The study aimed to investigate, in combination and individually, the association of 28 polymorphisms with the inflammatory responses of soccer athletes. The sample consisted of 47 male under-20 soccer athletes who belong to clubs in the first division of Brazilian soccer. Blood samples were collected at Pre, and 03, 24, and 48 hours after the training session to evaluate the inflammatory responses (hematological analyzes (hemogram), creatine phosphokinase (CK), high sensitivity quantitative C-reactive protein (CRP), tumor necrosis factor-alpha, (TNFα), interleukin 6 (IL-6) and insulin-like growth factor 1 (IGF-1)). DNA was obtained through scraping of buccal cells, where a sterile swab was rubbed on the inner side of the mouth of each participant 06 times. The database was built using the TruSeq DNA PCRFree kit (Illumina®) and the Covaris equipment for shearing genomic DNA (gDNA) by ultrasound. Of the analyzed SNPs, 09 (ACTN3 rs1815739, COL5A1 rs12722, COL5A1 rs3196378, HGF rs5745697, IGF1 rs35767, IL-6 rs1800795, MMP3 rs679620, SLC30A8 rs13266634, SOX15 rs4227) were individually associated with biomarkers and 07 SNPs, (COL5A1 rs12722, COL5A1 rs3196378, COL5A1 rs1800012, HGF rs5745697, IGF1 rs35767, IL-6 rs1800795 and MMP3 rs679620) analyzed in combination, explained 16% to 40% of the variation of inflammatory responses in soccer athletes. The results suggest that the genotypic profile can be taken into account for a more individualized distribution of the training load, along with the elaboration of recovery strategies for high-level athletes between training sessions and games of high physical and physiological demand.展开更多

However, although anemia and iron deficiency are significant global health problems, there are still disparities in the recognition and implementation of “Patient Blood Management” (PBM) as a comprehensive approach to mitigating the risks associated with these diseases. The purpose of the study is to review best practices for the treatment of anemia based on the Enhanced Recovery After Surgery (ERAS) protocol and PBM recommendations. Methods: This study reviewed the literature on preoperative iron deficiency anemia in patients undergoing gynecological surgery. We identified references through searches in PubMed using relevant search terms. Results: Among the various strategies used in PBM, perhaps the most important is the early detection and management of anemia. In gynecological surgery, there are several approaches to reducing perioperative blood loss, highlighting the use of gonadotropin-releasing hormone (GnRH) agonists (aGnRh) and antifibrinolytics. Oral and intravenous iron supplementation can be performed in addition to blood transfusion to treat anemia. Conclusion: Addressing preoperative and postoperative anemia through systematic correction, following the guidelines of the ERAS protocol and PBM guidelines, is essential to improving perioperative outcomes in women undergoing gynecological surgery.展开更多

This study aims to compare vesico-urethral anastomosis techniques (separate vs. continuous stitches) in laparotomy-based radical prostatectomies, evaluating peri and postoperative outcomes. Materials and Method: A retrospective analysis of 140 patients’ medical records yielded 86 eligible cases, divided into two groups based on anastomosis technique. Both groups underwent surgery at the same urology center between 2016 and 2019, with a follow-up period exceeding 12 months. Pre-operative characteristics were statistically similar between groups. Results: Perioperative complication rates did not significantly differ between the two techniques (p > 0.99). Background:Social distancing may affect athletes,training,causing negative effects on mental and physical health.Objective:This study therefore aimed to characterize the perception of Brazilian athletes about their ph...Background:Social distancing may affect athletes,training,causing negative effects on mental and physical health.Objective:This study therefore aimed to characterize the perception of Brazilian athletes about their physical and psychosocial aspects,sleep quality and coping strategies during the quarantine of the coronavirus disease 2019(COVID-19)pandemic.Methods:This was a cross-sectional study with online survey,performed with Brazilian athletes(amateur and professional)over 18 years.The main outcomes measures assessed were physical and psychosocial aspects,sleep quality and coping strategies.Results:A total of 214 athletes were included.The average weekly hours of training during the quarantine was 4.71±3.71 h,of which 64.5%athletes(138/214)were oriented by medical staff during training.For 52.8%(113/214)of athletes,training intensity during the quarantine was different/very different from the intensity before the quarantine.79.4%athletes(170/214)reported moderate to extreme difficulties in keeping the same level of training during the quarantine.77.1%athletes(165/214)had moderate to extreme anxiety and each of the athletes had concern about his or her athletic career future,including return to the sport.72.9%athletes(156/214)reported change in sleep schedule during the quarantine period.Conclusion:The quarantine period during COVID-19 pandemic negatively affected the athlete^perception about training routine,since athletes reported reduction in training hours and training intensity.Overall,the athletes reported that they were moderately to extremely anxious.They also had concerns about their career in the future,as well as concerns regarding return to sport.展开更多
In the last three decades,the understanding of the renin angiotensin system(RAS)has been changed by the discoveries of functional local systems,novel biologically active peptides,additional specific receptors,alternat...In the last three decades,the understanding of the renin angiotensin system(RAS)has been changed by the discoveries of functional local systems,novel biologically active peptides,additional specific receptors,alternative pathways of angiotensin(Ang)?Ⅱ?generation,and new roles for enzymes and precursor components other than those in Ang?Ⅱ?synthesis.In this regard,the discovery that Ang-(1-7)opposes the pressor,proliferative,pro-fibrotic,and pro-inflammatory effects mediated by Ang?Ⅱ?has contributed to the realization that the RAS is composed of two axes.The first axis consists of the angiotensin-converting enzyme(ACE),with Ang?Ⅱ?as the end product,and the angiotensin type 1(AT1)receptor as the main effector mediating the biological actions of Ang?Ⅱ.The second axis results from ACE2-mediated hydrolysis of Ang?Ⅱ,leading to the production of Ang-(1-7),with the Mas receptor as the main effector conveying the vasodilatory,antiproliferative,anti-fibrotic,and anti-inflammatory effects of Ang-(1-7).Experimental and clinical studies have shown that both axes of the RAS may take part in the pathogenesis of liver diseases.In this manuscript,we summarize the current evidence regarding the role of RAS in hepatic cirrhosis and its complications,including hemodynamic changes and hepatorenal syndrome.The therapeutic potential of the modulation of RAS molecules in liver diseases is also discussed.展开更多

