Low-Level Antibiotic Resistance among Staphylococcus aureus and Gram-Negative Pathogens from Infected Skin and Soft Tissues in Rural Kenya

Introduction: Bacterial skin and soft tissue infections (SSTIs) are a cause of frequent inpatient and outpatient care visits whose causative agents are associated with a high antimicrobial resistance burden. For insights on antimicrobial susceptibilities in a rural setting, we examined specimens from suspected SSTIs from two public health facilities in Kenya. We additionally assessed antibiotic use, appropriateness of empiric therapy and risk factors for SSTI. Methodology: Between 2021 and 2023, 265 patients at Kisii and Nyamira County Referral hospitals were enrolled. Wound swabs/aspirates were collected and processed following standard microbiological procedures. Identification and antimicrobial susceptibility were performed using the VITEK 2 Compact platform. Demographic, clinical, and microbiological data were analyzed with R Statistical software. Results: S. aureus was isolated in 16.2% (43/265) of patients with a methicillin resistance (MRSA) proportion of 14% (6/43). While 13/15 drugs elicited susceptibilities ranging from 84% - 100%, penicillin (16%) and trimethoprim-sulfamethoxazole [TMP-SXT] (23%) yielded the lowest susceptibilities. Escherichia coli (n = 33), Klebsiella pneumoniae (n = 8), Pseudomonas aeruginosa (n = 8), and Citrobacter species (n = 4) were the most commonly isolated gram-negative species. Gram-negative strains showed high susceptibilities to most of the tested drugs (71% - 100%) with the exception of ampicillin (18%), TMP-SXT (33%), and first and second generation cephalosporins. Conclusions: The low MRSA prevalence and generally high antibiotic susceptibilities for S. aureus and gram-negative bacteria present opportunities for antibiotic stewardship in the study setting. Diminished susceptibilities against penicillin/ampicillin and TMP-SXT accord with prevailing local data and add a layer of evidence for their cautious empiric use. 展开更多

The AYP living with HIV enrolled at the 68 NARHY, Lagos reflects the national challenges with poor viral suppression. The OTZ program aligns with the UNAIDS 95-95-95 goals. It seeks to empower AYPLHIV to be in charge of their treatment and commit to triple zero outcomeszero missed appointments, zero missed drugs, and zero viral loads. The purpose of the study was to assess the impact of the OTZ program on viral load suppression among members of the adolescent club in 68 NARHY, Lagos. Method: A cross-sectional retrospective study to evaluate the impact of the OTZ program on the viral load of 53 AYP enrolled in the OTZ program between March 2019 to December 2019 was analyzed. The Percentage of viral load suppression before enrollment compared with 6 and 12 months after enrollment into the OTZ program. The AYP is grouped into 10 - 14, 15 - 19, and 20 - 24 years. Activities conducted were peer driven monthly meetings with the AYP during which the adolescents interacted on issues relating to improving their treatment outcomes, healthcare workers reviewed their clinical status, viral load result, provider peer counseling, and caregivers engagement to support adherence to medication and ARV refills. Results: Before OTZ, 81% aged 10 - 14 years, 75% aged 15 - 19 years, and 25% aged 20 - 24 years were virally suppressed (VL less than 1000 copies/ml). Six months after enrollment, 94% were virally suppressed95% aged 10 - 14 years, 96% aged 15 - 19 years, and 66% aged 20-24 years. Twelve months after enrollment, 96% of AYP were virally suppressed100% aged 10-14 years, 93% aged 15 - 19 years, and 100% aged 20 - 24 years. Males viral load (VL) suppression improved from 79% to 96% and 92%, while females VL suppression improved from 69% to 93% and 100% at 6 and 12 months respectively. Conclusion: The OTZ activities contributed to improved viral load suppression in the AYP of the facility. 展开更多

