Ischemic stroke is a secondary cause of mortality worldwide,imposing considerable medical and economic burdens on society.Extracellular vesicles,serving as natural nanocarriers for drug delivery,exhibit excellent bioc...Ischemic stroke is a secondary cause of mortality worldwide,imposing considerable medical and economic burdens on society.Extracellular vesicles,serving as natural nanocarriers for drug delivery,exhibit excellent biocompatibility in vivo and have significant advantages in the management of ischemic stroke.However,the uncertain distribution and rapid clearance of extracellular vesicles impede their delivery efficiency.By utilizing membrane decoration or by encapsulating therapeutic cargo within extracellular vesicles,their delivery efficacy may be greatly improved.Furthermore,previous studies have indicated that microvesicles,a subset of large-sized extracellular vesicles,can transport mitochondria to neighboring cells,thereby aiding in the restoration of mitochondrial function post-ischemic stroke.Small extracellular vesicles have also demonstrated the capability to transfer mitochondrial components,such as proteins or deoxyribonucleic acid,or their sub-components,for extracellular vesicle-based ischemic stroke therapy.In this review,we undertake a comparative analysis of the isolation techniques employed for extracellular vesicles and present an overview of the current dominant extracellular vesicle modification methodologies.Given the complex facets of treating ischemic stroke,we also delineate various extracellular vesicle modification approaches which are suited to different facets of the treatment process.Moreover,given the burgeoning interest in mitochondrial delivery,we delved into the feasibility and existing research findings on the transportation of mitochondrial fractions or intact mitochondria through small extracellular vesicles and microvesicles to offer a fresh perspective on ischemic stroke therapy.展开更多

This study illustrates rehabilitative...Aim: Hip fracture implies severe problems to older people;special concerns regard persons with dementia, due either to cognitive impairment, or to behavioral and psychic symptoms. This study illustrates rehabilitative outcomes of these patients discharged by a special care unit ruled by “GentleCare” principles. Method: 54 patients [89% females, aged 82.3 years (range 66 - 94)] followed a post-surgery rehabilitative program carried out by a physiotherapist and an occupational therapist, supported by a psychologist. The multidimensional assessment consisted of cognition evaluation (Mini Mental State Examination, Clinical Dementia Rating, Global Deterioration Scale), functional evaluation (Barthel Index, Tinetti Gait and Balance, Bedford Alzheimer Nursing Severity scale), behavioral evaluation (UCLA Neuropsychiatric Inventory) and comorbidity evaluation (Cumulative Illness Rating Scale). Results: All parameters improved, including the 5 most frequent behavioral and psychic symptoms that usually preclude admission in ordinary rehabilitation units. 24% of improvement in Barthel Index total score was explained by agitation and apathy at discharge, in a multiple linear regression model: better functional levels corresponded to smoother behavioral problems. Most patients improved;70.5% of them were discharged to home. Conclusion: A prosthetic approach enables valuable results in the rehabilitation of severely demented patients with hip fracture also in presence of behavioral symptoms.展开更多
AIM: To assess the association of measured gait speed with hemodialysis (HD) patients’ hospitalization, in conjunction with, and apart from, recent fall history.METHODS: Gait speed was measured by a standard prot...AIM: To assess the association of measured gait speed with hemodialysis (HD) patients’ hospitalization, in conjunction with, and apart from, recent fall history.METHODS: Gait speed was measured by a standard protocol and falls during the past 12 mo were ascertained for a prevalent multi-center HD cohort (n = 668) aged 20-92. Hospitalization during the past 12 mo was identified in the patient’s clinic records, and the first hospitalization after gait speed assessment (or the competing event of death) was identifed in the 2013 United States Renal Data System Standard Analysis Files.characterized 34.7% of the patients, and 27.1% had experienced a recent fall. Patients with slow gait speed but without a history of recent falls were 1.79 times more likely to have been hospitalized during the past 12 mo (OR = 1.79, 95%CI: 1.11-2.88, P = 0.02), and patients with slow gait speed and a history of recent falls were over two times more likely to have been hospitalized (OR = 2.10, 95%CI: 1.19-3.73, P = 0.01), compared with patients having faster gait speed and no recent fall history. Prospective examination of gait speed/fall history status in relation to frst hospitaliza-tion (or death) incurred by the end of follow-up Decem-ber 31, 2011 also showed that slow gait speed was as-sociated with these events in conjunction with a history of falls (HR = 1.54, 95%CI: 1.04-2.30, P = 0.03).CONCLUSION: The International Task Force on Nutri-tion and Aging reported that gait speed is a powerful predictor for older adults of adverse outcomes such as hospitalization. The bottom surface sediments at the southern part of the Jordanian coast of the Gulf of Aqaba have been studied. Samples were collected from 10 m depth at nine coastal stations which are located within two main zones:...The bottom surface sediments at the southern part of the Jordanian coast of the Gulf of Aqaba have been studied. Samples were collected from 10 m depth at nine coastal stations which are located within two main zones: Marine Park Zone and Industrial Zone. Sedimentation rate, grain size oxygenation state, Loss on Ignition, hydrogen sulfide, calcium carbonate, total phosphorous, organic carbon, organic nitrogen, and heavy metals were analyzed to assess and evaluate the quality of bottom surface sediments. The sedimentation rate at the southern part of the Gulf has been affected by Khamassin winds from the south as a result of sand storm events in southern Jordan and adjacent regions. Floods and streams also played a vital role as a transportation agent of the sediments from the interstitial nearby areas to the Gulf. Also construction works played a minor role. Bottom surface sediments at the different sites were quite different in some chemical properties. Sediments from all stations other than the MSS were fine, gray-black and oxygen deficient. Sediments from the MSS and VC are whitish, better oxygenated and have higher calcium carbonate content since they are typically coral reef sediments coral reef sediments. Coral reef carbonate sediments are well known to have high organic nitrogen concentration as compared to silicate sediments, even those in close proximity. The concentration of phosphorous and calcium carbonate is attributed in the phosphate port to the phosphate by the generation of the dust and coral reef. Whilst, heavy metal concentration due to presence of major sources of metal pollution and intensive human activities. These include ship boat activities, domestic sewage discharges and phosphate rock particles from the phosphate port.展开更多

