This study aimed to identify the reasons for transferring athletes to local medical facilities during the Olympic and Paralympic Games.Data on 567 injuries and other illnesses of athletes treated at the on-site clinic...This study aimed to identify the reasons for transferring athletes to local medical facilities during the Olympic and Paralympic Games.Data on 567 injuries and other illnesses of athletes treated at the on-site clinics were collected from the Tokyo 2020 Organizing Committee.Of these,84 athletes who required outpatient care during the Games were registered for this survey.During the Olympic and Paralympic Games,66(8.3/1000)and 18(7.2/1000)athletes,respectively,consulted external medical facilities.In the Olympic Games,the reasons for these visits included 48 cases(72.7%)of injuries,13(19.7%)cases of illnesses,and 5(7.6%)cases of heat stroke illness(HSI).Of these patients,56(84.9%)were treated as outpatients and 10(15.1%)were hospitalized,while three of these patients required hospitalization for>7 days.On the other hand,in the Paralympics Games,there were 7(38.8%)cases of injuries,9(50.0%)other illnesses,1(5.6%)case of HSI,and 1(5.6%)other cases,of which 11(61.1%)were treated as outpatients and 7(38.9%)were hospitalized,but none was hospitalized for>7 days.Injuries accounted for 70%of the total cases at the 2021 Olympic Games,but only three(0.05%)were severe cases that required hospitalization for more than 1 week.In contrast,in the Paralympic Games,other illnesses accounted for approximately half of the total cases.This study provides details on the extent of injuries and other illnesses that were transferred to outside facilities,which has not been documented in previous games.展开更多
AIM:To assess the efficacy of endocytoscopic narrowband imaging(EC-NBI)for evaluating the severity of inflammation in ulcerative colitis(UC).METHODS:This retrospective study was conducted at a single tertiary care ref...AIM:To assess the efficacy of endocytoscopic narrowband imaging(EC-NBI)for evaluating the severity of inflammation in ulcerative colitis(UC).METHODS:This retrospective study was conducted at a single tertiary care referral center.We included UC patients who underwent colonoscopy with endocytoscopy from July 2010 to December 2013.ECNBI was performed,and the images were evaluated by assessing visibility,increased vascularization,and the increased calibers of capillaries and were classified as Obscure,Visible or Dilated.Obscure was indicative of inactive disease,while Visible and Dilated were indicative of acute inflammation.This study received Institutional Review Board approval.The primary outcome measures included the diagnostic ability of EC-NBI to distinguish between active and inactive UC on the basis of histological activity.The conventional endoscopic images were classified according to the Mayo endoscopic score.A score of 0 or 1 indicated inactive disease,whereas a score of 2 indicated active disease.RESULTS:Fifty-two patients were enrolled.There was a strong correlation between the EC-NBI findings and the histological assessment(r=0.871,P<0.01).The sensitivity,specificity,positive predictive value,negative predictive value,and accuracy of EC-NBI for diagnosing acute inflammation were 84.0%,100%,87.1%,100%,and 92.3%,respectively,while those for the Mayo endoscopic score were 100%,40.7%,100%,61.0%,and 69.2%,respectively.Compared with conventional endoscopy,EC-NBI was superior in diagnostic specificity,negative predictive value,and accuracy(P<0.001,P=0.001 and P=0.047,respectively).CONCLUSION:The EC-NBI finding of capillaries in the rectal mucosa was strongly correlated with histological inflammation and aided in the differential diagnosis between active and inactive UC.展开更多

Objectives: The present study aimed (1) to describe the social support needs of spouses of patients with cancer, and (2) to explore factors associated with social support needs of spouses of patients with cancer. Design: Spouses (n = 559) of patients with cancer were recruited by registered agents of an online survey company and completed a self-reporting, online questionnaire. Measurements: The questionnaires included demographic information and a tool to assess social support needs. Results: Factor analysis of social support needs of the spouses of patients with cancer indicated that (1) “social support needs regarding disease and treatment of patient” (54 items) comprised 3 factors (“medical condition and cure”, “daily life and social support”, “intimacy and employment”), and (2) “social support needs of spouse (19 items)” comprised 2 factors (“family psychological issues and social support” and “intimacy, employment and society”).