METHODS: Male Wistar rats were submitted to sham-surgery or bile duct ligation...AIM: To evaluate in bile duct ligated rats whether there were progressive alterations of renal function without changes in histopathology. METHODS: Male Wistar rats were submitted to sham-surgery or bile duct ligation (BDL) and divided according to the post-procedure time (2, 4 and 6-wk). To determine renal function parameters, rats wereplaced in metabolic cages and, at the end of the experiment, blood and urine samples were obtained. Histology and hydroxyproline content were analyzed in liver and renal tissue. RESULTS: Rats with 2 wk of BDL increased free water clearance (P = 0.02), reduced urinary osmolality (P = 0.03) and serum creatinine (P = 0.01) in comparison to the sham group. In contrast, rats at 6 wk of BDL showed features of HRS, including signif icant increase in serum creatinine and reductions in creatinine clearance, water excretion and urinary sodium concentration. Rats with 4 wk of BDL exhibited an intermediate stage of renal dysfunction. Progressive hepatic f ibrosis according to post-procedure time was confirmed by histology. The increased levels of liver hydroxyproline contrasted with the absence of structural changes in the kidney, as assessed by histology and unchanged hydroxyproline content in renal tissue. CONCLUSION: Our data show that BDL produced progressive renal dysfunction without structural changes in the kidney, characterizing HRS. The present model will be useful to understand the pathophysiology of HRS.展开更多
AIM:To propose an alternative model of hepatic encephalopathy(HE) in mice,resembling the human features of the disease.METHODS:Mice received two consecutive intraperitoneal injections of thioacetamide(TAA) at low dosa...AIM:To propose an alternative model of hepatic encephalopathy(HE) in mice,resembling the human features of the disease.METHODS:Mice received two consecutive intraperitoneal injections of thioacetamide(TAA) at low dosage(300 mg/kg).Liver injury was assessed by serum transaminase levels(ALT) and liver histology(hematoxylin and eosin).Neutrophil infiltration was estimated by confocal liver intravital microscopy.Coagulopathy was evaluated using prolonged prothrombin and partial thromboplastin time.Hemodynamic parameters were measured through tail cuff.Ammonia levels were quantified in serum and brain samples.Electroencephalography(EEG) and psychomotor activity score were performed to show brain function.Brain edema was evaluated using magnetic resonance imaging.RESULTS:Mice submitted to the TAA regime developed massive liver injury,as shown by elevation of serum ALT levels and a high degree of liver necrosis.An intense hepatic neutrophil accumulation occurred in response to TAA-induced liver injury.This led to mice mortality and weight loss,which was associated with severe coagulopathy.Furthermore,TAA-treated mice presented with increased serum and cerebral levels of ammonia,in parallel with alterations in EEG spectrum and discrete brain edema,as shown by magnetic resonance imaging.In agreement with this,neuropsychomotor abnormalities ensued 36 h after TAA,fulfilling several HE features observed in humans.In this context of liver injury and neurological dysfunction,we observed lung inflammation and alterations in blood pressure and heart rate that were indicative of multiple organ dysfunction syndrome.CONCLUSION:In summary,we describe a new murine model of hepatic encephalopathy comprising multiple features of the disease in humans,which may provide new insights for treatment.展开更多
BACKGROUND Bipolar disorder(BD)is a severe psychiatric disorder characterized by mood swings.Psychosocial interventions,such as psychoeducation,play an essential role in promoting social rehabilitation and improving p...BACKGROUND Bipolar disorder(BD)is a severe psychiatric disorder characterized by mood swings.Psychosocial interventions,such as psychoeducation,play an essential role in promoting social rehabilitation and improving pharmacological treatment.AIM To investigate the role of psychoeducation in BD.METHODS A systematic review of original studies regarding psychoeducation interventions in patients with BD and their relatives was developed.A systematic literature search was performed using the Medline,Scopus,and Lilacs databases.No review articles or qualitative studies were included in the analysis.There were no date restriction criteria,and studies published up to April 2021 were included.RESULTS A total of forty-seven studies were selected for this review.Thirty-eight studies included patients,and nine included family members.Psychoeducation of patients and family members was associated with a lower number of new mood episodes and a reduction in number and length of stay of hospitalizations.Psychoeducational interventions with patients are associated with improved adherence to drug treatment.The strategies studied in patients and family members do not interfere with the severity of symptoms of mania or depression or with the patient's quality of life or functionality.Psychoeducational interventions with family members do not alter patients'adherence to pharmacotherapy.CONCLUSION Psychoeducation as an adjunct strategy to pharmacotherapy in the treatment of BD leads to a reduction in the frequency of new mood episodes,length of hospital stay and adherence to drug therapy.展开更多
The present work aims to develop a new vegetable insulating fluid for power transformers based on Jatropha curcas oil. Besides its technical benefits, Jatropha curcas oil has a socio-economic role by promoting income ...The present work aims to develop a new vegetable insulating fluid for power transformers based on Jatropha curcas oil. Besides its technical benefits, Jatropha curcas oil has a socio-economic role by promoting income to rural families, contributing to the countryside development and avoiding rural exodus. Thus, the entire transformer oil production (extraction, processing, characterization and accelerated aging) was covered and a new process was developed. For oil extraction, the most suitable process was the solvent extraction (5 mL of hexane per gram of crushed non-peeled seeds during 30 minutes) with an oil yield of 32%. In raw oil processing stage, the degumming, with 0.4 g of phosphoric acid per 100 g of oil, at 70°C, was used to remove phosphatides. Then, free fatty acids were 96% neutralized with a sodium hydroxide solution (0.5% w/w) at room temperature. For the oil clarification, the combination of 5% w/woil of activated carbon and 1% w/woil of MgO resulted in a bright, odorless and clear oil with an acid number of 0.04 mgKOH·g﹣1. The oil drying in a vacuum rotary evaporator, at 70°C, for 2 hours reduced the water content to 177 ppm. The processed oil was characterized following ASTM D6871 methods. This oil presented higher dielectric breakdown voltage (55 kV) than commercial transformer fluids (BIOTEMP?, EnvirotempFR3?, and Bivolt?), which increases transformer safety, capacity and lifetime. In addition, the processed oil has a lower viscosity than BIOTEMP? fluid, which can enhance the heat dissipation efficiency in the transformer. Moreover, the processed oil flash and fire points of 310°C and >340°C, respectively, confirm the great security of vegetable insulating fluids. The analyzed properties of the processed oil fulfill all the ASTM D6871, ABNT NBR 15422 and IEC 62770 specifications. Therefore, Jatropha curcas oil is a potential substitute formineral insulating fluids.展开更多

