The association between adipokines and inflammatory periodontal diseases has been studied over the last two decades. This review was intended to explore the observation that periodontal therapy may lead to an improvement of adipokines in diabetic patients. In summary, substantial evidence suggests that diabetes is associated with increased prevalence, extent and severity of periodontitis. Numerous mechanisms have been elucidated to explain the impact of diabetes on the periodontium. However, current knowledge concerning the role of major adipokines indicates only some of their associations with the pathogenesis of periodontitis in type 2 diabetes. Conversely, treatment of periodontal disease and reduction of oral inflammation may have positive effects on the diabetic condition, although evidence for this remains somewhat equivocal. 展开更多

The effect of cold-light-activated bleaching treatment on enamel surfaces in vitro 被引量:5

This in vitrostudy aims to evaluate the crystal and surface microstructure of dental enamel after cold-light bleaching treatment. Twelve sound human premolars were cross-split into four specimens, namely, mesio-buccal (Group LP), disto-buccal (Group P), mesio-lingual (Group NP) and disto-lingual (Group L) specimens. These four groups were treated using the standard cold-light bleaching procedure, a bleaching agent, a peroxide-free bleaching agent and cold-light, respectively. Before and after treatment, all specimens were analyzed by high-resolution, micro-area X-ray diffraction and scanning electron microscopy. Using a spectrometer, tooth color of all specimens was measured before and after treatment. The phase of the enamel crystals was identified as hydroxyapatite and carbonated hydroxyapatite. After treatment, specimens in Groups LP and P showed significantly weaker X-ray diffraction peaks, significant reduction in crystal size and crystallinity, significant increase in L~ but decrease in a* and b*, and obvious alterations in the surface morphology. However, specimens in Groups NP and L did not show any significant changes. The cold-light bleaching treatment leads to demineralization in the enamel surface. The acidic peroxide-containing bleaching agent was the major cause of demineralization, whereas cold-light did not exhibit significant increase or decrease effect on this demineralization. 展开更多

Calcium channel blocker-induced gingival overgrowth (CCB-GO) is increasing in elderly patients who have been prescribed medication for hypertension for years. The purpose of the present study was to analyze the comprehensive protein expression levels of candidate biomarkers in gingival crevicular fluid (GCF) from CCB-GO patients. Eleven GO patients (10 males and one female, mean ± SD: age: 64.4 ± 14.0 years) who had been systemically prescribed CCBs, either amlodipine or nifedipine, for hypertension for at least 12 months were recruited. Before (baseline) and 4 weeks after initial periodontal treatments, subgingival plaque and GCF samples were taken from two sites per patient: sites affected by CCB-GO and chronic periodontitis. Measurement of clinical parameters and quantitative analysis of periodontopathic bacteria using real-time PCR were performed. Biomarkers/cytokines in GCF were examined using multiplex bead immunoassays. The Mann-Whitney U test was used to compare the collected data between groups. The correlations between pairs of biomarkers were assessed using the Spearman correlation relationship. Levels of two of the 14 biomarkers, interleukin (IL)-1β and transforming growth factor (TGF)- β, were significantly decreased in CCB-GO sites after initial periodontal therapy. The intragroup comparison at baseline showed that counts of Treponema denticola in the GO group were significantly higher than those in the chronic periodontitis group (P β and TGF-β in CCB-GO patients. These factors are involved in initiation and progression of GO as well as periodontitis. 展开更多

The relationship among periodontal condition, serum lipid, and electrocardiographic abnormalities in the elderly: A prospective cohort study

Periodontal disease is recognized as a risk factor for cardiovascular disease (CVD). Some studies have suggested that serum lipids might play a role in the association between periodontal disease and CVD. However, this hypothesis remains unproven. Present study evaluated the association between periodontal disease and CVD-related parameters including electrocardiographic (ECG) parameters and serum lipid levels in the elderly. This study was a prospective cohort study. A total of 107 subjects (57 males and 50 females) from Niigata City’s community-dwelling elderly (all aged 75 years old) who possessed at least 20 teeth and did not exhibit ECG abnormalities at the baseline underwent annual examinations for 5 years. Logistic regression analysis was used to assess the relationship between periodontal condition and the presence of ECG abnormalities, after adjusting for confounding factors including serum lipid levels. Logistic regression analysis showed that the subjects who exhibited a poor periodontal condition at the baseline had a 2.97 times greater risk of suffering from ECG abnormalities than the subjects who displayed a good periodontal condition at the baseline (p = 0.019). Furthermore, the subjects who displayed a poor periodontal condition and an unfavorable serum lipid profile at the baseline showed a significantly higher frequency of ECG abnormalities (63.6%) than those who exhibited a healthy periodontal condition and a favorable serum lipid profile (21.6%, p = 0.008) as well as those who displayed a healthy periodontal condition and an unfavorable serum lipid profile (21.4%, p = 0.010). As a conclusion, periodontal condition could be a marker of CVD, even in the healthy elderly. 展开更多
