Diverticular abscesses may be initially treated with antibiotics and/or percutaneous drainage and/or surgery. Endoscopic ultrasound(EUS)-guided drainage techniques are increasingly used as a minimally invasive alternative to percutaneous or surgical approaches, as they are associated with better treatment outcomes, shorter recovery time and duration of hospitalization.CASE SUMMARY A 57-year-old female presented to the emergency department on account of abdominal pain and fever. Clinical examination revealed tenderness in the left lower abdominal quadrant, with elevated inflammatory markers in laboratory tests. Abdominal computed tomography(CT) revealed an 8 cm × 8 cm × 5 cm well-encapsulated abscess of the sigmoid colon, surrounded by numerous diverticula. A diagnosis of Hinchey Ⅱ diverticular abscess was made, and the patient was admitted and commenced on appropriate antibiotic treatment. A transrectal EUS showed a fluid collection in direct contact with the sigmoid colon. Transluminal drainage was performed, and a lumen-apposing metal stent was inserted into the abscess collection. A follow-up CT scan showed a regression of the collection. The patient’s general condition improved, and the stent was removed during a follow-up transrectal EUS that revealed no visible collection.CONCLUSION We report the first successful management of a pelvic abscess in patient with Hinchey Ⅱ acute diverticulitis using EUS-guided transluminal drainage in Slovenia. The technique appears effective for well-encapsulated intra-abdominal abscesses larger than 4 cm in direct contact with the intestinal wall of left colon.展开更多

The novelty of the article is that it is possible to explain the nature of supersymmetry in terms of the geometric representation of the wave function and to propose a mechanism of spontaneous symmetry breaking of the excitation of the universe with different degrees of freedom. It is under such conditions that the well-known spontaneous symmetry breaking occurs and individual excitation acquires mass. At the same time, a phase transition of the first kind occurs with the formation of a new phase.展开更多

Development of direct reprogramming technology offers an alternative approach for generation of pluripotent stem cells, applicable also in farm animals. Induced pluripotent stem cells(i PSCs) represent practically limitless, ethically acceptable, individuum-specific source of pluripotent cells that can be generated from different types of somatic cells. i PSCs can differentiate to all cell types of an organism's body and have a tremendous potential for numerous applications in medicine, agriculture, and biotechnology. However, molecular mechanisms behind the reprogramming process remain largely unknown and hamper generation of bona fide i PSCs and their use in human clinical practice. Large animal models are essential to expand the knowledge obtained on rodents and facilitate development and validation of transplantation therapies in preclinical studies. Additionally, transgenic animals with special traits could be generated from genetically modified pluripotent cells, using advanced reproduction techniques. Despite their applicative potential, it seems that i PSCs in farm animals haven't received the deserved attention. The aim of this review was to provide a systematic overview on i PSC generation in the most important mammalian farm animal species(cattle, pig, horse, sheep, goat, and rabbit), compare protein sequence similarity of pluripotency-related transcription factors in different species, and discuss potential uses of farm animal i PSCs. Literature mining revealed 32 studies, describing i PSC generation in pig(13 studies), cattle(5),horse(5), sheep(4), goat(3), and rabbit(2) that are summarized in a concise, tabular format.展开更多

Highthroughput methods are rapidly changing our view and understanding of the molecular basis of gastric carcinogenesis. Today, it is widely accepted that the molecular complexity and heterogeneity, both interand intra-tumour, of gastric adenocarcinomas presentsignificant obstacles in elucidating specific biomarkers for early detection of the disease. Although genomewide sequencing and gene expression studies have revealed the intricate nature of the molecular changes that occur in tumour landscapes, the collected data and results are complex and sometimes contradictory.Several aberrant molecules have already been tested in clinical trials, although their diagnostic and prognostic utilities have not been confirmed thus far. The gold standard for the detection of sporadic gastric cancer is still the gastric endoscopy, which is considered invasive. In addition, genome-wide association studies have confirmed that genetic variations are important contributors to increased cancer risk and could participate in the initiation of malignant transformation. This hypothesis could in part explain the late onset of sporadic gastric cancers. The elaborate interplay of polymorphic low penetrance genes and lifestyle and environmental risk factors requires additional research to decipher their relative impacts on tumorigenesis. The purpose of this article is to present details of the molecular heterogeneity of sporadic gastric cancers at the DNA, RNA, and proteome levels and to discuss issues relevant to the translation of basic research data to clinically valuable tools. The focus of this work is the identification of relevant molecular changes that could be detected non-invasively.展开更多

