BACKGROUND Postpartum ovarian vein thrombophlebitis(POVT)is a rare but serious postpartum complication that affects mostly postpartum women.A high index of suspicion is required when faced with sudden postpartum abdom...BACKGROUND Postpartum ovarian vein thrombophlebitis(POVT)is a rare but serious postpartum complication that affects mostly postpartum women.A high index of suspicion is required when faced with sudden postpartum abdominal pain.CASE SUMMARY A 25-year-old healthy woman who accepted a vaginal delivery procedure suffered fever(temperature 39.6℃)one day after delivery,accompanied with left lower abdominal pain.Physical examination indicated mild tenderness in the left lower abdomen,accompanied with rebound pain.The patient was confirmed to have left ovarian venous thrombosis with inflammation after receiving a multidetector row computed tomography scan.CONCLUSION POVT is a rare and dangerous postpartum complication.A high index of suspicion is required for the occurrence of ovarian venous thrombosis when faced with postpartum abdominal pain and fever.Early application of Doppler ultrasound,computed tomography,magnetic resonance imaging and other auxiliary examinations is conducive to timely and accurate diagnosis of POVT,thus reducing maternal mortality.展开更多

BACKGROUND Researchers have investigated the diagnostic value of protein induced by vitamin K absence or antagonist II (PIVKA-II) and alpha-fetoprotein (AFP) in hepatitis B virus (HBV)-related hepatocellular carcinoma...BACKGROUND Researchers have investigated the diagnostic value of protein induced by vitamin K absence or antagonist II (PIVKA-II) and alpha-fetoprotein (AFP) in hepatitis B virus (HBV)-related hepatocellular carcinoma (HCC), and obtained abundant clinical diagnostic data. However, PIVKA-II and AFP have unsatisfactory specificity and sensitivity in the diagnosis of early-stage HBV-related HCC. Gamma-glutamyltransferase (γ-GT) and aspartate aminotransferase (AST) are common biomarkers for evaluating liver function, and we hypothesized that the γ-GT/AST ratio in combination with PIVKA-II and AFP would improve the diagnosis of early-stage HBV-related HCC. AIM To evaluate the diagnostic value of γ-GT/AST ratio alone or in combination with PIVKA-II and AFP in HBV-related HCC. METHODS Serum levels of γ-GT, AST, PIVKA-II, and AFP were detected and analysed in 176 patients with HBV-related HCC and in 359 patients with chronic hepatitis B. According to tumour size and serum level of HBV DNA, HBV-related HCC patients were divided into the following categories: Early-stage HCC patients, HCC patients, HBV DNA positive (HBV DNA+) HCC patients, and HBV DNA negative (HBV DNA-) HCC patients. Receiver-operating characteristic (ROC) curves were used to analyse and compare the diagnostic value of the single and combined detection of various biomarkers in different types of HBV-related HCC. RESULTS Tumour size was positively correlated with serum levels of PIVKA-II and AFP in HCC patients (r = 0.529, aP < 0.001 and r = 0.270, bP < 0.001, respectively), but there was no correlation between tumour size and the γ-GT/AST ratio (r = 0.073, P = 0.336). The areas under the receiver-operating characteristic curves (AUROCs) of the γ-GT/AST ratio in early-stage HCC patients, HBV DNA+ HCC patients and HBV DNA- HCC patients were not significantly different from that in the total HCC patients (0.754, 0.802, and 0.705 vs 0.779, respectively;P > 0.05). When PIVKA-II was combined with the γ-GT/AST ratio in the diagnosis of earlystage HCC, HCC, and HBV DNA+ HCC, the AUROCs of PIVKA-II increased, with values of 0.857 vs 0.835, 0.925 vs 0.913, and 0.958 vs 0.954, respectively. When AFP was combined with the γ-GT/AST ratio in the diagnosis of early-stage HCC, HCC, HBV DNA+ HCC, and HBV DNA- HCC, the AUROCs of AFP increased, with values of 0.757 vs 0.621, 0.837 vs 0.744, 0.868 vs 0.757, and 0.840 vs 0.828, respectively. CONCLUSION The γ-GT/AST ratio may be better than PIVKA-II and AFP in the diagnosis of early-stage HBV-related HCC, and its combination with PIVKA-II and AFP can improve the diagnostic value for HBV-related HCC.展开更多

METHODS: We searched all the eligible studies from the Co...AIM: To compare transcatheter arterial chemoembolization (TACE) and 3D conformal radiotherapy (3D-CRT) with TACE monotherapy in hepatocellular carcinoma (HCC). METHODS: We searched all the eligible studies from the Cochrane Library, PubMed, Medline, Embase, and CNKI. The meta-analysis was performed to assess the survival benefit, tumor response, and the decline in alpha-fetoprotein (AFP) level. According to the heterogeneity of the studies, pooled OR with 95% CI were calculated using the fixed-effects or random-effects model. An observed OR > 1 indicated that the addition of 3D-CRT to TACE offered survival benefits to patients that could be considered statistically significant. Statistical analyses were performed using Review Manager Software. RESULTS: Ten studies met the criteria to perform a meta-analysis including 908 HCC participants, with 400 patients in the TACE/3D-CRT combination group and 508 in the TACE alone group. TACE combined with 3D-CRT significantly improved 1-, 2- and 3-year overall survival compared with TACE monotherapy (OR = 1.87, 95% CI: 1.37-2.55, P < 0.0001), (OR = 2.38, 95% CI: 1.78-3.17, P < 0.00001) and (OR = 2.97, 95% CI: 2.10-4.21, P < 0.00001). In addition, TACE plus 3DCRT was associated with a higher tumor response (complete remission and partial remission) (OR = 3.81; 95% CI: 2.70-5.37; P < 0.00001), and decline rates of AFP level (OR = 3.24, 95% CI: 2.09-5.02, P < 0.00001). CONCLUSION: This meta-analysis demonstrated that TACE combined with 3D-CRT was better than TACE monotherapy for patients with HCC, which needs to be confirmed by large multicenter trials. Objective Fine particulate matter(PM^(2.5))is an air pollutant that has become of great concern in recent years.Numerous studies have found that PM^(2.5)may contribute to lung cancer,but the pathogenesis has not yet b...Objective Fine particulate matter(PM^(2.5))is an air pollutant that has become of great concern in recent years.Numerous studies have found