Drinking water sources in areas heavily impacted by cholera were systematically mapped and tested for microbiological quality. The quality assessment was carried out in April 2023 during an ongoing cholera outbreak in the county. A total of 109 samples were collected and tested for thermotolerant coliforms and other in situ parameters. The finding revealed that more than 87% of the samples did not meet the World Health Organization (WHO) standard for thermotolerant coliforms;and 30% had turbidity values above the recommended threshold values. None of the 109 samples had any traceable residual chlorine. Following these findings, the county government implemented the targeted interventions which resulted in a positive impact in the fight against cholera. The WHO supported key interventions which included capacity building in water quality monitoring and prepositioning of critical WASH commodities to the cholera affected areas.展开更多

The World Health Organization (WHO) has set a target for each United Nations member state to reach by 2030. Each member state is required to reach by 2022, 2025 and 2030 at least 60%, 80% and 100%, respectively of basic level of service of the five elements which are water, sanitation, hygiene, waste management and environmental cleaning. Methods: This study aimed to evaluate and document the current state of basic water, sanitation, and hygiene services in all lower-level health care facilities in the Dar es Salaam region of Tanzania as of July 2022. A cross-sectional study was conducted in 99 public dispensaries in the Dar es Salaam region’s five councils: Ubungo, Kigamboni, Kinondoni and Temeke Municipalities, and Ilala City. The interviewee form and observational checklists were both digitalized using the Kobo tool software. The respondents were health care facility in-charges or nurse in-charges. Data were downloaded, validated, and imported to Stata version 15 for analysis. Results: The basic WASH level per JMP is far below the target in 2022. Each member state by 2022 is required to reach at least 60% of the basic level of service of each element. We found a low coverage of basic WASH in the 99 dispensaries included in this study. The basic WASH coverage was met in only 10 (10.1%) of the dispensaries, while the remaining 89 (89.9%) dispensaries fall on limited WASH services. Conclusion: This study revealed lower coverage of basic WASH services in dispensaries. An urgent need is required to improve the status of WASH in all the dispensaries and facilitate the provision of quality health care services, patient safety and reduce health care associated infections.展开更多

This type of fishing does not yet meet the local population’s demand for fish products. In response to this concern, a Community Agricultural Estate housing an integrated fish farm was set up in the commune of Bandafassi, in the village of Itato. Since its creation, this production unit has been faced with the problem of sourcing high-quality, low-cost, monosex male Oreochromis niloticus fry. In order to overcome this constraint, the present research focuses on the contribution of Garcinia kola to the productivity and economic profitability of the Itato farm. The aim of the research is to assess fish production in the experimental set-up and the production costs of tilapia in a controlled environment. The comparative study of the various production factors shows disparities only in the input factor, where scenarios 2 and 3 use additional products. These are 17-α-methytestosterone for scenario 2 and Garcinia kola for scenario 3. These products significantly interfere with fish production, with a fairly high mortality rate for scenario 2 (25% for two production cycles/ cohort2 (B5, B6, B7 and B8) treated with 17-α-methytestosterone). As for scenarios 1 and 3, mortalities are 5% with or without recourse to additional products (G. kola). In addition, average fish production for the three (03) scenarios is estimated at 28687.5 kg/2 cycles. It varies from one scenario to another, i.e. 30937.5 kg/2 cycles for scenarios 1 and 3 and 24187.5 kg/2 cycles for scenario 2. It is therefore higher in scenarios 1 and 3 than in scenario 2. This difference is due to the fairly large losses of individuals in scenario 2. Furthermore, the analysis of the profit and loss accounts for tilapia production varies from one scenario to another depending on the type of farm: 476 Franc CFA for scenario 1, 610 Franc CFA for scenario 2 and 472 Franc CFA for scenario 3 (F CFA = franc of the French Colonies of Africa). The Average operating income for all the fish farming units is 34,726,142 Franc CFA. The highest (41,638,075 Franc CFA) and lowest (29,281,075 Franc CFA) ERs were observed in scenarios 3 and 2 respectively. It was 33,259,275 Franc CFA for scenario 1. The difference between the NERs of the three scenarios is more or less significant in terms of results. The operating result (OR) is positive in all 3 scenarios in our study. However, the scenario 3 system generates a higher rate of return (the ratio between an income and the capital employed to obtain that income) (74%) than that generated by the scenario 1 system (69%). As for the scenario 2 system, it generates a lower financial return than the two previous systems (67%). Above all, this work made it possible to construct an approach that would make it possible to answer such a question by relying successively on various methods: a typology, according to the production factors involved in the operation of the Community Agricultural Estate fish farm.展开更多

