Celiac disease:Management of persistent symptoms in patients on a gluten-free diet 被引量:2

AIM:To investigate all patients referred to our center with non-responsive celiac disease (NRCD),to establish a cause for their continued symptoms.METHODS:We assessed all patients referred to our center with non-responsive celiac disease over an 18-mo period.These individuals were investigated to establish the eitiology of their continued symptoms.The patients were first seen in clinic where a thorough history and examination were performed with routine blood work including tissue transglutaminase antibody measurement.They were also referred to a specialist gastroenterology dietician to try to identift any lapses in the diet and sources of hidden gluten ingestion.A repeat small intestinal biopsy was also performed and compared to biopsies from the referring hospital where possible.Colonoscopy,lactulose hydrogen breath testing,pancreolauryl testing and computed tomography scan of the abdomen were undertaken if the symptoms persisted.Their clinical progress was followed over a minimum of 2 years.RESULTS:One hundred and twelve consecutive patients were referred with NRCD.Twelve were found not to have celiac disease (CD).Of the remaining 100 patients,45% were not adequately adhering to a strict gluten-free diet,with 24 (53%) found to be inadvertently ingesting gluten,and 21 (47%) admitting noncompliance.Microscopic colitis was diagnosed in 12% and small bowel bacterial overgrowth in 9%.Refractory CD was diagnosed in 9%.Three of these were diagnosed with intestinal lymphoma.After 2 years,78 patients remained well,eight had continuing symptoms,and four had died.CONCLUSION:In individuals with NRCD,a remediable cause can be found in 90%:with continued gluten ingestion as the leading cause.We propose an algorithm for investigation. 展开更多

Objective: As a needs assessment for intervention, quantitative and qualitative methods were used to examine attitude, subjective norms, perceived behavioral control, intention, knowledge, and weight control status related to physical activity in female university students within the Theory of Planned Behavior (TPB). Methods: A two-phase mixed method design was used. In Phase I, 362 students participated in an online survey, and in Phase II, 33 students participated in five focus group discussions. Ages of participants ranged from 18 to 45 years old, with 18 - 25 year olds making up over 74% of the sample. Results: Attitude, subjective norms, and perceived behavioral control, along with weight control status of trying to lose weight, were found to be significant predictors of intention to follow physical activity recommendations, which in turn were the strongest predictor of physical activity. Knowledge was not found to be significant. Group discussions revealed barriers to meeting physical activity recommendations, which included lack of companionship and social support, lack of motivation, time and cost restrictions, and lack of privacy in the gym. Social norms exerted both positive and negative influences. Conclusion: The mixed method approach provided a deeper insight into the influential factors pertaining to physical activity among female students, and results could be used in further research to develop effective interventions. 展开更多

Fidget spinners have been marketed as repetitive motion devices that improve attention and motor performance, and as such, they have become quite appealing to individuals with Attention Deficit Hyperactive Disorder (ADHD). To date, no studies have explored changes in brain activity that may occur due to fidgeting in ADHD. Our aim was to use functional Near-Infrared Spectroscopy (fNIRS) to examine the prefrontal cortex (PFC) during the performance of a standardized fine motor skills test after using a fidget spinner. Eight right-handed adults with ADHD and eight age and gender matched adults without ADHD (4F/4M, 4 control/4 fidget) performed the Purdue Pegboard Test (PPT) while their brain oxygenation was monitored using fNIRS. Relative neural efficiency (RNE) and involvement (RNI) were calculated and analyzed for all subtasks of PPT including the less cognitively demanding fine motor subtasks and more complex assembly tasks. The fidget spinner improved both task performance and RNE in the ADHD group but not the non-ADHD group for the less cognitively demanding subtasks. Our results indicate Fidget spinners may improve both relative neural efficiency and fine motor performance in adults with ADHD for less cognitively demanding tasks. 展开更多