METHODS:Patients were allocated into two groups:outpatients with advanced liver disease(LD) and during liver transplantation(LT). Both groups were subdivided according to treatment with propranolol or not. Plasma was collected through peripheral venipuncture to determine plasma renin activity(PRA),Angiotensin(Ang) Ⅰ,Ang Ⅱ,and Ang-(1-7) levels by radioimmunoassay in LD group. During liver transplantation,hemodynamic parameters were determined and blood samples were obtained from the portal vein to measure renin angiotensin system(RAS) components.RESULTS:PRA,Ang Ⅰ,Ang Ⅱ and Ang-(1-7) were signifi cantly lower in the portal vein and periphery in all subgroups treated with propranolol as compared to non-treated. The relationships between Ang-(1-7) and Ang Ⅰ levels and between Ang Ⅱ and Ang Ⅰ were significantly increased in LD group receiving propranolol. The ratio between Ang-(1-7) and Ang Ⅱ remained unchanged in splanchnic and peripheral circulation in patients under β-blockade,whereas the relationship between Ang Ⅱ and Ang Ⅰ was significantly increased in splanchnic circulation of LT patients treated with propranolol. During liver transplantation,cardiac output and index as well systemic vascular resistance and index were reduced in propranolol-treated subgroup. CONCLUSION:In LD group,propranolol treatment reduced RAS mediators,but did not change the ratio between Ang-(1-7) and Ang Ⅱ in splanchnic and peripheral circulation. Furthermore,the modification of hemodynamic parameters in propranolol treated patients was not associated with changes in the angiotensin ratio.展开更多
Alternative strategies of management of sorghum diseases were implemented using field experiments conducted in Embrapa Maize and Sorghum Research Center,Brazil in 2001 until 2003 to evaluate the efficacy of lineage mi...Alternative strategies of management of sorghum diseases were implemented using field experiments conducted in Embrapa Maize and Sorghum Research Center,Brazil in 2001 until 2003 to evaluate the efficacy of lineage mixtures of Sorghum bicolor for the control of anthracnose(caused by Colletotrichum sublineolum).The calculated values for disease severity considered the area under the disease progress curve(AUDPC)observed for the nine susceptible and resistant elite strains of sorghum planted in 30 stands randomly.Each stand was drawn in mixtures of three genotypes each,in the same proportions per plant.This design was compared with the severity of the disease measured in AUDPC calculated for each of the same nine strains of sorghum in pure stands.In most of the cases,the disease severity on the susceptible cultivars was decreased;and in some cases,reached 85%of disease reduction compared with disease progress in pure stand of correspondent plant.The implication and effect of these sorghum mixtures on the populations of C.sublineolum on each treatment were evaluated and revealed that there was an increase of frequency of complex races in most of these mixtures and a highest phenotypic diversity for virulence to pathogen populations from pure stands,than the characterized on populations collected from the mixtures,despite of the effectiveness on decrease of the anthracnose severity.展开更多
BACKGROUND Patients undergoing solid organ transplantation,particularly those who live or have lived in tuberculosis(TB)endemic areas,are at a high risk of developing TB.The majority of post-transplantation TB cases a...BACKGROUND Patients undergoing solid organ transplantation,particularly those who live or have lived in tuberculosis(TB)endemic areas,are at a high risk of developing TB.The majority of post-transplantation TB cases are associated with reactivation of latent TB infection(LTBI).Brazil is in a single position with overlapping areas of high TB endemicity and high transplant activity.In liver transplant(LT),one should be aware of the potential hepatotoxicity associated with the treatment regimens for LTBI.AIM To evaluate the frequency of LTBI in LT patients and treatment-related issues.METHODS This was a retrospective analysis of a cohort of cirrhotic patients aged≥18 years,who underwent LT at a high-complexity teaching hospital from January 2005 to December 2012.RESULTS Overall,429 patients underwent LT during the study period.Of these,213(49.7%)underwent the tuberculin skin test(TST)during the pre-transplant period,and 35(16.4%)of them had a positive result.The treatment for LTBI was initiated after LT in 12(34.3%)of the TST-positive patients;in 3(25.0%),treatment was maintained for at least 6 mo.CONCLUSION The prevalence of LTBI was lower than expected.Initiation and completion of LTBI treatment was limited by difficulties in the management of these special patients.展开更多

