Observation of Suicide Prevention in Hospitalized Patients with Depression Based on Taihe Emotion-distress Index

[Objectives]To reduce suicidal ideation and suicidal behavior through initial screening of patients with suicidal tendencies and implementing suicide prevention interventions during their hospitalization.[Methods]The Taihe Emotion-distress Index(THEI)was used to conduct pre-admission and post-discharge tests to explore the effects of suicide prevention measures during hospitalization on the alleviation of the disease and the reduction of suicidal behaviors.The study selected patients who were diagnosed with depression in the psychological outpatient department of Taihe Hospital from April 2019 to September 2019 and had to be hospitalized,including patients with moderate depressive episodes,severe depressive episodes with or without psychotic symptoms,and patients with suicidal thoughts and self-harming behaviors.[Results]The pre-admission and post-discharge test data of hospitalized patients were analyzed,and the non-parametric paired sample T test was carried out,and the result was P<0.05,showing that there are significant differences between the pre-admission and post-discharge test data.[Conclusions]The measures of suicide prevention intervention are effective to a certain extent.

Application of Medical Failure Mode and Effect Analysis in Preoperative Preparation for Coronary Intervention

[Objectives]To explore application effect of healthcare failure mode and effect analysis(HFMEA)in preoperative management of coronary intervention.[Methods]The preoperative management process of 455 patients who underwent coronary intervention surgery from January to March in 2022 was sorted out,and the failure modes and causes in the preoperative management were analyzed.The priority risk value was calculated,and the top 6 failure modes were found out.Improvement measures were formulated according to the causes of failure modes.Two months after the implementation of the procedure,470 patients who underwent coronary intervention surgery from May to July in 2022 were selected as the intervention group,and the optimized preoperative preparation procedure by HFMEA was adopted.The decrease of risk priority number(RPN)and RPN value of two high-risk failure modes,as well as changes in the rate of the time of taking dual-antiplatelet drugs reaching the standard,the rate of passing the dress code,the rate of timely signing of informed consent,and the rate of filling in the handover form of preoperative preparation were analyzed.[Results]The rate of the time of taking dual-antiplatelet drugs reaching the standard,the rate of passing the dress code,the rate of timely signing of informed consent,and the rate of filling in the handover form of preoperative preparation in experimental groups were higher than those of the control group,and the difference was statistically significant(P<0.05).[Conclusions]Applying HFMEA to the management of preoperative preparation process for coronary intervention can effectively improve the timeliness and qualification rate of preoperative preparation for coronary intervention,and provide guarantee for the safe and smooth implementation of coronary intervention.

The Characteristics of the Drug Composition in the Treatment of Insomnia by External Bath Prescription of Traditional Chinese Medicine

[Objectives]The research aimed to carry out a retrospective analysis on the characteristics of the drug composition in the treatment of insomnia by external bath prescription of traditional Chinese medicine,to provide ideas for the research and development of new Chinese medicine for insomnia.[Methods]The external bath prescriptions of traditional Chinese medicine related to insomnia were collected,and the appearance frequency of drug was analyzed,and statistical analysis of drug with high appearance frequency was conducted.[Results]In the external bath prescriptions of traditional Chinese medicine related to insomnia,the top ten herbs with high appearance frequency were as follows:Chaozaoren,Drgonsbones,Margaritifera,Yejiaoteng,Schisandrae Chinensis Fructus,Lilii Bulbus,Platycladi Semen,Poria,Coptidis Rhizoma,and Scutellariae Radix.According to the classification by efficacy,these herbs mostly have the effects of calming,nourishing and heat clearing.[Conclusions]Traditional Chinese medicine is most closely related to heart,kidney,liver and other viscera.Tranquilizing and sedative drugs should be chosen,which corresponds with the understanding on pathogenesis of insomnia in traditional Chinese medical science,and embodies the characteristics of external bath prescription of traditional Chinese medicine in the treatment of insomnia.