that PM^(2.5)may contribute to lung cancer,but the pathogenesis has not yet been fully elucidated.In this study,we explored the roles of exosomes from bronchial epithelial cells in PM^(2.5)-promoted lung cancer metastasis.Methods Exosomes were isolated from cell supernatants.An animal model of lung metastasis(established by tail vein injection of A549-luc)and in vitro studies with lung cancer cell lines were used to investigate the effects of exosomes derived from PM^(2.5)-treated human bronchial epithelial cells(PHBE-exo).Results The animal experiments revealed that PHBE-exo-treated mice showed stronger luciferase activity and a larger relative metastatic region in the lungs,thus indicating that PHBE-exo promoted the metastatic potential of lung cancer.Additionally,PHBE-exo promoted the migration,invasion and epithelial-to-mesenchymal transition of lung cancer cells,in a manner mediated by activation of c-Jun Nterminal kinase.Conclusion These results implied that PM^(2.5)may promote the development of lung cancer through exosomes derived from bronchial epithelial cells,thus providing a potential interventional target for lung cancer.These findings broadened our understanding of cancer-promoting mechanisms of environmental pollutants.展开更多

Serum was collected to detect the content of MR-ProANP, NT-ProBNP as well as ventricular remodeling-related molecules, and cardiac color Doppler ultrasonography was conducted to determine the heart pump function- and myocardial remodeling-related parameters.Results:Serum MR-ProANP, NT-ProBNP, MMP2, MMP9, MMP10, PICP and PIIINP content as well as ultrasound parameters IVSd, PWT, LVMI, MWS levels of heart failure group were significantly higher than those of control group, ultrasound parameters LAEF, LAPEF and LAAEF levels were significantly lower than those of control group;serum MR-ProANP and NT-ProBNP content were positively correlated with ultrasound parameters IVSd, PWT, LVMI, MWS as well as serum MMP2, MMP9, MMP10, PICP and PIIINP content, negatively correlated with ultrasound parameters LAEF, LAPEF and LAAEF, and the correlation coefficients of MR-ProANP with these indexes were all higher than those of NT-ProBNP.Conclusion:Serum MR-ProANP and NT-ProBNP content in patients with left heart failure are closely related to the pump function and ventricular remodeling, and MR-ProANP is more valuable for evaluating pump function decrease and ventricular remodeling increase.展开更多
Background In full-thickness nasal defects,the internal lining is perhaps the most challenging aspect of the three layers to rebuild.Nasal damage is usually more concentrated on the tip,soft triangles,alar wings,and c...Background In full-thickness nasal defects,the internal lining is perhaps the most challenging aspect of the three layers to rebuild.Nasal damage is usually more concentrated on the tip,soft triangles,alar wings,and columella,but the lateral nasal arteries are often left intact and the damage to the dorsal sidewalls are normally superficial.Methods Twelve patients who required total nasal reconstruction received a forehead flap placement as external coverage and autologous rib cartilage as structural support.Residual normal/superficial scar tissue flaps on the dorsal sidewalls with lateral nasal artery pedicles were mobilized and designed for internal lining repair without creating secondary donor site damage.The flaps were then turned 180°downward and placed between the alar medial angles and the fornix.Results No total lining flap necrosis occurred in all the patients.Partial necrosis occurred on the distal edge owing to overpressure of the nostril splint to the flaps;however,the wounds eventually healed,and the nasal structural integrity was preserved.The patients were satisfied with the aesthetic results and had no complaints of airway stenosis.Conclusions Lateral nasal artery pedicle dorsal sidewall skin flaps are appropriately thick,providing enough nostril circumferential support to improve airway stenosis.It allows sufficient blood supply and creates no extra donor site damage.Blood vessels and skin flaps are often undamaged,thereby allowing maximum application in total nasal reconstruction.展开更多

The cerebrospinal fluid samples of two groups were taken and SP-D, brain injury-related indicators, inflammatory indicators and T lymphocyte subsets were detected. Pearson test was used to analyze the relationship between SP-D content in CSF and above indicators.Results: Content of SP-D in CSF of Viral encephalitis group was lower than that of control group (P<0.05). Contents of MBP, CK-BB, NSE and S100B in CSF of Viral encephalitis group were higher than those of Control group;contents of IL-1β, TNF-αand ICAM-1 in CSF of Viral encephalitis group were higher than those of Control group;distribution proportion of CD3+, CD4+ T lymphocytes and CD4+/CD8+ ratio in CSF were lower than those of Control group, and levels of CD8+ were higher than that of Control group (P<0.05). Correlation analysis showed that, SP-D content in CSF of viral encephalitis children was directly related to brain injury indexes, inflammatory indexes and T lymphocyte subsets distribution levels(P<0.05).Conclusion: Content of SP-D in CSF of viral encephalitis children decreases abnormally, the specific content is directly related to the degree of brain injury, intracranial inflammation and cellular immune dysfunction. Methods Before operation the area and quantity resected bone were designed according to facial measurement, mandible pantomagraphy and orthophoria and lateral localized radiograph of skull. The Incises of mandibular angle ostectomy included intraoral, retroauricular or intraoral associated with retroauricular. Howerer, the sagittal resection of mandible outer table was necessary in all intraoral incise. Results Single mandibular angle ostectomy was not satisfactory for the patients having mandible hypertrophy with over-width basifacial contouring. Mandibular angle ostectomy combined with the sagittal resection of outer table of mandibular angle were required. Good symmetry and appearance were gained in 206 cases. One case had facial paralysis. Two patients occured mandibular fracture during the operation. Three cases complicated angled deformity at mandible body. Conclusion Reduction mandibuloplasty should be selected depends on varied types of mandibular angle hypertrophy before operation.展开更多