In March 2020, the first cases of SARS-CoV-2 were reported in Accra, Ghana. These initial cases were diagnosed at the Advanced Research Laboratories (ARL) of the Noguchi Memorial Institute for Medical Research (NMIMR)... In March 2020, the first cases of SARS-CoV-2 were reported in Accra, Ghana. These initial cases were diagnosed at the Advanced Research Laboratories (ARL) of the Noguchi Memorial Institute for Medical Research (NMIMR), University of Ghana. The ARL which hitherto was used for routine clinical research in viral, bacteria and immunological studies has since been the facility of choice for testing for all suspected cases of COVID-19 submitted from across Ghana and beyond. The success of testing at the ARL hinged on the availability of several laboratory spaces furnished with state-of-the-art diagnostic equipment and working aids. During the “peak season” where overwhelming numbers of clinical specimens were received, the ARL processed and got results for close to four thousand samples daily. After general disinfection and re-bagging into smaller numbers, at the entrance of the ARL, the samples are taken to a central receiving laboratory, where they are received and entered in a database with accompanying case investigation forms. All samples that are successfully sorted and matched are sent to general laboratories for nucleic acid extraction and then referred to the Instrumentation laboratory for real time reverse-transcription polymerase chain reaction (RT-PCR). When the RT-PCRs were completed, results were analysed and transmitted via email and/or local network to the data reporting office. The data managers then reported results to the investigators and the Ghana Health Service (GHS). Additionally, the ARL provided a next-generation Genome Sequencing platform in partnership with the West African Centre for Cell Biology of Infectious Pathogens at the University of Ghana, which was essential in reporting the genome data of the circulating variants of SARS-CoV-2 in Ghana. Conclusively, it is worth noting, that the NMIMR fulfilled its mandate of supporting the country with specialized diagnostics through the judicious use of the ARL for SARS-CoV-2 testing, from sample receipt to data reporting. The ARL facility and the research faculty have trained and continue to train budding laboratories on biosafety, biosecurity, best practices and testing protocols. It is obvious that the success story of SARS-CoV-2 testing in Ghana, cannot be complete without the mention of the ARL at NMIMR. 展开更多

Background and objectives:Coronavirus disease 2019(COVID-19)is a pandemic that has become a major source of morbidity and mortality worldwide,affecting the physical and mental health of individuals influencing reproduction.Despite the threat,it poses to maternal health in sub-Saharan Africa and Nigeria,there is little or no data on the impact it has on fertility,conception,gestation and birth.To compare the birth rate between pre-COVID and COVID times using selected months of the year.Materials and methods:This was a secondary analysis of cross-sectional analytical study data from the birth registries of three tertiary hospitals,comparing two years[2019(Pre-COVID)]versus[2020(COVID era)]using three months of the year(October to December).The data relied upon was obtained from birth registries in three busy maternity clinics all within tertiary hospitals in South-East Nigeria and we aimed at discussing the potential impacts of COVID-19 on fertility in Nigeria.The secondary outcome measures were;mode of delivery,booking status of the participants,maternal age and occupation.Results:There was a significant decrease in tertiary-hospital based birth rate by 92 births(P=0.0009;95%CI:-16.0519 to-4.1481)among mothers in all the three hospitals in 2020 during the COVID period(post lockdown months)of October to December.There was a significant difference in the mode of delivery for mothers(P=0.0096)with a 95%confidence interval of 1.0664 to 1.5916,as more gave birth through vaginal delivery during the 2020 COVID-19 period than pre-COVID-19.Conclusion:Tertiary-hospital based birth rates were reduced during the pandemic.Our multi-centre study extrapolated on possible factors that may have played a role in this decline in their birth rate,which includes but is not limited to;decreased access to hospital care due to the total lockdowns/curfews and worsening inflation and economic recession in the country. 展开更多