Evaluation of Clinical Efficacy of Combined Use of Acupuncture and Medicine in Treating Sleep Disorder in Patients with Anxiety Disorder

[Objectives]To evaluate the clinical efficacy of combined acupuncture and medicine in the treatment of sleep disorders in patients with anxiety disorders.[Methods]A total of 300 anxiety patients with sleep disorders were randomly selected and divided into a research group and a control group,with 150 cases in each group.The control group was administered with western medicine,and the research group was treated with acupuncture and medicine to compare their clinical treatment effects.[Results]The total effective rate of treatment in the research group was significantly higher than that in the control group;after treatment,both the PSQI and SAS scores of the two groups were significantly reduced,and the reduction degree of the research group was higher than that of the control group,the difference was statistically significant(P<0.05).[Conclusions]Combined use of acupuncture and medicine to treat sleep disorders in patients with anxiety disorders has better curative effect and fewer adverse reactions,so it has high clinical application value.

Clinical Observation of Huatanjieyu Granule in the Treatment of Post-stroke Depression

[Objectives]To observe the clinical effect of Huatanjieyu granule on patients with post-stroke depression.[Methods]Total 76 patients with confirmed post-stroke depression were randomly divided into two groups(treatment group and control group)according to the double-blind principle.The treatment group(38 cases)was treated with Huatanjieyu granule combined with escitalopram,and the control group(38 cases)was treated with escitalopram alone.The course of treatment was 6 weeks.Through clinical efficacy,Hamilton Depression Scale(HAMD)and Treatment Emergent Symptom Scale(TESS)scores were compared,and blood,liver function,renal function and electrocardiogram were examined.[Results]The scores of HAMD and TESS in patients with post-stroke depression were significantly decreased by Huatanjieyu granule,and there were no obvious side effects.The total effective rate was 92.1%in the treatment group and 76.3%in the control group.There was significant difference between the two groups(P<0.01).[Conclusions]Huatanjieyu granule was effective in the treatment of post-stroke depression with little side effects and was easy to be accepted by patients.

Effects of Onion Extract on Male Sexual Dysfunction Induced by SSRI Antidepressants

[Objectives] To investigate the effect of onion extract on male sexual dysfunction caused by SSRI antidepressants.[Methods] 40 male patients with depression and anxiety who took SSRI antidepressants for a long time were randomly divided into observation group and control group. The observation group was treated with onion extract and SSRI drugs, while the control group was treated with SSRL drugs only. After 4 weeks of treatment, the scores of the two groups were evaluated by the International Index of Erectile Function (IIEF) questionnaire and the serum testosterone levels were measured in the two groups.[Results] Onion extract could improve the sexual dysfunction caused by SSRI drugs and significantly increase the level of serum testosterone.[Conclusions] Onion extract could significantly enhance the sexual function of men, and could alleviate the sexual dysfunction caused by SSRI drugs by increasing the level of serum testosterone.

Effect of Individualized Exercise Therapy on Cancer-related Fatigue and Negative Emotion in Patients with Gastrointestinal Tumor during Perioperative Period

[Objectives]To investigate the effect of exercise therapy in gastric cancer patients during perioperative period.[Methods]100 patients with gastric cancer who underwent elective operation in the Department of Gastrointestinal Surgery of Taihe Hospital Affiliated to Hubei University of Medicine were divided into observation group and control group by convenience sampling.The control group received routine nursing measures,and the experimental group received exercise therapy intervention measures on the basis of the control group.The patients were evaluated by the General Information Questionnaire,Self-Rating Anxiety Scale,Self-Rating Depression Scale and Cancer-related Fatigue Scale at the time of admission,the second week and the sixth week after operation.[Results]The time effect,intervention effect and interaction effect of anxiety score,depression score and cancer-related fatigue score were significant(all P<0.05)and the index of enhanced recovery after surgery was significant(P<0.05)in the two groups at the second and sixth week after operation.[Conclusions]Exercise therapy is beneficial to promoting the enhanced recovery after surgery in advance,reduce cancer-related fatigue,negative emotion and accelerate rehabilitation in patients with gastrointestinal tumors.