In this regard, with the attempt of improving the efficiency of DNA vaccines, co-expression of stimulatory sequences and diverse vector delivery systems are being optimized. We evaluated the effects of different concentrations of macro and micronutrients(i.e., 100, 50 and 25% of the concentration of a referenced solution) combined with indole-3-butyric acid(IBA) at 0 and 2 g L-1 on the survival rate of mini-stumps and the survival and adventitious rooting of K. anthotheca mini-cuttings. The mini-stumps were grown from a source of seeds imported from Ghana, West Africa. The ministumps survived at a high rate(97%). Consistently high shoot yields were obtained from the mini-stumps(average of 1.8 per mini-stump). High survival percentage of minicuttings and adventitious rooting were observed when solutions with 100 and 50% of the nutrients concentration were combined with 2 g L-1 IBA. Histological sections analyzed through optical microscopy indicated the tissues from mini-cutting stems were juvenile, which might have stimulated adventitious rooting. These results have important implications for further work aimed at establishing propagation strategies for K. anthotheca, which are of prime importance for assisting breeding programs of this species.展开更多
Objective:To investigate the antinociceptive effect of tingenone on inflammatory pain,as well as and the involvement of the cannabinoid receptors type 2(CB2)and spinal microglia in this process.Methods:Male Swiss mice...Objective:To investigate the antinociceptive effect of tingenone on inflammatory pain,as well as and the involvement of the cannabinoid receptors type 2(CB2)and spinal microglia in this process.Methods:Male Swiss mice were subjected to inflammatory pain induced by intraplantar injection of carrageenan.The nociceptive threshold was measured by von Frey filaments test.Tingenone was administered orally 60 min before carrageenan injection.To evaluate the involvement of CB2 receptor,endocannabinoids,and microglia,AM630(a CB2 receptor antagonist),MAFP(an inhibitor of an enzyme that hydrolyses endocannabinoids),and minocycline(a microglial inhibitor)were given intrathecally 20 min before tingenone administration.In addition,an immunofluorescence assay was used to evaluate CB2 receptor and CD11 B(a microglial marker)expression in the spinal cord dorsal horn.Results:Tingenone significantly reduced carrageenan-induced hyperalgesia,which was reversed by pretreatment with AM630.MAFP and minocycline potentiated and prolonged the tingenoneinduced antinociception.CD11 B expression was increased in the spinal cord dorsal horn of mice with inflammatory pain pretreated with tingenone,which was reduced by AM630,MAFP,and minocycline.Conclusions:CB2 receptors and endocannabinoids participate in the tingenone-induced antinociception which may involve the inhibition of microglia at spinal level.展开更多
文摘The practice of soccer involves carrying-out actions of high intensity, which demand a great generation of eccentric strength, which in turn results in an increase in the inflammatory response after training practice and game matches. The study aimed to investigate, in combination and individually, the association of 28 polymorphisms with the inflammatory responses of soccer athletes. The sample consisted of 47 male under-20 soccer athletes who belong to clubs in the first division of Brazilian soccer. Blood samples were collected at Pre, and 03, 24, and 48 hours after the training session to evaluate the inflammatory responses (hematological analyzes (hemogram), creatine phosphokinase (CK), high sensitivity quantitative C-reactive protein (CRP), tumor necrosis factor-alpha, (TNFα), interleukin 6 (IL-6) and insulin-like growth factor 1 (IGF-1)). DNA was obtained through scraping of buccal cells, where a sterile swab was rubbed on the inner side of the mouth of each participant 06 times. The database was built using the TruSeq DNA PCRFree kit (Illumina®) and the Covaris equipment for shearing genomic DNA (gDNA) by ultrasound. Of the analyzed SNPs, 09 (ACTN3 rs1815739, COL5A1 rs12722, COL5A1 rs3196378, HGF rs5745697, IGF1 rs35767, IL-6 rs1800795, MMP3 rs679620, SLC30A8 rs13266634, SOX15 rs4227) were individually associated with biomarkers and 07 SNPs, (COL5A1 rs12722, COL5A1 rs3196378, COL5A1 rs1800012, HGF rs5745697, IGF1 rs35767, IL-6 rs1800795 and MMP3 rs679620) analyzed in combination, explained 16% to 40% of the variation of inflammatory responses in soccer athletes. The results suggest that the genotypic profile can be taken into account for a more individualized distribution of the training load, along with the elaboration of recovery strategies for high-level athletes between training sessions and games of high physical and physiological demand.
文摘Purpose: Due to the high prevalence of iron deficiency anemia in women undergoing gynecological surgeries and its association with worse postoperative results, it is necessary to identify and treat anemia preoperatively. However, although anemia and iron deficiency are significant global health problems, there are still disparities in the recognition and implementation of “Patient Blood Management” (PBM) as a comprehensive approach to mitigating the risks associated with these diseases. The purpose of the study is to review best practices for the treatment of anemia based on the Enhanced Recovery After Surgery (ERAS) protocol and PBM recommendations. Methods: This study reviewed the literature on preoperative iron deficiency anemia in patients undergoing gynecological surgery. We identified references through searches in PubMed using relevant search terms. Results: Among the various strategies used in PBM, perhaps the most important is the early detection and management of anemia. In gynecological surgery, there are several approaches to reducing perioperative blood loss, highlighting the use of gonadotropin-releasing hormone (GnRH) agonists (aGnRh) and antifibrinolytics. Oral and intravenous iron supplementation can be performed in addition to blood transfusion to treat anemia. Conclusion: Addressing preoperative and postoperative anemia through systematic correction, following the guidelines of the ERAS protocol and PBM guidelines, is essential to improving perioperative outcomes in women undergoing gynecological surgery.