This study attempted to use the soil and water assessment tool(SWAT), integrated with geographic information systems(GIS), for assessment of climate change impacts on hydropower generation. This methodology of climate change impact modeling was developed and demonstrated through application to a hydropower plant in the Rio Jubones Basin in Ecuador. ArcSWAT 2012 was used to develop a model for simulating the river flow. The model parameters were calibrated and validated on a monthly scale with respect to the hydro-meteorological inputs observed from 1985 to 1991 and from 1992 to 1998, respectively. Statistical analyses produced Nash-Sutcliffe efficiencies(NSEs) of 0.66 and 0.61 for model calibration and validation, respectively, which were considered acceptable. Numerical simulation with the model indicated that climate change could alter the seasonal flow regime of the basin, and the hydropower potential could change due to the changing climate in the future.Scenario analysis indicates that, though the hydropower generation will increase in the wet season, the plant will face a significant power shortage during the dry season, up to 13.14% from the reference scenario, as a consequence of a 17% reduction of streamflow under an assumption of a 2.9℃ increase in temperature and a 15% decrease in rainfall. Overall, this study showed that hydrological processes are realistically modeled with SWAT and the model can be a useful tool for predicting the impact of climate change. 展开更多

The computation of the representative ground motions,to be used as input for the dynamic analyses of a struc- ture at a particular site,can be approached by several methods.The choice of the approach depends on two factors:the da- ta available and the type of problem to be solved.This paper reports the experience of the authors in approaching a specific case study:the Southern Memnon Colossus,located in Luxor,Egypt.The results are of interest when the hazard analysis estimation in developing countries and the safeguard of cultural heritage are concerned.Monuments have to be treated as important structures,due to their historical and economical value.Hence,standard procedures of probabilistic seismic haz- ard analysis for the seismic classification of common buildings have to be disregarded.On the other hand,the consequences of the collapse of a monument are not comparable to those related to structures such as nuclear power plants and large dams, for which the deterministic seismic hazard analysis provides a straightforward framework for evaluation of the worst case ground motions.An'intermediate'approach,which requites a lower amount of input data with respect to the deterministic one,is adopted.Its stochastic component can eapture significant eharacteristics of earthquakes,primarily the frequency contents which depend on the magnitude(often referred to as the earthquake scaling law). 展开更多

However, many women in Malawi seek treatment when the disease has reached inoperable stage. This study was conducted to explore factors that contribute to delay in seeking early diagnosis and treatment of cervical cancer among women in Malawi. The study was exploratory and utilized qualitative data collection and analysis method. In-depth interviews were conducted using a semi-structured interview guide on a purposive sample of 24 women who were diagnosed of cervical cancer at the gynaecological wards of Zomba and Queen Elizabeth Central Hospitals in Malawi between July and September, 2011. Thematic content analysis was used to analyze the qualitative data. Two major themes (individual and health facility) emerged from the participants’ narratives as factors that contributed to their delay in seeking early diagnosis and treatment. The individual factors included;limited knowledge on symptoms and signs and limited financial resources. The health facilities factors included;limited accessibility and unavailability of cancer screening facilities in the health centres. Results show that there is a need to strengthen the screening of cervical cancer among women in the country. In addition, there is a need to create community awareness on the signs and symptoms of cervical cancer and the merits of seeking early diagnosis and treatment. 展开更多

A study was conducted to describe the experiences of primiparous women with the support they received from their birth companions during labour and delivery in Malawi. The study design was descriptive and utilized qualitative data collection and analysis method on a sample of 20 primiparous women. The women were recruited from the postnatal ward of Bwaila hospital and were interviewed regarding their experiences on the support they received from their birth companions during labour and delivery using an open ended interview guide. Data was manually analyzed using content analysis. Primiparous women described the support they received from their birth companions as useful and beneficial. Birth companions provided advice, physical, emotional and spiritual support to the women during their labour and delivery. In addition, the birth companions were viewed as guardians of the women during labour and delivery. Results however, show that some women were not properly assisted by their companions because both the mothers and companions lacked knowledge on birth companionship. The results further show that birth companions play important roles during the birth and delivery of primiparous women and thereby improving birth outcomes. Therefore, there is a need to train the companions regarding support to a woman during labour and delivery. Primiparous women also need to be trained during antenatal care education so that they properly understand the roles of a birth companion as opposed to those of the midwives. 展开更多