Using sodium fluoride and Tea polyphenol as antimicrobial agent and Streptococcus mu-tans as target, sensitivity tests were studied. Results The modeling biofilm assay resulted in a MIC of 1. 28mg/ ml for fluoride against S. mutans, which was 32 times the MIC for broth maco-dilution method. The differential resistance of bacteria bioflim to antimicrobial agent relative to planktonic cells was also demonstrated. Conclusion The biofilm artificial mouth model may be useful in oral products test.展开更多

Methods Tibialis anterior pediculated triple flaps ( TAPTF) including distal lateral leg, dorsalis pedis flaps for skin defect covering and trimmed hallux for thumb reconstruction were presented. Results Mutilated hands in five patients were repaired by the technique with acceptable cosmetic and functional success. A three to twelve months follow-up in three patients resulted in the recovery of protective thermal sensation and pinch action of the reconstructed thumb. The donor sites were free from major disability. Conclusion TAPTF is ideal for one stage reconstruction of the mutilated hand with loss of digits.展开更多
Emphysema,myofibroblast accumulation and airway remodeling can occur in the lungs due to exposure to atmospheric pollution,especially fine particulate matter(PM_(2.5)),leading to chronic obstructive pulmonary disease(...Emphysema,myofibroblast accumulation and airway remodeling can occur in the lungs due to exposure to atmospheric pollution,especially fine particulate matter(PM_(2.5)),leading to chronic obstructive pulmonary disease(COPD).Specifically,bronchial epithelium-fibroblast communication participates in airway remodeling,which results in COPD.An increasing number of studies are now being conducted on the role of exosome-mediated cell-cell communication in disease pathogenesis.Here,we investigated whether exosomes generated from bronchial epithelial cells could deliver information to normal stromal fibroblasts and provoke cellular responses,resulting in airway obstruction in COPD.We studied the mechanism of exosome-mediated intercellular communication between human bronchial epithelial(HBE) cells and primary lung fibroblasts(pLFs).We found that PM_(2.5)-induced HBE-derived exosomes promoted myofibroblast differentiation in pLFs.Then,the exosomal lncRNA expression profiles derived from PM_(2.5)-treated HBE cells and nontreated HBE cells were investigated using an Agilent Human LncRNA Array.Combining coculture assays and direct exosome treatment,we found that HBE cell-derived exosomal HOTAIRM1 facilitated the myofibroblast differentiation of pLFs.Surprisingly,we discovered that exosomal HOTAIRM1 enhanced p LF proliferation to secrete excessive collagen secretion,leading to airway obstruction by stimulating the TGF-β/SMAD3 signaling pathway.Significantly,PM_(2.5)reduced FEV1/FVC and FEV1 and increased the level of serum exosomal HOTAIRM1 in healthy people;moreover,serum exosomal HOTAIRM1 was associated with PM_(2.5)-related reductions in FEV1/FVC and FVC.These findings show that PM_(2.5)triggers alterations in exosome components and clarify that one of the paracrine mediators of myofibroblast differentiation is bronchial epithelial cell-derived HOTAIRM1,which has the potential to be an effective prevention and therapeutic target for PM_(2.5)-induced COPD.展开更多
Natural enzymes,such as horseradish peroxidase(HRP),are a class of important biocatalysts with the high specificity,but their catalytic efficiency is usually unsatisfactory.Thus,the higher catalytic efficiency induced...Natural enzymes,such as horseradish peroxidase(HRP),are a class of important biocatalysts with the high specificity,but their catalytic efficiency is usually unsatisfactory.Thus,the higher catalytic efficiency induced by the confinement effect is promising in optical sensing systems.In this work,a dark-field light scattering sensing platform was fabricated by the confinement effect of HRP from hybridization chain reaction(HCR)and then released to solution by the toehold-mediated strand displacement reaction(TSDR).Then,HRP catalyzed the 3,3,5,5-tetramethylbenzidine(TMB)to TMB^(2+)with the assistance of hydrogen peroxide,which etched the gold nanorods(Au NRs)with the weakened light scattering.The single-particle assay was established based on the decreased light scattering intensity of AuNRs under dark-field microscope.The proposed assay revealed excellent analytical performance within a linear range from 25 pmol/L to 600 pmol/L,and a low limit of detection of 3.12 pmol/L.Additionally,it also manifested satisfactory recovery of mi RNA-21 in human serum samples.The high sensitivity,excellent specificity,and universal applicability make this sensing platform promising for disease diagnosis.展开更多
Previous studies have shown that exposure to black carbon(BC,a tracer of traffic-related air pollution)and psychosocial stress are both associated with adverse cardiac effects,but whether psychosocial stress could mod...Previous studies have shown that exposure to black carbon(BC,a tracer of traffic-related air pollution)and psychosocial stress are both associated with adverse cardiac effects,but whether psychosocial stress could modify the cardiac effects of BC is unclear.To investigate the potential modifying effect of psychosocial stress on the associations between acute exposure to BC and typical cardiac health variables,real-time personal 24 h measurements were conducted in a repeated-measure study among adults with elevated blood pressure(high-risk group)and a panel study among normal adults(low-risk group)in China.Measured cardiac health variables included ST-segment depression events,heart rate,and heart rate variability(HRV)variables.Perceived Stress Scale,State Anxiety Inventory and Self-rating Depression Scale were used to assess the recent psychosocial stress status of the participants,and a composite stress index was established based on these scales.Generalized