文摘BACKGROUND Helicobacter pylori(H.pylori)infection has been well-established as a significant risk factor for several gastrointestinal disorders.The urea breath test(UBT)has emerged as a leading non-invasive method for detecting H.pylori.Despite numerous studies confirming its substantial accuracy,the reliability of UBT results is often compromised by inherent limitations.These findings underscore the need for a rigorous statistical synthesis to clarify and reconcile the diagnostic accuracy of the UBT for the diagnosis of H.pylori infection.AIM To determine and compare the diagnostic accuracy of 13C-UBT and 14C-UBT for H.pylori infection in adult patients with dyspepsia.METHODS We conducted an independent search of the PubMed/MEDLINE,EMBASE,and Cochrane Central databases until April 2022.Our search included diagnostic accuracy studies that evaluated at least one of the index tests(^(13)C-UBT or ^(14)C-UBT)against a reference standard.We used the QUADAS-2 tool to assess the methodo-logical quality of the studies.We utilized the bivariate random-effects model to calculate sensitivity,specificity,positive and negative test likelihood ratios(LR+and LR-),as well as the diagnostic odds ratio(DOR),and their 95%confidence intervals.We conducted subgroup analyses based on urea dosing,time after urea administration,and assessment technique.To investigate a possible threshold effect,we conducted Spearman correlation analysis,and we generated summary receiver operating characteristic(SROC)curves to assess heterogeneity.Finally,we visually inspected a funnel plot and used Egger’s test to evaluate publication bias.endorsing both as reliable diagnostic tools in clinical practice.CONCLUSION In summary,our study has demonstrated that ^(13)C-UBT has been found to outperform the ^(14)C-UBT,making it the preferred diagnostic approach.Additionally,our results emphasize the significance of carefully considering urea dosage,assessment timing,and measurement techniques for both tests to enhance diagnostic precision.Nevertheless,it is crucial for researchers and clinicians to evaluate the strengths and limitations of our findings before implementing them in practice.
文摘Background: Prostate cancer ranks as the second most prevalent malignancy in men, with treatment strategies ranging from observation to various interventions. Radical prostatectomy is a common approach, aiming for a cure, and the technique for vesico-urethral anastomosis varies, employing separate or continuous stitches. However, a consensus on the optimal technique is lacking. This study aims to compare vesico-urethral anastomosis techniques (separate vs. continuous stitches) in laparotomy-based radical prostatectomies, evaluating peri and postoperative outcomes. Materials and Method: A retrospective analysis of 140 patients’ medical records yielded 86 eligible cases, divided into two groups based on anastomosis technique. Both groups underwent surgery at the same urology center between 2016 and 2019, with a follow-up period exceeding 12 months. Pre-operative characteristics were statistically similar between groups. Results: Perioperative complication rates did not significantly differ between the two techniques (p > 0.99). However, the continuous stitch group experienced a 20-minute longer procedure time (p 0.05), patients with continuous stitches had 2.5 times higher diaper usage at twelve months (p Conclusion: This research suggests that vesico-urethral anastomosis with separate stitches yields superior results in laparotomy-based radical prostatectomies compared to continuous stitches, demonstrating benefits in surgical time, hospital stay, sentinel drain duration, stenosis rates, and long-term urinary incontinence outcomes.

文摘The high costs incurred due to attacks and the increasing number of different devices in the Internet of Things(IoT)highlight the necessity of the early detection of botnets(i.e.,a network of infected devices)to gain an advantage against attacks.However,early botnet detection is challenging because of continuous malware mutations,the adoption of sophisticated obfuscation techniques,and the massive volume of data.The literature addresses botnet detection by modeling the behavior of malware spread,the classification of malicious traffic,and the analysis of traffic anomalies.This article details ANTE,a system for ANTicipating botnEt signals based on machine learning algorithms.The system adapts itself to different scenarios and detects different types of botnets.It autonomously selects the most appropriate Machine Learning(ML)pipeline for each botnet and improves the classification before an attack effectively begins.The system evaluation follows trace-driven experiments and compares ANTE results to other relevant results from the literature over four representative datasets:ISOT HTTP Botnet,CTU-13,CICDDoS2019,and BoT-IoT.Results show an average detection accuracy of 99.06%and an average bot detection precision of 100%.

