuroplasticity,such as synaptic plasticity and neurogenesis,exists throughout the normal lifespan but declines with age and is significantly impaired in individuals with Alzheimer’s disease.Hence,promoting neuroplasticity may represent an effective strategy with which Alzheimer’s disease can be alleviated.Due to their significant ability to self-renew,differentiate,and migrate,neural stem cells play an essential role in reversing synaptic and neuronal damage,reducing the pathology of Alzheimer’s disease,including amyloid-β,tau protein,and neuroinflammation,and secreting neurotrophic factors and growth factors that are related to plasticity.These events can promote synaptic plasticity and neurogenesis to repair the microenvironment of the mammalian brain.Consequently,neural stem cells are considered to represent a potential regenerative therapy with which to improve Alzheimer’s disease and other neurodegenerative diseases.In this review,we discuss how neural stem cells regulate neuroplasticity and optimize their effects to enhance their potential for treating Alzheimer’s disease in the clinic. 展开更多

Objective:Moxibustion has been widely used in the prevention and treatment of COVID-19.However,there is no systematic review of current topics and clinical findings on moxibustion for COVID-19.We conducted this scoping review to systematically summarize and analyze the themes and findings of published articles,and to provide an overview of current knowledge and practice of moxibustion for COVID-19.Methods:PubMed,Embase,Cochrane Library,Web of Science,China National Knowledge Infrastructure,SinoMed,Wan Fang Data,and VIP databases were searched from inception until April 2022.The relevant data were presented through bar graphs,structured tables,and figures along with descriptive statistics and analysis.This scoping review was conducted based on the PRISMA-ScR Checklist.Results:A total of 76 articles were reviewed:47 reviews,19 clinical research studies,seven systematic reviews(all were protocols),and three guidelines.All the studies were conducted by Chinese researchers and published from January 1,2020 to March 14,2022.The feasibility of moxibustion in the prevention and treatment of mild or moderate COVID-19 is based on the consensus of therapeutic mechanisms and effectiveness.The most adopted approach was the suspended and gentle moxibustion,and the most frequently applied or recommended acupoints were found to be ST36,CV8,CV6,CV4,CV12,GV14,BL13,LI4,ST25,and LR3.Conclusions:As a convenient and safe traditional Chinese medicine(TCM)therapy with its specific feature,moxibustion has been significantly effective at ameliorating mild or moderate symptoms among COVID-19 patients.Further large-scale,welldesigned research and international cooperation are still warranted in clinical evaluations of moxibustion. 展开更多

Background: The Xingnao Kaiqiao acupuncture method, founded by Academician ShiXuemin, has outstanding efficacy in the treatment of ischemic stroke and has been widelyused at home and abroad. However, after half a century of animal experiments and clinicalstudies, clearly and intuitively revealing its therapeutic mechanism is still a great challengefor researchers. Therefore, this experiment is based on the combination of medicine andengineering to study the immediate effects of the acupuncture method in treating patientsby recording the electroencephalography (EEG) activities of subjects during the acupunctureprocess and to further reveal the therapeutic mechanism at the brain level. Methods: Thistrial is an exploratory, prospective, single-arm interventional study involving a total ofpatients with ischemic stroke. Physicians will record EEG data from patients duringacupuncture as the primary outcome indicator. After pre-processing the EEG data,researchers will use various methods to analyze the immediate effects of acupuncture toobtain brain effectiveness. Deep learning will then be used to identify acupoint stimuli andreceive correspondence between the acupuncture effect and the brain’s internal state.National Institutes of Health Stroke Scale score before and after the acupuncture process willbe used as the secondary outcome indicator. Conclusion: This is the first study protocol toapply dynamic changes in EEG to explore a range of mechanisms of action of acupuncture inthe treatment of ischemic stroke. We propose a method to analyze EEG signals ofacupuncture patients. The deep learning model will be applied for supervised training toobtain the compelling relationship between the acupuncture method and internal brainstates, providing a new prospect for the digitization of traditional Chinese medicine. 展开更多

Objective:To systematically evaluate the efficacy and safety of Qianliexin capsule combined with western medicine in benign prostatic hyperplasia treatment.Methods:Randomized controlled trials of Qianliexin capsule combined with western medicine in the treatment of benign prostatic hyperplasia were searched from PubMed,the Cochrane Library,EMBASE,Web of Science,China National Knowledge Infrastructure,VIP,Wanfang and China Biomedical Literature Databases from establishment of the database to February 2021.Meta-analysis was performed using RevMan5.4 software after two researchers independently screened the literatures,extracted the data and evaluated the bias risks in the included studies according to the Cochrane Collaboration System Evaluator Manual.Results:6 randomized controlled trials involving 1060 patients were included.Meta-analysis results showed that the treatment group was better than the control group in total effective rate(Relative Risk=1.19,95%Confidence Interval(1.10,1.28),P<0.00001),International Prostate Symptom Score(Mean Difference=-2.99,95%confidence interval(-4.00,-1.99),P<0.00001),prostate volume(Mean Difference=-3.77,95%Confidence Interval(-4.67,-2.86),P<0.00001),postvoid residual urine volume(Mean Difference=-5.96,95%Confidence Interval(-8.15,-3.77),P<0.00001)and maximum urine flow rate(Mean Difference=3.45,95%Confidence Interval(2.53,4.37),P<0.00001).While there was no significant difference in the adverse effects rate(Relative Risk=0.71,95%Confidence Interval(0.33,1.56),P=0.40)between the treatment group and the control group.Conclusion:Qianliexin capsule combined with western medicine in the treatment of benign prostatic hyperplasia showed good efficacy and high safety.However,more high-quality and large-sample studies are needed to confirm the above conclusions. 展开更多