The ability to understand the magnitude of the needs and highlight existing gaps is supported by improved data which is critical to informing effective policies, programming and funding decisions. The purpose of this report is to present the results of a research prioritization exercise on sexual and reproductive health and rights services in humanitarian settings for the WHO Africa region for the next three years. Methods: We adapted the Child Health and Nutrition Research Initiative method in three phases. Experts from the region participated in an online survey to identify key areas for research in sexual and reproductive health and rights. To identify potential areas for research, the experts ensured answerability, effectiveness, deliverability, equity and potential impact of the questions. The research areas they identified were reviewed by World Health Organization technical team from headquarters. In a meeting of 67 participants, the questions were subjected to further review and analysis. Using a modified for scoring criteria, the questions were scored and ranked to provide the top ten priority questions to address sexual and reproductive health and rights services in humanitarian settings. Results: A list of 21 priority research questions on sexual and reproductive health and rights services in humanitarian settings were scored and ranked. Top ten priorities research questions were identified. Those that scored highly by scoring 30 points out of the possible maximum of 30 include: “determining the prevalence and associated factors of unwanted pregnancies and abortions performed in emergency and humanitarian settings”, “evidence on gender-based violence in humanitarian situations and its associated factors” and “defining an optimal model for coordinating sexual and reproductive health and rights interventions and responses in crisis situations”. Conclusions: Top ten research priorities in sexual and reproductive health and rights services in humanitarian settings were identified. The priority research areas have the potential to identify the best areas for programming of services in humanitarian settings. It is our hope that the identified research areas will be prioritized to support programming of services in humanitarian settings based on scientific evidence.展开更多

To study whether irrigation and leaching practices and/or groundwater rise have contributed to the accumulation of Mg2+ on the cation exchange complex of these soils, historical changes in soil and groundwater quality were used as source data and the Visual MINTEQ model was applied to analyze the chemical composition of water and soils in the study area. The imposed irrigation regime and the leaching of light sierosem soils led to the dissolution and subsequent leaching of inherent gypsum and organic matter from the soil profile. Further, the domination of bicarbonate in the irrigation water promoted weathering of the carbonate minerals present as calcite. The higher concentrations of Mg2+ in comparison to Ca2+ in the irrigation water resulted in the replacement of Na+ by Mg2+ on the cation exchange complex. In the lower part of the command area, shallow groundwater has contributed to the accumulation of Na+ and to a large extent of Mg2+ on the cation exchange sites.展开更多

Two models were developed based on two different groups of stands. Both models indicate that the priority of a forest stand increases with an augment in the number of bushes and trees, and also with the mean diameter of trees. On the other hand, the priority is low with large number of pines and small trees. Stands represented by photos receive better priority values than those represented by virtual reality images. When the background of the judges (gender, country or occupation) was included into the model as additional predictors, no significant improvements are achieved.展开更多

Despite these documented benefits, however, there is a concern on the downward trend of manure use in agriculture in China. This paper examines factors contributing to this downward trend, with a particular focus on human excreta used in agriculture. Empirical analysis based on data from stratified random sampling of rural households in five provinces of China shows that about 85% of human excreta was still used as manure in agriculture in 2007 which was less than a decade ago when nearly all human excreta was used as manure. Econometric results suggest that income growth, rising population density and improvement in rural transportation significantly contribute to declining use of human excreta as manure in agriculture. These results imply that the current downward trend will continue given China's rising economic growth, urbanization and rural infrastructural improvement.展开更多