文摘Background:Social distancing may affect athletes,training,causing negative effects on mental and physical health.Objective:This study therefore aimed to characterize the perception of Brazilian athletes about their physical and psychosocial aspects,sleep quality and coping strategies during the quarantine of the coronavirus disease 2019(COVID-19)pandemic.Methods:This was a cross-sectional study with online survey,performed with Brazilian athletes(amateur and professional)over 18 years.The main outcomes measures assessed were physical and psychosocial aspects,sleep quality and coping strategies.Results:A total of 214 athletes were included.The average weekly hours of training during the quarantine was 4.71±3.71 h,of which 64.5%athletes(138/214)were oriented by medical staff during training.For 52.8%(113/214)of athletes,training intensity during the quarantine was different/very different from the intensity before the quarantine.79.4%athletes(170/214)reported moderate to extreme difficulties in keeping the same level of training during the quarantine.77.1%athletes(165/214)had moderate to extreme anxiety and each of the athletes had concern about his or her athletic career future,including return to the sport.72.9%athletes(156/214)reported change in sleep schedule during the quarantine period.Conclusion:The quarantine period during COVID-19 pandemic negatively affected the athlete^perception about training routine,since athletes reported reduction in training hours and training intensity.Overall,the athletes reported that they were moderately to extremely anxious.They also had concerns about their career in the future,as well as concerns regarding return to sport.

文摘In the last three decades,the understanding of the renin angiotensin system(RAS)has been changed by the discoveries of functional local systems,novel biologically active peptides,additional specific receptors,alternative pathways of angiotensin(Ang)?Ⅱ?generation,and new roles for enzymes and precursor components other than those in Ang?Ⅱ?synthesis.In this regard,the discovery that Ang-(1-7)opposes the pressor,proliferative,pro-fibrotic,and pro-inflammatory effects mediated by Ang?Ⅱ?has contributed to the realization that the RAS is composed of two axes.The first axis consists of the angiotensin-converting enzyme(ACE),with Ang?Ⅱ?as the end product,and the angiotensin type 1(AT1)receptor as the main effector mediating the biological actions of Ang?Ⅱ.The second axis results from ACE2-mediated hydrolysis of Ang?Ⅱ,leading to the production of Ang-(1-7),with the Mas receptor as the main effector conveying the vasodilatory,antiproliferative,anti-fibrotic,and anti-inflammatory effects of Ang-(1-7).Experimental and clinical studies have shown that both axes of the RAS may take part in the pathogenesis of liver diseases.In this manuscript,we summarize the current evidence regarding the role of RAS in hepatic cirrhosis and its complications,including hemodynamic changes and hepatorenal syndrome.The therapeutic potential of the modulation of RAS molecules in liver diseases is also discussed.
文摘Hepatic encephalopathy(HE) is a major complication that is closely related to the progression of end-stage liver disease.Metabolic changes in advanced liver failure can promote cognition impairment,attention deficits and motor dysfunction that may result in coma and death.HE can be subdivided according to the type of hepatic injury,namely,type A,which results from acute liver failure,type B,which is associated with a portosystemic shunting without intrinsic liver disease,and type C,which is due to chronic liver disease.Several studies have investigated the pathogenesis of the disease,and most of the mechanisms have been explored using animal models.This article aimed to review the use of preclinical models to investigate HE.The most used animal species are rats and mice.Experimental models of type A HE include surgical procedures and the administration of hepatotoxic medications,whereas models of types B and C HE are generally surgically induced lesions in liver tissue,which evolve to hepatic cirrhosis.Preclinical models have allowed the comprehension of the pathways related to HE.

文摘AIM: To evaluate in bile duct ligated rats whether there were progressive alterations of renal function without changes in histopathology. METHODS: Male Wistar rats were submitted to sham-surgery or bile duct ligation (BDL) and divided according to the post-procedure time (2, 4 and 6-wk). To determine renal function parameters, rats wereplaced in metabolic cages and, at the end of the experiment, blood and urine samples were obtained. Histology and hydroxyproline content were analyzed in liver and renal tissue. RESULTS: Rats with 2 wk of BDL increased free water clearance (P = 0.02), reduced urinary osmolality (P = 0.03) and serum creatinine (P = 0.01) in comparison to the sham group. In contrast, rats at 6 wk of BDL showed features of HRS, including signif icant increase in serum creatinine and reductions in creatinine clearance, water excretion and urinary sodium concentration. Rats with 4 wk of BDL exhibited an intermediate stage of renal dysfunction. Progressive hepatic f ibrosis according to post-procedure time was confirmed by histology. The increased levels of liver hydroxyproline contrasted with the absence of structural changes in the kidney, as assessed by histology and unchanged hydroxyproline content in renal tissue. CONCLUSION: Our data show that BDL produced progressive renal dysfunction without structural changes in the kidney, characterizing HRS. The present model will be useful to understand the pathophysiology of HRS.