AIM:To evaluate the effects of soy supplementation on insulin resistance,fatty liver and alanine aminotransferase(ALT) levels in non-diabetic patients with chronic hepatitis C(CHC).METHODS:In a prospective,randomized and singleblinded clinical trial,we compared patients with CHC who had casein as a supplement(n = 80)(control group),with patients who consumed a soy supplement diet(n = 80) [intervention group(IG)].Both groups received 32 g/d of protein for 12 wk.RESULTS:Patients' baseline features showed that 48.1% were overweight,43.7% had abdominal fat accumulation,34.7% had hepatic steatosis and 36.3% had an homeostasis model assessment index of insulin resistance(HOMA-IR) ≥ 3.0.Descriptive analysis showed that protein supplementation diet reduced hepatic steatosis in both groups;however,significant reductions in ALT levels occurred in the soy group.Multiple regression modeling indicated that in the presence of severe fibrosis(F3/F4),g glutamyl transferase elevation and high density lipoprotein(HDL) reduction,the intervention group had 75% less chance of developing hepatic steatosis(OR= 0.25;95% CI:0.06-0.82) and 55% less chance of presenting with an ALT level ≥ 1.5 × the upper limit of normal(ULN)(OR = 0.45,95% CI:0.22-0.89).Soy treatment did not have any effect on insulin resistance(OR = 1.92;95% CI:0.80-4.83),which might be attributed to the fact that the HOMAIR values at baseline in most of our patients were in the normal range.Advanced hepatic fibrosis,an ALT level > 1.5 × ULN and visceral fat were predictors of an HOMA-IR ≥ 3.The IG group had a reduced risk of an ALT level > 1.5 × ULN.An HOMA-IR ≥ 3.0 and HDL < 35 mg/dL were also risk factors for increased ALT.CONCLUSION:Soy supplementation decreased ALT levels and thus may improve liver inflammation in hepatitis C virus(HCV) patients;it also reduced hepatic steatosis in a subgroup of patients but did not change insulin resistance.It should be considered in the nutritional care of HCV patients. 展开更多

Quality of Canola Oil Obtained by Conventional and Supercritical Fluid Extraction

Quality of canola oil obtained by the supercritical fluid extraction (SFE), using CO2 with ethanol as a co-solvent, was evaluated and compared to that of the conventionally-obtained oils using either n-hexane or chloroform methanol mixture. Physical characteristics, chemical properties, fatty acid composition and phenolic profile of oils were investigated. The SFE oil showed significantly lower melting point, peroxide value (PV) and higher free fatty acids (FFAs) and iodine value (IV) than the n-hexane-extracted one. There were no significant differences in the fatty acid composition of different oils. The SFE oil showed significantly higher phenolic content (35.91, 10.15, 3.16, 0.32 and47.48mg/g of sinapic acid, sinapine, sinapoyl glucose, canolol and total phenolics) as compared to 0.08, 0.70, 0.88, 0.45 and0.71mg/g, respectively in the n-hexane-extracted oil. These results indicate the superiority of SFE and advocate its use for the extraction of highly stable and functional canola oil for further health and nutraceutical uses. The present results have an industrial and technological relevance as SFE could be competitive with the traditional extraction techniques providing an environmental approach and enhancing the obtained oil quality and stability. After recovery of the initial installation costs, SFE could be more economic than conventional extraction. However, further economical studies are needed to validate this last conclusion. 展开更多

Pancreatic cancer(PaCa)is one of the most aggressive and lethal malignancies with rapid progression and poor prognosis with the 5-year survival rate remaining less than 5%because approximately 80%of PaCa patients are diagnosed at an advanced stage and lose the chance of curative resection.1 Accumulated evidence has revealed that noncoding RNAs(ncRNAs)are involved in the pathogenesis of PaCa and serve as potential biomarkers for PaCa.2,3 Exosomal lncRNAs also play a vital role in tumorigenesis and some exosomal lncRNAs have been reported to accelerate tumor progression.4 In this study,we first employed RNA-seq of PaCa plasma samples to identify significantly differentially expressed lncRNAs in PaCa patients and revealed that LINC01268 was up-regulated in PaCa tumor tissues and plasma.LINC01268 is an emerging lncRNA associated with glioma malignancy grade.5 Thus.we further analyzed the malignant biological behaviors of this lncRNA in PaCa and found that LINC01268 promoted cell proliferation,migration,and invasion as an oncogene,which promoted epithelial–mesenchymal transition(EMT)in PaCa via the miR-217-KIF2A-PI3K/AKT axis. 展开更多