The biological intracellular vesicles, formed from the cell membrane or from different cell organelles, play a main role in the intracellular transport, transfer and exchange of molecules and information. Extracellular vesicles are also detected in organisms belonging to any of the three main branches of evolution, Archaea, Bacteria and Eukarya. There is an increasing consensus that these vesicles are important mediators of intercellular communication. All the intracellular and extracellular vesicles present a characteristic lipid composition and organization that governs their formation, targeting and function. This paper gives an overview of the lipid chemical and physical structure, strongly related to their biological function. The properties and role of the different types of lipids from membranes and vesicles are described. Then, their physical structure is shown as self-associated in a bilayer and organized as a lyotropic liquid crystal. The present paper underlies the structural similarity between these biological vesicles and a new synthetic material, the “liquid crystalline fullerodendrimers” obtained from the biological model. It is composed of a basket of carbon associated with a liquid crystalline material and has been shown to exhibit highly efficient properties of information transfer. Our observation stresses the essential role of the liquid crystalline structure of lipids in their function as biological nanovehicles of information. The comparison with the synthetic material contributes to a better understanding of the role of lipids for cell communication in living organisms. 展开更多

Seroprevalence of Hepatitis E Virus among Voluntary Blood Donors in Nairobi County, Kenya: A Pilot Study

HEV disease is mainly spread via stool contamination of water and food supplies. The virus has recently been identified as a global threat to blood safety. The seroprevalence of HEV among blood donors in Kenya remains unknown. This study was carried out as a pilot study to determine the seroprevalence of the virus among blood donors at Nairobi Blood Transfusion Center during the period from September 2018 to January 2019. A total of three hundred and fifty eight (n = 358) plasma samples were used in this study. The plasma was tested for HEV IgM and IgG antibody using a qualitative membrane-based immunoassay (Biopanda reagents Belfast UK). Demographic characteristics of the blood donors were also collected. The age of the blood donors ranged from 16 to 61 years with a mean age of 29 years (±14). Of the study subjects, 36% (n = 130) were females while 64% (n = 228) were males. Overall, 10.6% (38) and 7.8% (28) of the samples were HEV IgG and IgM seropositive respectively. Anti-HEV was distributed among all age groups;however donors aged 20 - 40 years had the highest prevalence. This study shows a relatively high prevalence of anti-HEV among healthy blood donors in Nairobi, Kenya. Further studies would be needed in other population groups to assess the potential benefit of incorporating HEV screening of blood products to the current blood donor selection criteria. Moreover, further studies to determine the circulating genotypes of HEV among this group are required for epidemiological reasons. 展开更多

A field experiment was conducted using Ceranock bait station, "attract and kill" system to combat Mediterranean fruit fly, Ceratitis capitata, in apricot orchards, Central of lraq during the 2013 field season. Control program was implemented in three orchards of mixed apricot varieties. Ceranock traps were hung in each tree of two separate orchards. Delta and McPhail traps were used to monitor Medfly population density in each treated and control orchards. Results indicated that Medfly activity in apricot orchards started during the last week of April coincided with the beginning of maturity for fruits of early varieties. The total of trapped insects in monitoring traps after one week of Ceranock application were 0, 1 and 31 adults for the 1st, 2nd and the control orchards, respectively. Three weeks later and on, the number of trapped adults started to increase dramatically. At the end of apricot season and after 45 days of Ceranock trap application, the number of trapped insects in monitoring traps reached 110, 111 and 2,349 adults/week, respectively, for the 1st, 2nd and 3rd orchard. The use of Ceranock traps reduced Medfly population density by 86%-97%. The percentage of fruit injuries in early maturing varieties were 0.75%, 1% and 34% for the 1st, 2nd and control orchards, respectively. While, late varieties were 2% and 3% for 1st and 2nd orchards, and 69% for the control. The percentage of reduction in fruit damages reached to 95% and 97%, respectively in the 1st and 2nd orchard. The results of this study demonstrate clearly the efficacy of Ceranock bait station, "attract and kill" system as a control measure for Medfly in apricot orchards. 展开更多