文摘AIM:To propose an alternative model of hepatic encephalopathy(HE) in mice,resembling the human features of the disease.METHODS:Mice received two consecutive intraperitoneal injections of thioacetamide(TAA) at low dosage(300 mg/kg).Liver injury was assessed by serum transaminase levels(ALT) and liver histology(hematoxylin and eosin).Neutrophil infiltration was estimated by confocal liver intravital microscopy.Coagulopathy was evaluated using prolonged prothrombin and partial thromboplastin time.Hemodynamic parameters were measured through tail cuff.Ammonia levels were quantified in serum and brain samples.Electroencephalography(EEG) and psychomotor activity score were performed to show brain function.Brain edema was evaluated using magnetic resonance imaging.RESULTS:Mice submitted to the TAA regime developed massive liver injury,as shown by elevation of serum ALT levels and a high degree of liver necrosis.An intense hepatic neutrophil accumulation occurred in response to TAA-induced liver injury.This led to mice mortality and weight loss,which was associated with severe coagulopathy.Furthermore,TAA-treated mice presented with increased serum and cerebral levels of ammonia,in parallel with alterations in EEG spectrum and discrete brain edema,as shown by magnetic resonance imaging.In agreement with this,neuropsychomotor abnormalities ensued 36 h after TAA,fulfilling several HE features observed in humans.In this context of liver injury and neurological dysfunction,we observed lung inflammation and alterations in blood pressure and heart rate that were indicative of multiple organ dysfunction syndrome.CONCLUSION:In summary,we describe a new murine model of hepatic encephalopathy comprising multiple features of the disease in humans,which may provide new insights for treatment.
文摘BACKGROUND Bipolar disorder(BD)is a severe psychiatric disorder characterized by mood swings.Psychosocial interventions,such as psychoeducation,play an essential role in promoting social rehabilitation and improving pharmacological treatment.AIM To investigate the role of psychoeducation in BD.METHODS A systematic review of original studies regarding psychoeducation interventions in patients with BD and their relatives was developed.A systematic literature search was performed using the Medline,Scopus,and Lilacs databases.No review articles or qualitative studies were included in the analysis.There were no date restriction criteria,and studies published up to April 2021 were included.RESULTS A total of forty-seven studies were selected for this review.Thirty-eight studies included patients,and nine included family members.Psychoeducation of patients and family members was associated with a lower number of new mood episodes and a reduction in number and length of stay of hospitalizations.Psychoeducational interventions with patients are associated with improved adherence to drug treatment.The strategies studied in patients and family members do not interfere with the severity of symptoms of mania or depression or with the patient's quality of life or functionality.Psychoeducational interventions with family members do not alter patients'adherence to pharmacotherapy.CONCLUSION Psychoeducation as an adjunct strategy to pharmacotherapy in the treatment of BD leads to a reduction in the frequency of new mood episodes,length of hospital stay and adherence to drug therapy.
文摘The present work aims to develop a new vegetable insulating fluid for power transformers based on Jatropha curcas oil. Besides its technical benefits, Jatropha curcas oil has a socio-economic role by promoting income to rural families, contributing to the countryside development and avoiding rural exodus. Thus, the entire transformer oil production (extraction, processing, characterization and accelerated aging) was covered and a new process was developed. For oil extraction, the most suitable process was the solvent extraction (5 mL of hexane per gram of crushed non-peeled seeds during 30 minutes) with an oil yield of 32%. In raw oil processing stage, the degumming, with 0.4 g of phosphoric acid per 100 g of oil, at 70°C, was used to remove phosphatides. Then, free fatty acids were 96% neutralized with a sodium hydroxide solution (0.5% w/w) at room temperature. For the oil clarification, the combination of 5% w/woil of activated carbon and 1% w/woil of MgO resulted in a bright, odorless and clear oil with an acid number of 0.04 mgKOH·g﹣1. The oil drying in a vacuum rotary evaporator, at 70°C, for 2 hours reduced the water content to 177 ppm. The processed oil was characterized following ASTM D6871 methods. This oil presented higher dielectric breakdown voltage (55 kV) than commercial transformer fluids (BIOTEMP?, EnvirotempFR3?, and Bivolt?), which increases transformer safety, capacity and lifetime. In addition, the processed oil has a lower viscosity than BIOTEMP? fluid, which can enhance the heat dissipation efficiency in the transformer. Moreover, the processed oil flash and fire points of 310°C and >340°C, respectively, confirm the great security of vegetable insulating fluids. The analyzed properties of the processed oil fulfill all the ASTM D6871, ABNT NBR 15422 and IEC 62770 specifications. Therefore, Jatropha curcas oil is a potential substitute formineral insulating fluids.

文摘AIM:To evaluate the effect of β-blockade on angiotensins in the splanchnic and peripheral circulation of cirrhotic patients and also to compare hemodynamic parameters during liver transplantation according to propranolol pre-treatment or not. METHODS:Patients were allocated into two groups:outpatients with advanced liver disease(LD) and during liver transplantation(LT). Both groups were subdivided according to treatment with propranolol or not. Plasma was collected through peripheral venipuncture to determine plasma renin activity(PRA),Angiotensin(Ang) Ⅰ,Ang Ⅱ,and Ang-(1-7) levels by radioimmunoassay in LD group. During liver transplantation,hemodynamic parameters were determined and blood samples were obtained from the portal vein to measure renin angiotensin system(RAS) components.RESULTS:PRA,Ang Ⅰ,Ang Ⅱ and Ang-(1-7) were signifi cantly lower in the portal vein and periphery in all subgroups treated with propranolol as compared to non-treated. The relationships between Ang-(1-7) and Ang Ⅰ levels and between Ang Ⅱ and Ang Ⅰ were significantly increased in LD group receiving propranolol. The ratio between Ang-(1-7) and Ang Ⅱ remained unchanged in splanchnic and peripheral circulation in patients under β-blockade,whereas the relationship between Ang Ⅱ and Ang Ⅰ was significantly increased in splanchnic circulation of LT patients treated with propranolol. During liver transplantation,cardiac output and index as well systemic vascular resistance and index were reduced in propranolol-treated subgroup. CONCLUSION:In LD group,propranolol treatment reduced RAS mediators,but did not change the ratio between Ang-(1-7) and Ang Ⅱ in splanchnic and peripheral circulation. Furthermore,the modification of hemodynamic parameters in propranolol treated patients was not associated with changes in the angiotensin ratio.
文摘Alternative strategies of management of sorghum diseases were implemented using field experiments conducted in Embrapa Maize and Sorghum Research Center,Brazil in 2001 until 2003 to evaluate the efficacy of lineage mixtures of Sorghum bicolor for the control of anthracnose(caused by Colletotrichum sublineolum).The calculated values for disease severity considered the area under the disease progress curve(AUDPC)observed for the nine susceptible and resistant elite strains of sorghum planted in 30 stands randomly.Each stand was drawn in mixtures of three genotypes each,in the same proportions per plant.This design was compared with the severity of the disease measured in AUDPC calculated for each of the same nine strains of sorghum in pure stands.In most of the cases,the disease severity on the susceptible cultivars was decreased;and in some cases,reached 85%of disease reduction compared with disease progress in pure stand of correspondent plant.The implication and effect of these sorghum mixtures on the populations of C.sublineolum on each treatment were evaluated and revealed that there was an increase of frequency of complex races in most of these mixtures and a highest phenotypic diversity for virulence to pathogen populations from pure stands,than the characterized on populations collected from the mixtures,despite of the effectiveness on decrease of the anthracnose severity.