linear mixed-effects model was used to analyze the associations between BC exposure and cardiac health variables and potential effect modification by psychosocial stress.A total of 9724 h measurements among 97 participants in the repeated-measure study and 20224 h measurements among 87 participants in the panel study were included in the final analysis.Acute BC exposure was significantly associated with increased ST-segment depression events and heart rate and decreases in HRV in both studies.The marginal effects of acute BC exposure on most cardiac health variables generally tended to be amplified under higher vs low levels of psychosocial stress in both studies,with the composite stress index apparently modifying the associations of BC exposure with most ST-segment depression events and HRV variables.These findings suggest that psychosocial stress may increase the participants’cardiac susceptibility to BC exposure,which could be helpful for the identification of susceptible individuals in the context of traffic-related air pollution.展开更多
Background:Dual regimen dolutegravir(DTG)plus lamivudine(3TC)has demonstrated non-inferior efficacy compared to DTG-based three-drug regimens(3DRs),yet directly comparative data regarding the efficacy and safety of DT...Background:Dual regimen dolutegravir(DTG)plus lamivudine(3TC)has demonstrated non-inferior efficacy compared to DTG-based three-drug regimens(3DRs),yet directly comparative data regarding the efficacy and safety of DTG+3TC and bictegravir/emtricitabine/tenofovir alafenamide(B/F/TAF)for therapy-naïve people with human immunodeficiency virus(HIV)-1(PWH)are still limited.We aimed to assess the antiviral potency and safety profiles of DTG+3TC vs.B/F/TAF based on antiretroviral therapy(ART)-naïve PWH in China.Methods:This retrospective multicenter study enrolled PWH initiating ART with DTG+3TC or B/F/TAF from 2020 to 2022 in Guangdong and Guangxi.We analyzed response rates based on target not detected(TND)status using intention-to-treat(ITT)analysis.Subgroups were formed based on baseline viral load(VL)(<100,000 vs.≥100,000 copies/mL)and CD4^(+)cell count(<200 vs.≥200 cell/µL).Median time to TND VL was assessed by Kaplan-Meier method.We also measured changes from baseline in CD4^(+)cell counts,CD4/CD8 ratio,lipid parameters,weight,creatinine(Cr),estimated glomerular filtration rate(eGFR),and drug-related adverse effects(DRAEs).Results:We enrolled 280 participants,including 137(48.9%)on DTG+3TC and 143(51.1%)on B/F/TAF.At week 48,96.4%(132/137)on DTG+3TC and 100%(143/143)on B/F/TAF achieved TND(P=0.064).At week 12,TND responses were higher with B/F/TAF(78.3%[112/143])than DTG+3TC(30.7%[42/137])(P<0.001).This trend held across subgroups.B/F/TAF achieved TND faster(12 weeks)than DTG+3TC(24 weeks)(P<0.001).No differences were seen in CD4^(+)cell count and CD4/CD8 ratio,except in the high-VL subgroup,where B/F/TAF showed better recovery.DRAEs were significantly lower with B/F/TAF(4.9%[7/143])than with DTG+3TC(13.1%[18/137])(P=0.016).Lipid parameters,body weight,and Cr increased in both groups over 48 weeks,with DTG+3TC showing a more favorable effect on triglycerides,high-density lipoprotein(HDL)cholesterol,and weight gain.Conclusions:In this real-life study,B/F/TAF led to a faster viral decline and fewer DRAEs compared to DTG+3TC.No significant difference was observed in the TND rate at week 48,regardless of baseline VL and CD4^(+)cell count.CD4^(+)recovery was superior for B/F/TAF in participants with high VL.The DTG+3TC regimen had less impact on metabolic changes than B/F/TAF.展开更多
Achieving efficient hemostasis and wound management is vital to preserve life and restore health in case of extensive hemor-rhagic skin damage.Here,we develop a filter pump-like hierarchical porous-structure(HPS)dress...Achieving efficient hemostasis and wound management is vital to preserve life and restore health in case of extensive hemor-rhagic skin damage.Here,we develop a filter pump-like hierarchical porous-structure(HPS)dressing based on a non-woven substrate,konjac glucomannan(KGM)aerogel,and bi-functional microporous starch(BMS).The KGM aerogel intercalates into the non-woven network structure,forming a hydrophilic frame to stimulate the plasma permeation toward the interior in synergy with the hydrophilic pores of the BMS.The BMS surface forms a hydrophobic matrix that fills the spaces of the KGM hydrophilic frame,contributing to the isolation and aggregation of blood cells on the surface of the HPS dressing to establish rapid hemostasis.Animal model experiments suggest reliable HPS dressing hemostatic capacity,as it is able to stop ear artery and liver bleeding within 97.6±15.2 s and 67.8±5.4 s,respectively.Furthermore,the dressings exhibit antibacterial properties and enabled wound healing within 2 weeks.In vitro hemolysis and cytotoxicity tests also confirm the biocompatibility of HPS dressings.This novel“two-in-one”hemostatic dressing facilitates tissue repair of bleeding wounds over the entire recovery period,thereby providing a convenient strategy for wound management.展开更多

文摘BACKGROUND Postpartum ovarian vein thrombophlebitis(POVT)is a rare but serious postpartum complication that affects mostly postpartum women.A high index of suspicion is required when faced with sudden postpartum abdominal pain.CASE SUMMARY A 25-year-old healthy woman who accepted a vaginal delivery procedure suffered fever(temperature 39.6℃)one day after delivery,accompanied with left lower abdominal pain.Physical examination indicated mild tenderness in the left lower abdomen,accompanied with rebound pain.The patient was confirmed to have left ovarian venous thrombosis with inflammation after receiving a multidetector row computed tomography scan.CONCLUSION POVT is a rare and dangerous postpartum complication.A high index of suspicion is required for the occurrence of ovarian venous thrombosis when faced with postpartum abdominal pain and fever.Early application of Doppler ultrasound,computed tomography,magnetic resonance imaging and other auxiliary examinations is conducive to timely and accurate diagnosis of POVT,thus reducing maternal mortality.