Field experiments were conducted using Ceranock bait station, "attract and kill" system to combat Mediterranean fruit fly, Ceratitis capitata, in citrus orchards of Baghdad and Wasit governorates, Iraq, during 2013-2014 season. Control program was implemented in six citrus orchards of mixed citrus varieties: Orange, Citrus sinensis; Mandarin, C. reticulate; Lemon, C. lemon; and Bergamot, C. aurantium. Ceranock traps 240/ha were hanged in trees of two separate orchards in each location, with third orchard served as control. Delta and McPhail traps supplied with pheromone were used to monitor Medfly population density in treated and control orchards. Results indicated that Medfly activity and presence in citrus orchards started during the first week of October coincide with the beginning of fruits ripening. The result indicated that Medfly population density in Wasit orchards was more than that of Baghdad orchards. The total trapped insects after two weeks of Ceranock were 59, 94, 142 and 205, 277, 765 adults for the firsst, second and the control orchards in the two locations, respectively. The percentage of fruit injuries were increased dramatically when fruits mature and its color turn to yellowish reaching of 35%, 56%, 21% and 32% in mandarin and orange fruits in Baghdad and Wasit orchards, respectively, compared with 3%, 4%, 4% and 5% in treated orchards, respectively. Calculated percentage of fruit damage indicated that reduction in injuries were 97% in mandarin and 96% in orange at Baghdad orchards and were 95% and 96%, respectively, in Wasit orchards. The results of this study demonstrate clearly the efficacy of Ceranock bait station, "attract and kill" system as a control measure for Medfly in citrus orchards. 展开更多

This study was conducted to assess birth preparedness and complication readiness among postnatal mothers at Khombedza Health Centre in Salima District, Malawi. The study design was descriptive cross sectional and utilized qualitative data collection and analysis method on a random sample of 15 postnatal mothers. A semi structured questionnaire was used to assess birth preparedness and complication readiness among the postnatal mothers during their most recent pregnancy and child birth. The findings indicate that overall, all the mothers had attended antenatal care and were aware of the importance of seeking health facility delivery. The mothers were also conversant with the items to bring with them during labour and delivery. Results further show that the participants had some knowledge of danger signs during postpartum and also for the new born baby but had limited knowledge of danger signs during antenatal, labour and delivery. Although the mothers had planned to deliver at the hospital, they did not save money for transport. There is therefore a need to strengthen antenatal care education on birth preparedness and complication readiness. Such knowledge would assist pregnant mothers to identify danger signs during antenatal, labour and delivery and therefore seek emergency obstetric care on time to minimize maternal and neonatal mortalities. 展开更多

Zoonotic Pathogens in Ticks Collected from Livestock in Kenya

We used PCR to detect pathogens of public health importance in ticks collected from diverse regions of Kenya. 503 tick pools were collected from 982 cattle, 300 sheep and 379 goats that were presented for slaughter at major abattoirs in Nairobi and Mombasa. Tick DNA was screened by qPCR optimized for single-plex detection of Babesia microti, two-plex Coxiella burnetii/Ehrlichia chaffeensis or Bartonella henselae/Borrelia recurrentis or three-plex for the non-human Babesia spp/Anaplasma phagocytophylum/Borrelia burgdorferi. Pathogen prevalence was calculated against tick type and the geographical origin. Computational analysis was performed with Graphpad prism 5. Out of 503 tick analyzed, 21% (106) were positive for at least one pathogen. C. burnetii was the most abundant at 70% (74/106), followed by non-human Babesia at 17% (18/106), 5% (5/106) for B. burgdorferi, 7% (7/106) for other Borrelia species and <1% (1/106) for E. chaffeensis and A. phagocytophylum. B. henselae and the human infective B. microti were not detected. Rh. pulchellus was the most promiscuous one in carrying pathogens: C. burnetti, Babesia, B. burgdorferi, E. chaffeensis and A. phagocytophylum. Non-human infective Babesia were detected in all ticks except Amblyomma. Four counties had 70% of the infected ticks: Marsabit 25% (n = 26/106), Kajiado 17% (18/106), Wajir 16% (17/106) and Narok 11% (12/106). This study identified a number of tick-borne pathogens that cause febrile infections often confused with malaria. Follow-up research will be needed to determine prevalence in humans. 展开更多

Compliance of state registered nurses to nursing standards during practice in tertiary facilities in Malawi