Logistic Regression Analysis on the Correlation between Physical Diseases and Life Events and Depression in the Elderly

[Objectives]This study was conducted to explore the effects of physical diseases and life events on depression among urban elderly people.[Methods]Five hundred urban elderly people in Shiyan City were selected using a convenient sampling method to investigate and diagnose depression using the Geriatric Mental State Schedule and its accompanying computer diagnostic system.The Minimum Data Set(MDS)and related factor survey form were used to investigate physical diseases and life events,and statistical analysis was conducted using methods such as logistic regression analysis and factor analysis.[Results]The prevalence rates of angina,stroke,or partial paralysis,migraine,vision problems,hyperthyroidism or hypothyroidism,as well as the number of physical diseases were significantly higher in elderly depression patients than in objects free of depression.Stroke or partial paralysis[OR(95%CI)=6.389(1.684,24.237)],emotional trauma events[OR(95%CI)=1.438(1.069,1.934)]and adverse health and economic events[OR(95%CI)=1.652(1.099,2.483)]were risk factors for late life depression(LLD).[Conclusions]Some physical diseases and life events may affect the onset process of depression in urban elderly people,and various measures need to be taken to reduce the risk of physical diseases and life events on depression.

Relationship between Serum Uric Acid Level and Clinical Symptom Improvement in Patients with Bipolar Disorder Type I in Their Manic Episode

[Objectives]To evaluate whether the level of serum uric acid in patients with bipolar disorder type I in their manic episode was different from that in healthy subjects,and to evaluate whether the level of serum uric acid is related to the severity of manic episode and the improvement of clinical symptoms.[Methods]A total of 70 patients with bipolar disorder type I in their manic episode were selected,their serum uric acid levels were measured at the beginning of the enrollment and at the end of the first,second and third week,and the clinical symptoms were evaluated with Young Mania Rating Scale(YMRS).65 healthy subjects were enrolled,and their serum uric acid levels were measured only at the beginning of the enrollment.[Results]The level of serum uric acid in patients with bipolar disorder type I in their manic episode was higher than that in healthy adults(t=8.153,p=0.039).At the end of the third week,the YMRS score and uric acid level of the patients were lower than those of the patients at the beginning of the enrollment(t=17.107,p=0.000;t=35.864,p=0.000).[Conclusions]The level of serum uric acid in patients with bipolar disorder type I in their manic episode was higher than that in healthy subjects,and the decrease of serum uric acid level may be related to the improvement of clinical symptoms.

Tongdu Tiaoshen Acupuncture Method for Promoting Hippocampal Nerve Repair and Regeneration and Improving Learning and Memory Dysfunction in Stroke Model Rats

[Objectives]To evaluate the effects of Tongdu Tiaoshen Acupuncture method on improving learning and memory function and hippocampal nerve regeneration and repair after stroke.[Methods]Animal models of cognitive impairment in the recovery phase of cerebral infarction in rats were prepared,and rats were randomly divided into control group,model group,donepezil group,and acupuncture group.Morris water maze experiment was carried out to evaluate learning and memory ability.Nissl staining,BrdU and DCX immunofluorescence staining were used to observe the number and morphological structure of neuronal pyramidal and granular cells in the dentate gyrus(DG)area of the hippocampus,as well as the proliferation and differentiation of neural progenitor cells.TUNEL method was used to detect cell apoptosis.[Results]Before intervention,the scores and ELP of the model group,donepezil group and acupuncture group were significantly higher than those of the blank group(P<0.05);after the intervention,the neurological scores and ELP of the blank group and the model group were not significantly different from those before the intervention(P<0.05);the donepezil group and acupuncture group were significantly reduced,there was no difference between the donepezil group and acupuncture group(P>0.05),but it was still significantly higher than the blank group(P<0.05).The results of BrdU and DCX immunofluorescence staining showed that there were only a few BrdU positive cells and DCX protein in the hippocampal DG area of mice in the blank group,and the positive cells in the model group were significantly increased(P<0.05);donepezil group and acupuncture group showed significant increase compared with the model group(P<0.05);compared with the blank group,the number of neurons in the hippocampal DG area of each group was significantly reduced(P<0.01),and the neuronal apoptosis index was significantly increased(P<0.01);compared with the blank group,the number of neurons in the hippocampal DG area of each group was significantly reduced(P<0.01),and the neuronal apoptosis index was significantly increased(P<0.01).The number of neurons in the hippocampal DG area of rats in the acupuncture group and donepezil group was significantly increased(P<0.01),and the neuronal apoptosis index was significantly decreased(P<0.01).[Conclusions]Tongdu Tiaoshen Acupuncture method can improve the learning and memory function of stroke model rats by promoting the regeneration and repair of hippocampal nerve in rats with cerebral infarction.