文摘BACKGROUND Researchers have investigated the diagnostic value of protein induced by vitamin K absence or antagonist II (PIVKA-II) and alpha-fetoprotein (AFP) in hepatitis B virus (HBV)-related hepatocellular carcinoma (HCC), and obtained abundant clinical diagnostic data. However, PIVKA-II and AFP have unsatisfactory specificity and sensitivity in the diagnosis of early-stage HBV-related HCC. Gamma-glutamyltransferase (γ-GT) and aspartate aminotransferase (AST) are common biomarkers for evaluating liver function, and we hypothesized that the γ-GT/AST ratio in combination with PIVKA-II and AFP would improve the diagnosis of early-stage HBV-related HCC. AIM To evaluate the diagnostic value of γ-GT/AST ratio alone or in combination with PIVKA-II and AFP in HBV-related HCC. METHODS Serum levels of γ-GT, AST, PIVKA-II, and AFP were detected and analysed in 176 patients with HBV-related HCC and in 359 patients with chronic hepatitis B. According to tumour size and serum level of HBV DNA, HBV-related HCC patients were divided into the following categories: Early-stage HCC patients, HCC patients, HBV DNA positive (HBV DNA+) HCC patients, and HBV DNA negative (HBV DNA-) HCC patients. Receiver-operating characteristic (ROC) curves were used to analyse and compare the diagnostic value of the single and combined detection of various biomarkers in different types of HBV-related HCC. RESULTS Tumour size was positively correlated with serum levels of PIVKA-II and AFP in HCC patients (r = 0.529, aP < 0.001 and r = 0.270, bP < 0.001, respectively), but there was no correlation between tumour size and the γ-GT/AST ratio (r = 0.073, P = 0.336). The areas under the receiver-operating characteristic curves (AUROCs) of the γ-GT/AST ratio in early-stage HCC patients, HBV DNA+ HCC patients and HBV DNA- HCC patients were not significantly different from that in the total HCC patients (0.754, 0.802, and 0.705 vs 0.779, respectively;P > 0.05). When PIVKA-II was combined with the γ-GT/AST ratio in the diagnosis of earlystage HCC, HCC, and HBV DNA+ HCC, the AUROCs of PIVKA-II increased, with values of 0.857 vs 0.835, 0.925 vs 0.913, and 0.958 vs 0.954, respectively. When AFP was combined with the γ-GT/AST ratio in the diagnosis of early-stage HCC, HCC, HBV DNA+ HCC, and HBV DNA- HCC, the AUROCs of AFP increased, with values of 0.757 vs 0.621, 0.837 vs 0.744, 0.868 vs 0.757, and 0.840 vs 0.828, respectively. CONCLUSION The γ-GT/AST ratio may be better than PIVKA-II and AFP in the diagnosis of early-stage HBV-related HCC, and its combination with PIVKA-II and AFP can improve the diagnostic value for HBV-related HCC.
文摘AIM: To compare transcatheter arterial chemoembolization (TACE) and 3D conformal radiotherapy (3D-CRT) with TACE monotherapy in hepatocellular carcinoma (HCC). METHODS: We searched all the eligible studies from the Cochrane Library, PubMed, Medline, Embase, and CNKI. The meta-analysis was performed to assess the survival benefit, tumor response, and the decline in alpha-fetoprotein (AFP) level. According to the heterogeneity of the studies, pooled OR with 95% CI were calculated using the fixed-effects or random-effects model. An observed OR > 1 indicated that the addition of 3D-CRT to TACE offered survival benefits to patients that could be considered statistically significant. Statistical analyses were performed using Review Manager Software. RESULTS: Ten studies met the criteria to perform a meta-analysis including 908 HCC participants, with 400 patients in the TACE/3D-CRT combination group and 508 in the TACE alone group. TACE combined with 3D-CRT significantly improved 1-, 2- and 3-year overall survival compared with TACE monotherapy (OR = 1.87, 95% CI: 1.37-2.55, P < 0.0001), (OR = 2.38, 95% CI: 1.78-3.17, P < 0.00001) and (OR = 2.97, 95% CI: 2.10-4.21, P < 0.00001). In addition, TACE plus 3DCRT was associated with a higher tumor response (complete remission and partial remission) (OR = 3.81; 95% CI: 2.70-5.37; P < 0.00001), and decline rates of AFP level (OR = 3.24, 95% CI: 2.09-5.02, P < 0.00001). CONCLUSION: This meta-analysis demonstrated that TACE combined with 3D-CRT was better than TACE monotherapy for patients with HCC, which needs to be confirmed by large multicenter trials. 文摘Objective Fine particulate matter(PM^(2.5))is an air pollutant that has become of great concern in recent years.Numerous studies have found that PM^(2.5)may contribute to lung cancer,but the pathogenesis has not yet been fully elucidated.In this study,we explored the roles of exosomes from bronchial epithelial cells in PM^(2.5)-promoted lung cancer metastasis.Methods Exosomes were isolated from cell supernatants.An animal model of lung metastasis(established by tail vein injection of A549-luc)and in vitro studies with lung cancer cell lines were used to investigate the effects of exosomes derived from PM^(2.5)-treated human bronchial epithelial cells(PHBE-exo).Results The animal experiments revealed that PHBE-exo-treated mice showed stronger luciferase activity and a larger relative metastatic region in the lungs,thus indicating that PHBE-exo promoted the metastatic potential of lung cancer.Additionally,PHBE-exo promoted the migration,invasion and epithelial-to-mesenchymal transition of lung cancer cells,in a manner mediated by activation of c-Jun Nterminal kinase.Conclusion These results implied that PM^(2.5)may promote the development of lung cancer through exosomes derived from bronchial epithelial cells,thus providing a potential interventional target for lung cancer.These findings broadened our understanding of cancer-promoting mechanisms of environmental pollutants.