We previously demonstrated that endogenous phosphatidic acid(PA)promotes liver regeneration after acetaminophen(APAP)hepatotoxicity.Here,we hypothesized that exogenous PA is also beneficial.To test that,we treated mice with a toxic APAP dose at 0 h,followed by PA or vehicle(Veh)posttreatment.We then collected blood and liver at 6,24,and 52 h.Post-treatment with PA 2 h after APAP protected against liver injury at 6 h,and the combination of PA and N-acetyl-L-cysteine(NAC)reduced injury more than NAC alone.Interestingly,PA did not affect canonical mechanisms of APAP toxicity.Instead,transcriptomics revealed that PA activated interleukin-6(IL-6)signaling in the liver.Consistent with that,serum IL-6 and hepatic signal transducer and activator of transcription 3(Stat3)phosphorylation increased in PA-treated mice.Furthermore,PA failed to protect against APAP in IL-6-deficient animals.Interestingly,IL-6 expression increased 18-fold in adipose tissue after PA,indicating that adipose is a source of PA-induced circulating IL-6.Surprisingly,however,exogenous PA did not alter regeneration,despite the importance of endogenous PA in liver repair,possibly due to its short half-life.These data demonstrate that exogenous PA is also beneficial in APAP toxicity and reinforce the protective effects of IL-6 in this model. 展开更多

Bacterial infection and osteogenic integration are the two main problems that cause severe complications after surgeries. In this study, the antibacterial and osteogenic properties were simultaneously introduced in biomaterials, where copper nanoparticles(Cu NPs) were generated by in situ reductions of Cu ions into a mussel-inspired hyperbranched polyglycerol(MI-h PG) coating via a simple dip-coating method.This hyperbranched polyglycerol with 10 % catechol groups’ modification presents excellent antifouling property, which could effectively reduce bacteria adhesion on the surface. In this work, polycaprolactone(PCL) electrospun fiber membrane was selected as the substrate, which is commonly used in biomedical implants in bone regeneration and cardiovascular stents because of its good biocompatibility and easy post-modification. The as-fabricated Cu NPs-incorporated PCL membrane [PCL-(MI-h PG)-Cu NPs]was confirmed with effective antibacterial performance via in vitro antibacterial tests against Staphylococcus aureus(S. aureus), Escherichia coli(E. coli), and multi-resistant E. coli. In addition, the in vitro results demonstrated that osteogenic property of PCL-(MI-h PG)-Cu NPs was realized by upregulating the osteoblast-related gene expressions and protein activity. This study shows that antibacterial and osteogenic properties can be balanced in a surface coating by introducing Cu NPs. 展开更多

Objective:Despite numerous experimental studies in the literature,there are few clinical trials regarding the effect of date palm pollen(DPP)supplementation on sexual function improvement.In the present study,we sought to evaluate the impact of DPP on female sexual function in Iranian non-menopausal women.Methods:Between October 2019 and December 2019,health centers in the city of Khalkhal,volunteers meeting the inclusion criteria were recruited in randomized clinical trials.Sixty-eight women were randomly stratified and assigned to one of the two study groups:placebo group(n=35)and palm pollen group(n=35),and received a starch or palm pollen capsule(300 mg per day),respectively,for 35 d.The Female Sexual Function Index(FSFI)instrument was used to assess female sexual function.Results:After DPP supplementation,the increase in desire,lubrication,and the overall score,was statistically significant compared to the placebo group(P=0.002,P=0.000,and P=0.042;respectively);Whilst there was no significant differences in the remaining domains(arousal:P=0.763;orgasm:P=0.370;satisfaction:P=0.474;pain:P=0.259).There was a statistically significant positive correlation between the coitus and preintervention levels of desire(r=0.298,P=0.038),arousal(r=0.328,P=0.021),lubrication(r=0.361,P=0.011),orgasm(r=0.320,P=0.025),satisfaction(r=0.327,P=0.022),and overall scores(r=0.338,P=0.018).Conclusion:This study suggests that DPP(300 mg supplementation for 35 d),given to non-menopausal women,could improve the lubrication and desire domains of FSFI. 展开更多