文摘BACKGROUND Patients undergoing solid organ transplantation,particularly those who live or have lived in tuberculosis(TB)endemic areas,are at a high risk of developing TB.The majority of post-transplantation TB cases are associated with reactivation of latent TB infection(LTBI).Brazil is in a single position with overlapping areas of high TB endemicity and high transplant activity.In liver transplant(LT),one should be aware of the potential hepatotoxicity associated with the treatment regimens for LTBI.AIM To evaluate the frequency of LTBI in LT patients and treatment-related issues.METHODS This was a retrospective analysis of a cohort of cirrhotic patients aged≥18 years,who underwent LT at a high-complexity teaching hospital from January 2005 to December 2012.RESULTS Overall,429 patients underwent LT during the study period.Of these,213(49.7%)underwent the tuberculin skin test(TST)during the pre-transplant period,and 35(16.4%)of them had a positive result.The treatment for LTBI was initiated after LT in 12(34.3%)of the TST-positive patients;in 3(25.0%),treatment was maintained for at least 6 mo.CONCLUSION The prevalence of LTBI was lower than expected.Initiation and completion of LTBI treatment was limited by difficulties in the management of these special patients.
文摘DNA vaccines are the third generation vaccines based on purified plasmid preparations containing transgenes that encode antigenic/therapeutic proteins or peptides capable of triggering an immune response against a wide range of diseases. This vaccine platform presents several attributes that confer distinct advantages over other vaccine technologies in terms of safety, ease of fabrication and stability. Many aspects, such as antigen expression and especially vector design, are under study because of their great influence on immunogenicity and efficacy of DNA vaccines. In this regard, with the attempt of improving the efficiency of DNA vaccines, co-expression of stimulatory sequences and diverse vector delivery systems are being optimized. With this in mind, this review aims to giving a conceptual approach of DNA vaccines, explaining their mechanisms of action and listing the already licensed veterinary DNA vaccines presented in the market.

文摘Khaya anthotheca is a hardwood species from Africa and recently introduced to Brazil. This species yields high-quality wood for diverse applications, but little has been done toward an effective propagation method for large-scale production in nurseries. We evaluated the effects of different concentrations of macro and micronutrients(i.e., 100, 50 and 25% of the concentration of a referenced solution) combined with indole-3-butyric acid(IBA) at 0 and 2 g L-1 on the survival rate of mini-stumps and the survival and adventitious rooting of K. anthotheca mini-cuttings. The mini-stumps were grown from a source of seeds imported from Ghana, West Africa. The ministumps survived at a high rate(97%). Consistently high shoot yields were obtained from the mini-stumps(average of 1.8 per mini-stump). High survival percentage of minicuttings and adventitious rooting were observed when solutions with 100 and 50% of the nutrients concentration were combined with 2 g L-1 IBA. Histological sections analyzed through optical microscopy indicated the tissues from mini-cutting stems were juvenile, which might have stimulated adventitious rooting. These results have important implications for further work aimed at establishing propagation strategies for K. anthotheca, which are of prime importance for assisting breeding programs of this species.

文摘Objective:To investigate the antinociceptive effect of tingenone on inflammatory pain,as well as and the involvement of the cannabinoid receptors type 2(CB2)and spinal microglia in this process.Methods:Male Swiss mice were subjected to inflammatory pain induced by intraplantar injection of carrageenan.The nociceptive threshold was measured by von Frey filaments test.Tingenone was administered orally 60 min before carrageenan injection.To evaluate the involvement of CB2 receptor,endocannabinoids,and microglia,AM630(a CB2 receptor antagonist),MAFP(an inhibitor of an enzyme that hydrolyses endocannabinoids),and minocycline(a microglial inhibitor)were given intrathecally 20 min before tingenone administration.In addition,an immunofluorescence assay was used to evaluate CB2 receptor and CD11 B(a microglial marker)expression in the spinal cord dorsal horn.Results:Tingenone significantly reduced carrageenan-induced hyperalgesia,which was reversed by pretreatment with AM630.MAFP and minocycline potentiated and prolonged the tingenoneinduced antinociception.CD11 B expression was increased in the spinal cord dorsal horn of mice with inflammatory pain pretreated with tingenone,which was reduced by AM630,MAFP,and minocycline.Conclusions:CB2 receptors and endocannabinoids participate in the tingenone-induced antinociception which may involve the inhibition of microglia at spinal level.