文摘Objective:To study the correlation of serum MR-ProANP and NT-ProBNP content with pump function and ventricular remodeling in patients with left heart failure.Methods:A total of 104 patients with chronic heart failure treated in our hospital between May 2013 and May 2016 were selected as the heart failure group of the research and 95 healthy volunteers receiving physical examination in our hospital over the same period were selected as the control group of the research. Serum was collected to detect the content of MR-ProANP, NT-ProBNP as well as ventricular remodeling-related molecules, and cardiac color Doppler ultrasonography was conducted to determine the heart pump function- and myocardial remodeling-related parameters.Results:Serum MR-ProANP, NT-ProBNP, MMP2, MMP9, MMP10, PICP and PIIINP content as well as ultrasound parameters IVSd, PWT, LVMI, MWS levels of heart failure group were significantly higher than those of control group, ultrasound parameters LAEF, LAPEF and LAAEF levels were significantly lower than those of control group;serum MR-ProANP and NT-ProBNP content were positively correlated with ultrasound parameters IVSd, PWT, LVMI, MWS as well as serum MMP2, MMP9, MMP10, PICP and PIIINP content, negatively correlated with ultrasound parameters LAEF, LAPEF and LAAEF, and the correlation coefficients of MR-ProANP with these indexes were all higher than those of NT-ProBNP.Conclusion:Serum MR-ProANP and NT-ProBNP content in patients with left heart failure are closely related to the pump function and ventricular remodeling, and MR-ProANP is more valuable for evaluating pump function decrease and ventricular remodeling increase.
文摘Background In full-thickness nasal defects,the internal lining is perhaps the most challenging aspect of the three layers to rebuild.Nasal damage is usually more concentrated on the tip,soft triangles,alar wings,and columella,but the lateral nasal arteries are often left intact and the damage to the dorsal sidewalls are normally superficial.Methods Twelve patients who required total nasal reconstruction received a forehead flap placement as external coverage and autologous rib cartilage as structural support.Residual normal/superficial scar tissue flaps on the dorsal sidewalls with lateral nasal artery pedicles were mobilized and designed for internal lining repair without creating secondary donor site damage.The flaps were then turned 180°downward and placed between the alar medial angles and the fornix.Results No total lining flap necrosis occurred in all the patients.Partial necrosis occurred on the distal edge owing to overpressure of the nostril splint to the flaps;however,the wounds eventually healed,and the nasal structural integrity was preserved.The patients were satisfied with the aesthetic results and had no complaints of airway stenosis.Conclusions Lateral nasal artery pedicle dorsal sidewall skin flaps are appropriately thick,providing enough nostril circumferential support to improve airway stenosis.It allows sufficient blood supply and creates no extra donor site damage.Blood vessels and skin flaps are often undamaged,thereby allowing maximum application in total nasal reconstruction.
文摘Wound healing requires abundant nutrition and blood supply, thus angiogenesis is a vital stage in this process. Angiogenesis involves diverse kinds of immune cells, growth factors, cytokines and inhibitors. ADSCs, especially ADSCs cultured in hypoxic condition, are reported to be able to facilitate angiogenesis and promote wound healing process. Significant efforts have been made on the development of ADSCs-based therapies with wound-healing applications. Here the results showed that expression of Angiopoietin-2 (ANGPT2) in ADSCs was up-regulated in the hypoxic condition.
文摘Objective:To detect the content of SP-D in cerebrospinal fluid(CSF) of children with viral encephalitis, evaluate the correlation between SP-D and indexes of brain injury, inflammatory reaction and cellular immune disorder.Methods: 78 cases of viral encephalitis children diagnosed in our hospital during December 2016 to May 2018 were selected as Viral encephalitis group, 51 cases of fever without intracranial infection children were selected as Control group. The cerebrospinal fluid samples of two groups were taken and SP-D, brain injury-related indicators, inflammatory indicators and T lymphocyte subsets were detected. Pearson test was used to analyze the relationship between SP-D content in CSF and above indicators.Results: Content of SP-D in CSF of Viral encephalitis group was lower than that of control group (P<0.05). Contents of MBP, CK-BB, NSE and S100B in CSF of Viral encephalitis group were higher than those of Control group;contents of IL-1β, TNF-αand ICAM-1 in CSF of Viral encephalitis group were higher than those of Control group;distribution proportion of CD3+, CD4+ T lymphocytes and CD4+/CD8+ ratio in CSF were lower than those of Control group, and levels of CD8+ were higher than that of Control group (P<0.05). Correlation analysis showed that, SP-D content in CSF of viral encephalitis children was directly related to brain injury indexes, inflammatory indexes and T lymphocyte subsets distribution levels(P<0.05).Conclusion: Content of SP-D in CSF of viral encephalitis children decreases abnormally, the specific content is directly related to the degree of brain injury, intracranial inflammation and cellular immune dysfunction. It is important to evaluate the severity of the disease.
文摘Colorectal laterally spreading tumor (LST) is a flat protuberant lesion originating from the colorectal mucosa, which is mainly charactherized by lateral superficial diffusion, and rarely vertically invades the deep wall of the intestine. LST is closely related to the occurrence and development of colorectal cancer, but its pathological morphology and molecular mechanism are different from those of general adenoma, and it has some particularity. At present, the related research on LST in China has just started, but we find that its incidence is not low. Therefore, the research progress of epidemiology, molecular biology as well as clinical diagnosis and treatment of this disease is reviewed.
