The pattern of clinical forms of respiratory tuberculosis in children shows a preponderance of intrathoracic lymph node tuberculosis (89.4%) that is characterized by a complicated process in every third child under present-day conditions. Positive result of PCR closely correlates with the severity and extent of the specific process in children. Real-time PCR (RT-PCR) was ascertain to exhibit the highest sensitivity in detecting Mycobacterium tuberculosis DNA in children with primary generalized tuberculosis (62.5%) and in those with a disseminated specific process (55.6%), which was much higher than conventional bacteriological study of diagnostic materials. By taking into account the findings, the RT-PCR detection of M. tuberculosis was considered as a substantial criterion for evaluating the magnitude of specific changes and the degree of tuberculosis infection activity in children. 展开更多

Efficacy of Fixed-Dose Drugs in Treatment of Pulmonary Tuberculosis in Children

The main problem in treatment of children with various forms of tuberculosis is the lack of pediatric dosages of anti-TB drugs. The aim of the study is to determine efficacy of the 〈Isoniazid 150 mg+Pyrazinamid 375 mg+Rifampicin 150 mg〉 (Ftizamax ) fixed-dose combination. We estimated the clinical, radiological and laboratory parameters during treatment of 73 children with pulmonary tuberculosis, of which 34 children were treated by the 〈Isoniazid 150 mg+Pyrazinamid 375 mg+Rifampicin 150 mg〉 in combination with the Ethambutol; while the others were treated with mono-drugs under the standard regimen I. Effective use of 〈Isoniazid 150 mg+Pyrazinamid 375 mg+Rifampicin 150 mg〉 (Ftizamax ) for children is supported by positive results towards the end of the intensive phase of the therapy and the continuation phase, as well as by minimized adverse drug reactions during treatment. Combination of active substances in the drug and its dosage is easy to use, which allows recommending this drug for treatment of tuberculosis in children. 展开更多

背景:六个俄罗斯地区的结核病服务机构实施的结核病(TB)患者社会支持项目。目的:检出病人不依从治疗的危险因素并评价社会支持的可能影响。方法:回顾研究2003年2~3季度期间登记的涂片阳性和阴性的新结核病人。通过病例——对照研究,包... 背景:六个俄罗斯地区的结核病服务机构实施的结核病(TB)患者社会支持项目。目的:检出病人不依从治疗的危险因素并评价社会支持的可能影响。方法:回顾研究2003年2~3季度期间登记的涂片阳性和阴性的新结核病人。通过病例——对照研究,包括不依从治疗病人作为病例组、成功治疗病人作为对照组进行的资料分析,采用多变量logistic回归模型分析。结果:共纳入1805例肺结核(PTB)病人。病人不依从治疗的概率为2.3%-6.3%。经多变量分析,不依从治疗的独立相关危险因素有:失业(OR=4.44;95%CI:2.23-8.86),酗酒(OR=1.99;95%CI: 1.04-3.81),无家可归(OR=3.49;95%CI:1.25-9.77)。社会支持降低了不依从结果(OR=0.13;95%CI:0.06-0.28)。该分析控制了年龄、性别、地区、居住地和痰涂片抗酸染色因素的影响。结论:失业、酗酒和无家可归是增加新结核病患者不依从治疗风险的相关因素,相反,社会支持降低了不依从。在俄罗斯为了评估和确定最佳成本——效益的社会支持对改善结核病人治疗结果的作用进一步进行前瞻性随机研究是必须的,尤其在有较高不依从治疗风险的人群。 展开更多

The biocomposite films were prepared from poly(L-lactic acid)and cellulose nanocrystals.To improve interfacial compatibility of hydrophilic cellulose nanocrystals with hydrophobic matrix polymer as well as to provide the osteoconductive properties,cellulose was functionalized with poly(glutamic acid).The modified cellulose nanocrystals were better distributed and less aggregated within the matrix,which was testified by scanning electron,optical and polarized light microscopy.According to mechanical tests,composites filled with nanocrystals modified with PGlu demonstrated higher values of Young’s modulus,elongation at break and tensile strength.Incubation of composite materials in model buffer solutions for 30 weeks followed with staining of Ca^(2+)deposits with Alizarin Red S assay testified better mineralization of materials containing PGlu-modified cellulose nanocrystals as filler.As the result of in vivo experiment,the developed composite materials showed less level of inflammation in comparison with pure polymer matrix and composites filled with non-functionalized cellulose nanocrystals. 展开更多