METHODS:The population-based cancer registry of Valdivia, included in the International Agency for Research on Cancer system, covers 356 396 residents of Valdivia Province,Southern Chile. We studied all SC cases entered in this Registry during 1998-2002 (529 cases).Population data came from the Chilean census(2002).Standardized incidence rates per 100 000 inhabitants(SIR)using the world population, cumulative risk of developing cancer before age 75, and rate ratios by sex, age, ethnicity and social factors were estimated.Relative survival(EdererⅡmethod) and age-standardized estimates(Brenner method) were calculated.Specific survival rates(Kaplan-Meier) were measured at 3 and 5 years and survival curves were analyzed with the Logrank and Breslow tests. Survival was studied in relation to demographics, clinical presentation, laboratory results and medical management of the cases. Those variables significantly associated with survival were later included in a Cox multivariate model. RESULTS:Between 1998 and 2002,529 primary gastric cancers occurred in Valdivia(crude incidence rate 29.2 per 100000 inhabitants). Most cases were male(69.0%), residents of urban areas(57.5%)and Hispanic(83.2%), with a low education level(84.5%<8 school years). SC SIR was higher in men than women(40.8 and 14.8 respectively,P<0.001),risk factors were low education RR 4.4(95%CI:2.9-6.8)and 1.6,(95%CI:1.1-2.1) for women and men respectively and Mapuche ethnicity only significant for women(RR 2.2,95%CI:1.2-3.7).Of all cases,76.4%were histologically confirmed,11.5% had a death certificate only(DCO),56.1%were TNM stageⅣ;445 cases(84.1%)were eligible for survival analysis, all completed five years follow-up;42 remained alive,392 died of SC and 11 died from other causes. Specific 5-year survival, excluding cases with DCO,was 10.6%(95%CI:7.7-13.5);5-year relative survival rate was 12.3%(95%CI:9.1-16.1),men 10.9%(95%CI: 7.4-15.2)and women 16.1%(95%CI:9.5-24.5).Fiveyear specific survival was higher for patients aged<55 years(17.3%),with intestinal type of cancer(14.6%), without metastasis(22.2%),tumor size<4 cm(60.0%), without lymphatic invasion(77.1%),only involvement of the mucous membrane(100%).Statistically significant independent prognostic factors were:TNM staging, diffuse type, metastasis, supraclavicular adenopathy, palpable tumor, and hepatitis or ascites. CONCLUSION:Social determinants are the main risk factors for SC,but not for survival. An advanced clinical stage at consultation is the main cause of poor SC survival.展开更多

Interventions that suppress this response benefit UC patients. We tested whether sodium arsenite (SA) benefits rats with dextran sulfate sodium (DSS)-colitis. The DSS-colitis was induced by 5% DSS in drinking water. SA (10 mg/kg; intraperitoneally) was given 8 h before DSS treatment and then every 48 h for 3 cycles of 7, 14 or 21 d. At the end of each cycle rats were sacrificed and colon sections processed for histological examination. DSS induced diarrhea, loose stools, hemoccult positive stools, gross bleeding, loss of body weight, loss of epithelium, crypt damage, depletion of goblet cells and infiltration of inflammatory cells. The severity of these changes increased in the order of Cycles 1, 2 and 3. Treatment of rats with SA significantly reduced this severity and improved the weight gain.展开更多

Methods: Random cluster sampling was used to select 82 clusters of 50 residents in the 2 counties. Each survey team included an ophthalmologist, a nurse and a coordinator, who went to door to door in each cluster to identify eligible people. A torch, direct ophthalmoscope and portable slit lamp were used for eye examination. Visual acuity (VA) was tested for each eye of every subject. Those with VA below 6/18 in either eye were examined and causes identified. Results: The survey identified a prevalence of blindness in people aged 50+ in Tuoketuo of 1.2% (95% Confidence Interval: 0.7% - 1.7%) and in Shangdu of 1.4% (95% CI: 1.0% - 1.9%). Cataract was identified as the leading cause of blindness (BL) and severe visual impairment (SVI), and uncorrected refractive errors were the major causes of moderate visual impairment (MVI) in both counties. Over two thirds of blindness, SVI and MVI were identified as avoidable. Conclusions: The prevalence of blindness in people aged 50+ in Tuoketuo and Shangdu was low compared to other studies conducted in China [1] [2]. The prevalence of blindness of people aged 50 years and over could be reduced by up to two thirds through better eye services in the two study areas.展开更多

文摘This study assesses the literature evidence on climate change risk, resilience, and adaptation measures used among rural farmers in East Africa.A systematic literature review was conducted comprising 30 papers from the Web of Science database published during 2000-2022.The results of the literature review showed that climate change risks have direct impacts on agricultural practices, limit rural farmers’resilience, and exacerbate their food insecurity. The most prominent risks are increasingly shorter wet seasons and heat stress, which lead to droughts and food production losses.Responding to climate risks, farmers in East Africa adopt various adaptation strategies such as mixed-and inter-cropping,conservation tillage, early planting, crop diversification, etc. Also, this review summarizes the determinants of climate change adaptation strategy selection by farmers in East Africa, including age, gender, household size, economic status and household assets, landownership and livestock, education and training, etc.Overall, the choice of adaptation strategies to climate change is strongly determined by the gender of household heads, the results of gender as a determinant of adaptation differ greatly between different case studies.Although female-headed households(FHHs)tend to perceive changes in temperature more readily than male-headed households(MHHs),the latter are generally more likely to adopt different adaptation strategies.Despite the resilience and adaptation measures used by rural farmers in East Africa now, improved weather forecasting and early warning systems are needed as a better direction towards the future.

