Effectiveness and safety of electroacupuncture analgesia in controlling intraoperative pain and hemodynamics during total thyroidectomy:A randomized controlled trial

Objective:To evaluate the effectiveness and safety of electroacupuncture in conjunction with additional medications in providing analgesia and stabilizing hemodynamic parameters during total thyroidectomy.Methods:This randomized controlled trial included 100 patients who underwent a total thyroidectomy between October 2022 and October 2023 at the Vietnam National Hospital of Acupuncture.The patients were randomized into two groups.The electroacupuncture analgesia(EA)group received EA stimulation at five acupuncture points:Hegu(LI 4),Neiguan(PC 6),Shuitu(ST 10),Quepen(ST 12),and Yifeng(SJ 17),while the control group received a bilateral superficial cervical plexus block.Primary outcomes included the level of analgesia and perioperative vital signs in both groups.Additionally,pain thresholds and serum b-endorphin levels were measured before and after electroacupuncture in the EA group.Results:Complete analgesia(Level A)was attained in 86%and 76%of the patients in the EA and control groups,respectively,with no significant difference between the two groups(P=1.00).In the EA group,the mean pain threshold after receiving EA doubled(648.7(77.4)g/s vs.305.3(45.3)g/s,P<.001),and the mean serum b-endorphin level increased by approximately 13.5 pg/mL(P<.001).All patients remained hemodynamically stable throughout the surgery.Conclusion:EA,in conjunction with additional medications that stimulate five acupuncture points,LI 4,PC 6,ST 10,ST 12,and SJ 17,was well tolerated and effectively maintained a suitable level of analgesia and hemodynamic stability during total thyroidectomy.

A Brief Introduction to Viet Nam National Hospital of Acupuncture

The Viet Nam National Hospital of Acupuncture （VNHA） was founded on April 24, 1982. After 25 years of development, VNHA now has 450 hospital beds, including 300 hospital beds for inpatients and 150 for outpatients, and 19 departments, all of which strive to detect and cure diseases for patients in the whole country. VNHA has 350 employees, including one professor of acupuncture （acup）, 7 PhoDs of acup, 24 Masters of acup, 42 doctors specialized grade Ⅰ, 2 doctors specialized grade Ⅱ, 5 university-degree pharmacists, 60 doctors of acup, engineers and 5 university graduates from other areas of expertise, and 60 technicians and nurses, etc.

高效液相色谱指纹图谱结合化学模式识别评价龙葵质量

目的建立评价龙葵质量的高效液相色谱(HPLC)指纹图谱和化学模式识别方法。方法色谱柱为Waters C18柱(150 mm×4.6 mm,5μm),流动相为乙腈-0.3%磷酸溶液(梯度洗脱),流速为1.0 mL/min,检测波长为205 nm,柱温为26℃,进样量为10μL。建立13批样品的HPLC叠加指纹图谱,采用中药色谱指纹图谱相似度评价系统(2004A版)进行相似度评价,确定并指认共有峰,同时测定指认成分的含量。通过层次聚类分析(HCA)、主成分分析(PCA)和正交偏最小二乘判别分析(OPLS-DA)对不同批次样品进行聚类。结果13批样品共标定了9个共有峰,相似度为0.701~0.984;指认出2个成分,分别为澳洲茄碱(6号峰)和澳洲茄边碱(7号峰),其质量浓度均在3.9~500.0μg/mL范围内与峰面积线性关系良好(R^(2)≥0.9950,n=5)。越南产少花龙葵中澳洲茄碱含量(895.57 mg/kg)远高于国内含量最高的广西南宁产少花龙葵(68.05 mg/kg),但越南产少花龙葵中澳洲茄边碱含量(68.05 mg/kg)仅为广西南宁产少花龙葵(867.42 mg/kg)的7.85%。HCA,PCA,OPLS-DA结果显示,13批龙葵药材按基原被分为2类,1类为龙葵,另1类为少花龙葵。结论该方法操作简单、结果准确,能快速、科学、准确地评价龙葵的质量。

Effectiveness of bee venom acupuncture for patients suffering from periarthritis humeroscapularis

OBJECTIVE:To evaluate the efficacy of bee venom acupuncture in humeroscapularis(PHS)patients.METHODS:One hundred and twenty patients diagnosed with PHS were assigned into four groups:BV1(0.01 mg/kg),BV2(0.005 mg/kg),BV3(0.0025 mg/kg),and control group(vitamin B1 plus novocain 3%injection)with 15 d of treatment.The outcomes of the study including visual analogue scale(VAS)score andβ-endorphin,inflammatory cytokines including interleukin-10(IL-10),IL-1βand tumor necrosis factorα(TNF-α)and shoulder function score were assessed at baseline,after 10 and 15 d of treatment.RESULTS:All four groups reported statistically significant improvement in VAS score,motion range,and shoulder function score(P<0.01),only the BV3 group showed significant increase of anti-inflammatory(IL-10)and decrease of pro-inflammatory(IL-1β,TNF-α)cytokines after treatment(P<0.05).The BV3 group presented a significant difference between all outcomes compared to the control and other groups.CONCLUSION:BV3 groups showed better recovery including reduced pain,improved motor function and normalized inflammatory cytokines than current therapy used in Vietnam and other groups.

Effectiveness on post-stroke hemiplegia in patients:electroacupuncture plus cycling vs electroacupuncture alone

OBJECTIVE:To evaluate the effectiveness of cycling in combination with electroacupuncture in treatment of poststroke hemiplegia patients at National Hospital of Acupuncture,Vietnam.METHODS:The study was designed as a single-centre,outcome-assessor-blinded parallel randomised controlled trial with 120 post-stroke hemiplegia patients randomly assigned into two groups:electroacupuncture plus cycling(CT group) and electroacupuncture(AT group).Patients were assessed before and after the treatment(using muscle grading,modified Rankin,Barthel,Orgorozo scores and electromyography).Statistical Man–Whitney U test,and Fisher’s exact tests were used to compare between CT and AT groups.RESULTS:The results reported statistically significant improvement in motor function in patients suffering from hemiplegia following ischemic stroke in both CT and AT groups.Patients in CT group experienced a greater improvement compared to those in AT group including better muscle contraction(increased frequency and amplitude of electromyography and increased muscle grading scale);increased recovery(Orgogozo scale),increased independency(Barthel scale) and decreased disability(Modified Rankin scale)(P < 0.01).CONCLUSIONS:Combination with cycling training significantly improves the recovery of post-stroke patients treated with electroacupuncture.

龙葵不同药用部位及药效成分的抗肿瘤机理研究进展

中药龙葵在我国各地均有分布,药材资源丰富,整株入药,可散瘀消肿、清热解毒。龙葵在传统民间医学中被用来治疗不同的癌症,且大量的药理及临床研究工作表明龙葵尤其是鲜龙葵果具有良好的抗肿瘤作用,极具开发利用前景。文章从龙葵的不同药用部位或所含化学成分出发,综述近年来龙葵抗肿瘤的研究机理,为进一步开展全面而深入的研究工作提供思路。