The aim of this study was to describe the quality of practice offered by nurses to patients who underwent surgical incision into the abdomen (post-laparotomy) at tertiary hospitals (Kamuzu, Mzuzu, Queen Elizabeth and Zomba) in Malawi. The study design was descriptively cross sectional and utilized a quantitative data collection and analysis method. All available 48 registered nurses in the surgical ward of four central hospitals and 100 patients that were admitted in the ward during the time of study were recruited. A 3-point scale rating consisting of compliance (C = 1), partial compliance (C 1.0-0.5) and non compliance (C 0.5-0.0) was used to describe the nurse midwives compliance with the process standards of care. Results show that nurses in all the 4 central hospitals partially complied with assessment and planning standards. During assessment, the nurses assessed the physical aspects of care but did not assess the psychological, spiritual and cultural aspects of care. At planning the nurses assigned and delegated tasks based on the knowledge and skills of the provider selected but did not comply with factors related to safety, effectiveness and cost of care. All the facilities fully complied with implementation standard because they implemented care in a safe and appropriate manner and communicated with patients/ significant others and other health care providers. However, regarding systematic and ongoing evaluation of patients’ condition only Mzuzu Central hospital partially complied while the rest of the facilities were not compliant. All the facilities did not comply with documentation standard of care because the patient records were not legible and did not precisely depict comprehensiveness of care nor bore signatures of the implementers of the care. Results are discussed by relating the level of compliance to standards and the quality of patient care. 展开更多

Comparison of Nurses’ and Families’ Perception of Family Needs in Critical Care Unit at Referral Hospitals in Malawi

The study design was descriptive and utilized quantitative data collection and analysis method. Nurses (n = 62) working in the Intensive Care Units (ICU) and High Dependency Units (HDU) of three tertiary health facilities in Malawi at the time of the study consented to participate in the study. In addition, family members (n = 62) who were looking after a critically sick relative in the ICU and HDU in the same tertiary facilities consented and participated in the study. Data were collected using a questionnaire developed from the Critical Care Family Needs Inventory for a period of 7 days. STATA version 10 was used to analyze data. The rank correlation between the mean scores of perceived needs across major need categories of support, comfort, information, proximity and assurance between the nurses and family members was significantly different from zero (r = 0.97, p = 0.005). Nurses and family members, respectively ranked assurance (90% and 92%) as the highest priority need, followed by information (78% and 85%) and comfort (78% and 84%) and then support (70% and 73%) and proximity (66% and 69%). The ranking however between the 2 groups on 16 out of 45 individual needs were significantly different (p < 0.05). Among the nurses, the rankings by registered nurses were significantly higher (p < 0.05) from those of nurse midwife technicians. There were also significant differences (p < 0.05) in the mean scores between the gender of family members with men demanding more “comfort” than females. Results show a need for facility authorities to formulate ICU policies and strategies that ensure provision of friendly services to family members of critically ill patients. 展开更多

To improve sorghum productivity, it is crucial to provide farmers with high yielding, stable sorghum cultivars that are tolerant to drought and other constraints. The stable performance of sorghum varieties in a growing region is critical to obtain a high and stable yield. In the 2012-2014 crop year, 24 genotypes, including standard controls, were evaluated at the national variety trial stage over six main dry lowland sorghum growing sites and two years made 7 environments to evaluate their performance, stability and to quantify Genotype by Environment Interaction (GEI) across moisture stress sorghum growing areas of Ethiopia. Spatial modeling has been used to estimate predicted mean (BLUPs) results and Performance and estimation of environmental correlation, heritability, GEI, and other parameters using the ASReml3-R analysis package. The predicted mean yield of the test genotypes across the environment ranged from 3.45 to 1.56 t<span style="font-family:Verdana;">·</span><span style="font-family:;" "=""><span style="font-family:Verdana;">ha</span><sup><span style="font-family:Verdana;">-1</span></sup><span style="font-family:Verdana;">. Based on the result genotype G13, it could be further promoted because of its yield advantage and other important attributes over the standard checks, but it is the least stable.