文摘We analyzed over 8 decades of change in forest composition(represented by species proportion of basal area)and size class from more than 400 permanent plots located on the Bartlett Experimental Forest in the White Mountains of New Hampshire. These data represent one of the longest-term landscape-scale records of forest change based on permanent plots in North America. We analyzed the plots based on elevation class, land type indicating assumed successional direction(grouped into coniferous and deciduous),and inventory period within managed and unmanaged portions of the forest. An ongoing shift from small-to large-diameter stems is clear across all species,in response to the overall aging of the forest following exploitative harvesting in the 19th century.Major compositional changes include a continuing decline in shade-intolerant species(paper birch and aspen),along with the mid-tolerant yellow birch. An increase in red maple abundance through the early 1990s has leveled off or reversed.Among shade-tolerant species, increases in beech and red spruce were largely consistent with assumed land type on unmanaged plots, but heavy marking against diseased beech on managed plots restricted increase of that species.Sugar maple declined in abundance except where silvicultural intervention helped maintain it.By contrast, eastern hemlock showed a continuing expansion at all elevations below 600 m.The data continue to show little or no evidence of upward migration of species, despite evidence of recent regional change in climate. However, the BEF is poised for substantial changes when emerald ash borer and hemlock woolly adelgid, both of which are known to infest nearby areas,do arrive.

文摘Irrational irrigation practices in the Arys Turkestan Canal command area in the southern part of Kazakhstan have led to the formation of soils with poor physical and chemical properties. In the lower part of the command area, shallow groundwater has contributed to the accumulation of Na+ and to a large extent of Mg2+ on the cation exchange sites.
文摘Relative preferences of 90 images of forest stands, photos and virtual reality images were investigated by the intemet to develop a quantitative model for estimating scenic beauty preferences at the stand level, The relative priority values obtained from the questionnaire of a total of 259 judges were analyzed using regression methods for pairwise comparisons. When the background of the judges (gender, country or occupation) was included into the model as additional predictors, no significant improvements are achieved.

文摘Empirical research has shown that the use of manure significantly improves crop yield, soil fertility and water and moisture conservation. These results imply that the current downward trend will continue given China's rising economic growth, urbanization and rural infrastructural improvement.
文摘AIM:To study the incidence and survival rate of stomach cancer(SC)and its associated factors in a high risk population in Chile. An advanced clinical stage at consultation is the main cause of poor SC survival.
文摘The histopathological features and the associated clinical findings of ulcerative colitis (UC) are due to persistent inflammatory response in the colon mucosa. Treatment of rats with SA significantly reduced this severity and improved the weight gain.

文摘A comprehensive desk review of malaria trends was conducted between 2000-2010 in Zambia to study malaria epidemiology and trends to guide strategies and approaches for effective malaria control. This review considered data from the National Health Information Management System,Malaria Surveys and Programme Review reports and analyzed malaria in-patient cases and deaths in relation to intervention coverage for all ages.Data showed three distinct epidemiological strata after a notable malaria reduction(66%)in in-patient cases and deaths, particularly between 2000-2008.These changes occurred following the(re-)introduction and expansion of indoor residual spraying up to 90%coverage,scale-up of coverage of long-lasting insecticidetreated nets in household from 50%to 70%,and artemisin-based combination therapy nationwide. However, malaria cases and deaths re-surged,increasing in 2009-2010 in the northern-eastern parts of Zambia.Delays in the disbursement of funds affected the implementation of interventions, which resulted in resurgence of cases and deaths. In spite of a decline in malaria disease burden over the past decade in Zambia,a reversal in impact is notable in the year 2009-2010,signifying that control gains are fragile and must be sustained toeliminate malaria.
文摘Aim: A population-based survey was conducted in Tuoketuo and Shangdu Counties in Inner Mongolia Autonomous Region, China, in the Autumn of 2010, to assess the prevalence and causes of blindness and visual impairment of people aged 50 years and over. The prevalence of blindness of people aged 50 years and over could be reduced by up to two thirds through better eye services in the two study areas.