文摘Lactating mastitis is usually caused by pathogenic bacteria infection and milk stasis, but it has been found that the transformation of the bacterial community of breast milk from cocolonization to infection leads to the incidence of mastitis. Milk probiotics and opportunistic bacteria and other living bacteria sources, in addition to the invasion of foreign bacteria, but also a part of the maternal intestinal tract. Because of the pathway of specific intestinal bacteria to the mammary gland and the symbiotic advantage of lactobacillus and human intestinal tract, oral probiotics to prevent and improve the supplemental treatment of mastitis has achieved significant efficacy. Therefore, this paper reviews the literature on the pathogenesis and treatment of dysbacteriosis in order to provide a new direction for its prevention and treatment.
文摘Objective Mandibular angle ostectomy is usually applied to the facial contouring sculpture. We evaluated the various techniques in order to enhance the precision and avoid unnecessary damage. Methods Before operation the area and quantity resected bone were designed according to facial measurement, mandible pantomagraphy and orthophoria and lateral localized radiograph of skull. The Incises of mandibular angle ostectomy included intraoral, retroauricular or intraoral associated with retroauricular. Howerer, the sagittal resection of mandible outer table was necessary in all intraoral incise. Results Single mandibular angle ostectomy was not satisfactory for the patients having mandible hypertrophy with over-width basifacial contouring. Mandibular angle ostectomy combined with the sagittal resection of outer table of mandibular angle were required. Good symmetry and appearance were gained in 206 cases. One case had facial paralysis. Two patients occured mandibular fracture during the operation. Three cases complicated angled deformity at mandible body. Conclusion Reduction mandibuloplasty should be selected depends on varied types of mandibular angle hypertrophy before operation.
文摘Objective To develop a new antimicrobial sensitivity test model for oral products in vitro.Methods A biofilm artificial mouth model for antimicrobial sensitivity tests was established by modifying the LKB chromatography chamber. Using sodium fluoride and Tea polyphenol as antimicrobial agent and Streptococcus mu-tans as target, sensitivity tests were studied. Results The modeling biofilm assay resulted in a MIC of 1. 28mg/ ml for fluoride against S. mutans, which was 32 times the MIC for broth maco-dilution method. The differential resistance of bacteria bioflim to antimicrobial agent relative to planktonic cells was also demonstrated. Conclusion The biofilm artificial mouth model may be useful in oral products test.
文摘Objective To introduce a one stage technique in the reconstruction of mutilated hand with lossof thumb and eventual digits. Methods Tibialis anterior pediculated triple flaps ( TAPTF) including distal lateral leg, dorsalis pedis flaps for skin defect covering and trimmed hallux for thumb reconstruction were presented. Results Mutilated hands in five patients were repaired by the technique with acceptable cosmetic and functional success. A three to twelve months follow-up in three patients resulted in the recovery of protective thermal sensation and pinch action of the reconstructed thumb. The donor sites were free from major disability. Conclusion TAPTF is ideal for one stage reconstruction of the mutilated hand with loss of digits.

文摘Emphysema,myofibroblast accumulation and airway remodeling can occur in the lungs due to exposure to atmospheric pollution,especially fine particulate matter(PM_(2.5)),leading to chronic obstructive pulmonary disease(COPD).Specifically,bronchial epithelium-fibroblast communication participates in airway remodeling,which results in COPD.An increasing number of studies are now being conducted on the role of exosome-mediated cell-cell communication in disease pathogenesis.Here,we investigated whether exosomes generated from bronchial epithelial cells could deliver information to normal stromal fibroblasts and provoke cellular responses,resulting in airway obstruction in COPD.We studied the mechanism of exosome-mediated intercellular communication between human bronchial epithelial(HBE) cells and primary lung fibroblasts(pLFs).We found that PM_(2.5)-induced HBE-derived exosomes promoted myofibroblast differentiation in pLFs.Then,the exosomal lncRNA expression profiles derived from PM_(2.5)-treated HBE cells and nontreated HBE cells were investigated using an Agilent Human LncRNA Array.Combining coculture assays and direct exosome treatment,we found that HBE cell-derived exosomal HOTAIRM1 facilitated the myofibroblast differentiation of pLFs.Surprisingly,we discovered that exosomal HOTAIRM1 enhanced p LF proliferation to secrete excessive collagen secretion,leading to airway obstruction by stimulating the TGF-β/SMAD3 signaling pathway.Significantly,PM_(2.5)reduced FEV1/FVC and FEV1 and increased the level of serum exosomal HOTAIRM1 in healthy people;moreover,serum exosomal HOTAIRM1 was associated with PM_(2.5)-related reductions in FEV1/FVC and FVC.These findings show that PM_(2.5)triggers alterations in exosome components and clarify that one of the paracrine mediators of myofibroblast differentiation is bronchial epithelial cell-derived HOTAIRM1,which has the potential to be an effective prevention and therapeutic target for PM_(2.5)-induced COPD.

文摘Natural enzymes,such as horseradish peroxidase(HRP),are a class of important biocatalysts with the high specificity,but their catalytic efficiency is usually unsatisfactory.Thus,the higher catalytic efficiency induced by the confinement effect is promising in optical sensing systems.In this work,a dark-field light scattering sensing platform was fabricated by the confinement effect of HRP from hybridization chain reaction(HCR)and then released to solution by the toehold-mediated strand displacement reaction(TSDR).Then,HRP catalyzed the 3,3,5,5-tetramethylbenzidine(TMB)to TMB^(2+)with the assistance of hydrogen peroxide,which etched the gold nanorods(Au NRs)with the weakened light scattering.The single-particle assay was established based on the decreased light scattering intensity of AuNRs under dark-field microscope.The proposed assay revealed excellent analytical performance within a linear range from 25 pmol/L to 600 pmol/L,and a low limit of detection of 3.12 pmol/L.Additionally,it also manifested satisfactory recovery of mi RNA-21 in human serum samples.The high sensitivity,excellent specificity,and universal applicability make this sensing platform promising for disease diagnosis.

