Short-term and middle-term evaluation of laparoscopic hepatectomies compared with open hepatectomies: A propensity score matching analysis 被引量:2

AIM To compare short-term results between laparoscopic hepatectomy and open hepatectomy using a propensity score matching. METHODS A patient in the laparoscopic liver resection(LLR) groupwas randomly matched with another patient in the open liver resection(OLR) group using a 1:1 allocated ratio with the nearest estimated propensity score. Patients of the LLR group without matches were excluded. Matching criteria included age, gender, body mass index, American Society of Anesthesiologists score, potential co-morbidities, hepatopathies, size and number of nodules, preoperative chemotherapy, minor or major liver re-sections. Intraoperative and postoperative data were compared in both groups.RESULTS From January 2012 to January 2015, a total of 241 hepa-tectomies were consecutively performed, of which 169 in the OLR group(70.1%) and 72 in the LLR group(29.9%). The conversion rate was 9.7%(n = 7). The mortality rate was 4.2% in the OLR group and 0% in the LLR group. Prior to and after propensity score matching, there was a statistically significant difference favorable to the LLR group regarding shorter operative times(185 min vs 247.5 min; P = 0.002), less blood loss(100 m L vs 300 m L; P = 0.002), a shorter hospital stay(7 d vs 9 d; P = 0.004), and a significantly lower rate of medical complications(4.3% vs 26.4%; P < 0.001). CONCLUSION Laparoscopic liver resections seem to yield better short-term and mid-term results as compared to open hepatectomies and could well be considered a privileged approach and become the gold standard in carefully selected patients. 展开更多

A novel chiral Nickel (II) complex of N-(2-pyridylmethyl)-L-alanine (Hpyala) <b>1</b> has been prepared and structurally characterized by elemental analysis, FT-IR, UV-visible, TGA and single crystal X-ray diffraction techniques. Complex <b>1</b> crystallizes in an orthorhombic P2<sub>1</sub>2<sub>1</sub>2<sub>1</sub> space group. The nickel (II) centre in the complex adopts a distorted octahedral geometry. This compound has been seen to exhibit structural diversity resulting from the number of lattice water molecules. The photoluminescent properties of this compound which have also been investigated, indicates the potential application in luminescence. 展开更多

基于对最终产品形状的认识,提出一种新的快速的针对多工步轴对称冷锻建模方法—"伪逆法(PIA)"。为了考虑变形路径,引入了一些中间构型并且采用自由表面对这些中间构形进行修正而不采用接触算法。利用等效应力的概念和拉伸曲线... 基于对最终产品形状的认识,提出一种新的快速的针对多工步轴对称冷锻建模方法—"伪逆法(PIA)"。为了考虑变形路径,引入了一些中间构型并且采用自由表面对这些中间构形进行修正而不采用接触算法。利用等效应力的概念和拉伸曲线,提出了一种新的快速而健壮的塑性积分直接算法。数值试验表明:与增量法相比,伪逆法速度非常快。PIA被用于多工步冷锻工艺预制体模具优化设计过程中,该优化过程以最大限度减少在锻造过程中产生的等效塑性应变和需要的冲压力为目标,预制体模具的形状采用B样条曲线描述,以B样条曲线控制点的纵坐标为设计变量。目标函数的演变表明了模具形状优化对锻造件的质量和节能的重要性。通过与传统增量法的结果相比较,体现出了PIA的高效性和准确性。 展开更多

A Displacement Based Analytical Model to Determine Residual Stress Components in a Finite Elastic Thin Plate with Hole-Drilling Method

In order to measure the residual stress components in an elastic thin plate, the hole-drilling strain-gage method has been used. This method enables to determine the relation between the magnitudes and directions of the principal stresses and the strain relaxation about the hole. In the existing analytical models based on stress field, the formulations associated with the hole-drilling method are based on the assumption of an infinite plate, this may cause some errors for a finite plate and it’s difficult to validate these solutions by FE methods. Furthermore, in the composite, the displacement field is continuous but the stress field is not necessarily continuous, the displacement field based method has to be used. In the present paper an analyt-ical model based on a displacement field described by a function with coefficients to determine for a finite round thin plate is presented. The coefficients used in the displacement field are independent on the three residual stress components, and they are determined by minimization of the internal strain energy during the hole-drilling process. Once the coefficients in the dis-placement field are determined, three strains measured in three radial directions are utilized to determine the three residual stress components. The proposed analytical model can be also adapted to infinite plate by assuming that the diameter of the round plate tends to infinite. 展开更多

Despite of many traditional uses of this plant, scientific research on the content of its chemical compounds is still limited. This study aims to isolate the chemical compounds contained in the n-butanol fraction of Lannea kerstingii leaves. The chemical investigation of the leaves of Lannea kerstingii led to isolation of three undescribed C-13 norisoprenoids, lankerstinol A-C (1-3), together with six (4-9) known flavonoid glycosides. The structures of these compounds were established by spectroscopic analyses. 展开更多

The title compound, [N-(2-pyridylmethyl)-(L)-alanine]Co(III) thiocyanate (1) was obtained from the reaction of Co(OOCH3)2·H2O with the tridentate reduced Schiff base ligand, N-(2-pyridylmethyl)-(L)-alanine (L) and NH4SCN and characterized by elemental analysis, IR, UV-visible, TGA and single- crystal X-ray diffraction. Structural and spectroscopic analyses reveal [Co(L)2)]SCN to be monomeric with Cobalt(III) adopting a pseudo-octahedral geometry, coordinating to two reduce Schiff base ligands. In the crystal lattice, the thiocyanate anion forms an intermolecular SCN···HNamine hydrogen bond, while adjacent monomers are linked by intermolecular Ocarboxyl···HNamine···H-bonds to form a supramolecular network. This work is therefore undertaken in an attempt to construct coordination framework structures of varying properties using the mixed-ligand strategy involving reduced Schiff bases and the thiocyanate ion. 展开更多

A new flavonoid, acacetin-7-O-β-D-glucopyranosyl-(1 → 2)-β-D-glucuronopyranoside (1), together with four known flavonoids, apigenin-7-O-β-D-glucopyranosyl-(1 → 2)-β-D-glucuronopyranoside (2), acacetin (3), acacetin-7-O-β-D-glucuronide (4) and genkwanin-5-O-primveroside (5) were isolated from the leaves of Glyphaea brevis. Their structures were elucidated by spectroscopic techniques. The isolated compounds (1 - 5) were tested for their antioxidant activity using the 1,1-diphenyl-2-picrylhydrazyl (DPPH) radical scavenging assay. The tested compounds showed slight negative antioxidant activities against DPPH radicals. 展开更多