The objective was to determine the rate of compliance and t...Introduction: Primary open-angle glaucoma, a chronic, potentially blinding disease, requires lifelong medical treatment that demands full patient compliance. The objective was to determine the rate of compliance and to study the determinants of compliance in glaucoma patients followed at CADES/O. Patients and Methods: This was an observational study, which included old and new glaucoma patients. Compliance was assessed on the availability of medication, regularity of dosing schedules, compliance with prescribed doses, and regularity at check-ups. Compliance was judged to be good when at least four (4) of these criteria were met, fair when only three (3) of these criteria were met and poor when only two (2) of these criteria were met. Results: Compliance was considered good in 38.4% of cases, fair in 48.2% of cases and poor in 13.4% of cases with an overall compliance rate of 86.6%. The correlation was not statistically significant for age, sex, origin, length of illness, route of administration, instiller, or therapeutic regime. On the other hand, profession, level of education, presence of systemic arterial hypertension, compliance with instillation schedules, prescribed dosage, regularity of treatment, compliance with control visits and the climate of trust between patient and treating physician were the determining factors with the greatest statistical influence on compliance with treatment. Conclusion: Compliance with glaucoma treatment is linked to the patient factor but especially to the doctor factor. Doctors should take their time to explain to patients their disease, its evolution with or without treatment and above all the necessity of a good therapeutic compliance.展开更多

The pathological and biological characteristics of superficial spreading focal penetrating types, microcarcinoma and small carcinoma, "Pin-point cancer", multiple early cancer and gastric stump cancer were analysed. The authors believe that understanding these special types of early gastric cancer is of great importance not only in the study of histogenesis but also in clinical treatment and prevention of gastric cancer.展开更多

文摘通过焊接热模拟研究了在超大线能量下焊接时Al元素、Mg元素和Ti元素含量对EH36高强船板钢热影响区粗晶区组织、性能的影响规律,采用Thermo-Calc热力学计算与SEM,EDS测试相结合的方法揭示了Al元素、Mg元素和Ti元素含量与母材中氧化物类型、尺寸、数量及粗晶区相变的关系.结果表明,Al_(2)O_(3)无法诱导针状铁素体相变,当Al元素质量分数低于0.005%时,钢中可形成Mg元素、Ti元素或其复合氧化物,可促进粗晶区针状铁素体相变.Mg元素和Ti元素联合添加时,当Mg元素质量分数由0.0042%降低为0.0013%,氧化物类型由MgO转变为Mg_(2)TiO_(4),经统计20个视场内的氧化物数量由408个提高到503个,平均直径由1.37μm减小到1.10μm,显著提高了非均匀形核的比表面积,抑制了晶界铁素体的形成,使t8/5=300 s时粗晶区热模拟试样-20℃冲击吸收能量由43 J提升到127 J.