文摘Previous studies have shown that exposure to black carbon(BC,a tracer of traffic-related air pollution)and psychosocial stress are both associated with adverse cardiac effects,but whether psychosocial stress could modify the cardiac effects of BC is unclear.To investigate the potential modifying effect of psychosocial stress on the associations between acute exposure to BC and typical cardiac health variables,real-time personal 24 h measurements were conducted in a repeated-measure study among adults with elevated blood pressure(high-risk group)and a panel study among normal adults(low-risk group)in China.Measured cardiac health variables included ST-segment depression events,heart rate,and heart rate variability(HRV)variables.Perceived Stress Scale,State Anxiety Inventory and Self-rating Depression Scale were used to assess the recent psychosocial stress status of the participants,and a composite stress index was established based on these scales.Generalized linear mixed-effects model was used to analyze the associations between BC exposure and cardiac health variables and potential effect modification by psychosocial stress.A total of 9724 h measurements among 97 participants in the repeated-measure study and 20224 h measurements among 87 participants in the panel study were included in the final analysis.Acute BC exposure was significantly associated with increased ST-segment depression events and heart rate and decreases in HRV in both studies.The marginal effects of acute BC exposure on most cardiac health variables generally tended to be amplified under higher vs low levels of psychosocial stress in both studies,with the composite stress index apparently modifying the associations of BC exposure with most ST-segment depression events and HRV variables.These findings suggest that psychosocial stress may increase the participants’cardiac susceptibility to BC exposure,which could be helpful for the identification of susceptible individuals in the context of traffic-related air pollution.
文摘Background:Dual regimen dolutegravir(DTG)plus lamivudine(3TC)has demonstrated non-inferior efficacy compared to DTG-based three-drug regimens(3DRs),yet directly comparative data regarding the efficacy and safety of DTG+3TC and bictegravir/emtricitabine/tenofovir alafenamide(B/F/TAF)for therapy-naïve people with human immunodeficiency virus(HIV)-1(PWH)are still limited.We aimed to assess the antiviral potency and safety profiles of DTG+3TC vs.B/F/TAF based on antiretroviral therapy(ART)-naïve PWH in China.Methods:This retrospective multicenter study enrolled PWH initiating ART with DTG+3TC or B/F/TAF from 2020 to 2022 in Guangdong and Guangxi.We analyzed response rates based on target not detected(TND)status using intention-to-treat(ITT)analysis.Subgroups were formed based on baseline viral load(VL)(<100,000 vs.≥100,000 copies/mL)and CD4^(+)cell count(<200 vs.≥200 cell/µL).Median time to TND VL was assessed by Kaplan-Meier method.We also measured changes from baseline in CD4^(+)cell counts,CD4/CD8 ratio,lipid parameters,weight,creatinine(Cr),estimated glomerular filtration rate(eGFR),and drug-related adverse effects(DRAEs).Results:We enrolled 280 participants,including 137(48.9%)on DTG+3TC and 143(51.1%)on B/F/TAF.At week 48,96.4%(132/137)on DTG+3TC and 100%(143/143)on B/F/TAF achieved TND(P=0.064).At week 12,TND responses were higher with B/F/TAF(78.3%[112/143])than DTG+3TC(30.7%[42/137])(P<0.001).This trend held across subgroups.B/F/TAF achieved TND faster(12 weeks)than DTG+3TC(24 weeks)(P<0.001).No differences were seen in CD4^(+)cell count and CD4/CD8 ratio,except in the high-VL subgroup,where B/F/TAF showed better recovery.DRAEs were significantly lower with B/F/TAF(4.9%[7/143])than with DTG+3TC(13.1%[18/137])(P=0.016).Lipid parameters,body weight,and Cr increased in both groups over 48 weeks,with DTG+3TC showing a more favorable effect on triglycerides,high-density lipoprotein(HDL)cholesterol,and weight gain.Conclusions:In this real-life study,B/F/TAF led to a faster viral decline and fewer DRAEs compared to DTG+3TC.No significant difference was observed in the TND rate at week 48,regardless of baseline VL and CD4^(+)cell count.CD4^(+)recovery was superior for B/F/TAF in participants with high VL.The DTG+3TC regimen had less impact on metabolic changes than B/F/TAF.

文摘Achieving efficient hemostasis and wound management is vital to preserve life and restore health in case of extensive hemor-rhagic skin damage.Here,we develop a filter pump-like hierarchical porous-structure(HPS)dressing based on a non-woven substrate,konjac glucomannan(KGM)aerogel,and bi-functional microporous starch(BMS).The KGM aerogel intercalates into the non-woven network structure,forming a hydrophilic frame to stimulate the plasma permeation toward the interior in synergy with the hydrophilic pores of the BMS.The BMS surface forms a hydrophobic matrix that fills the spaces of the KGM hydrophilic frame,contributing to the isolation and aggregation of blood cells on the surface of the HPS dressing to establish rapid hemostasis.Animal model experiments suggest reliable HPS dressing hemostatic capacity,as it is able to stop ear artery and liver bleeding within 97.6±15.2 s and 67.8±5.4 s,respectively.Furthermore,the dressings exhibit antibacterial properties and enabled wound healing within 2 weeks.In vitro hemolysis and cytotoxicity tests also confirm the biocompatibility of HPS dressings.This novel“two-in-one”hemostatic dressing facilitates tissue repair of bleeding wounds over the entire recovery period,thereby providing a convenient strategy for wound management.
文摘Clinical imperatives for the reconstruction of jaw bone defects or resorbed alveolar ridge require new therapies or procedures instead of autologous/allogeneic bone grafts.Regenerative medicine,based on stem cell science and tissue engineering technology,is considered as an ideal alternative strategy for bone regeneration.In this paper,we review the current choices of cell source and strategies on directing the osteogenic differentiation of stem cells.The preclinical animal models for bone regeneration and the key translational points to clinical success in oral and maxillofacial region are also discussed.We propose comprehensive strategies based on stem cell and tissue engineering researches,allowing for clinical application in oral and maxillofacial region.