Methods: Seven propolis samples were collected from different locations in Morocco, which are characterized by different plant predominant vegetations. The resin, wax and balsam of hydroalcoholic extract of propolis content were identified, and the antioxidant content was analyzed with the use of HPLC and colorimetric methods. The antioxidant activity was assessed by DPPH, ABTS.^+ and ferric reducing power assays. The antimicrobial activity was assessed against bacterial species, including methicillin resistant Staphylococcus aureus and Candida albicans, and expressed as the minimal inhibitory concentration. Results: The propolis samples showed significant variations in the chemical composition and in the antioxidant or antimicrobial activities even when the samples were collected from the same location. Propolis with high resin and low wax content had high level of antioxidant compounds, and strong antioxidant and antimicrobial activities. Gram-positive bacteria, especially, methicillin resistant Staphylococcus aureus were more sensitive to all propolis samples than Gram-negative bacteria and Candida albicans. Conclusions: The chemical composition and the antioxidant and antimicrobial activities of various propolis samples are different and rely on the geographic and plant origin of propolis collection. Propolis samples with low wax and high resin content might be more suitable to be used in future preclinical or clinical investigations.展开更多

文摘BACKGROUND Acute diverticulitis is one of the most prevalent complications of diverticular disease and may result in abscess formation, perforation, fistula formation, obstruction, or bleeding. The technique appears effective for well-encapsulated intra-abdominal abscesses larger than 4 cm in direct contact with the intestinal wall of left colon.

文摘BACKGROUND Pancreatoduodenectomy represents a complex procedure involving extensive organ resection and multiple alimentary reconstructions. It is still associated with high morbidity, even in high-volume centres.Prediction tools including preoperative patient-related factors to preoperatively identify patients at high risk for postoperative complications could enable tailored perioperative management and improve patient outcomes.AIM To evaluate the clinical significance of preoperative albumin-bilirubin score and other risk factors in relation to short-term postoperative outcomes in patients after open pancreatoduodenectomy.METHODS This retrospective study included all patients who underwent open pancreatic head resection(pylorus-preserving pancreatoduodenectomy or Whipple resection)for various pathologies during a five-year period(2017-2021)in a tertiary care setting at University Medical Centre Ljubljana,Slovenia and Cattinara Hospital,Trieste,Italy.Short-term postoperative outcomes, namely, postoperative complications, postoperative pancreatic fistula, reoperation, and mortality, were evaluated in association with albumin-bilirubin score and other risk factors.Multiple logistic regression models were built to identify risk factors associated with these short-term postoperative outcomes.RESULTS Data from 347 patients were collected.Postoperative complications, major postoperative complications, postoperative pancreatic fistula, reoperation, and mortality were observed in 52.7%,22.2%,23.9%,21.3%,and 5.2%of patients, respectively. There was no statistically significant association between the albumin-bilirubin score and any of these short-term postoperative complications based on univariate analysis. When controlling for other predictor variables in a logistic regression model, soft pancreatic texture was statistically significantly associated with postoperative complications[odds ratio(OR):2.09;95%confidence interval(95%CI):1.19-3.67];male gender(OR:2.12;95%CI:1.15-3.93),soft pancreatic texture(OR:3.06;95%CI:1.56-5.97),and blood loss(OR:1.07;95%CI:1.00-1.14)were statistically significantly associated with major postoperative complications;soft pancreatic texture was statistically significantly associated with the development of postoperative pancreatic fistula(OR:5.11;95%CI:2.38-10.95);male gender(OR:1.97;95%CI:1.01-3.83),soft pancreatic texture(OR:2.95;95%CI:1.42-6.11),blood loss(OR:1.08;95%CI:1.01-1.16),and resection due to duodenal carcinoma(OR:6.58;95%CI:1.20-36.15)were statistically significantly associated with reoperation.CONCLUSION The albumin-bilirubin score failed to predict short-term postoperative outcomes in patients undergoing pancreatoduodenectomy. However, other risk factors seem to influence postoperative outcomes, including male sex, soft pancreatic texture, blood loss, and resection due to duodenal carcinoma.
文摘The main goal of this article is to present a new result of a possible approach to the geometrical description of the birth and evolution of the universe. At the same time, a phase transition of the first kind occurs with the formation of a new phase.
文摘Douglas-fir(Pseudotsuga menziesii(Mirb.)Franco),one of the most commercially important tree species in western North America and one of the most valuable timber trees worldwide, was introduced to Europe in 1827.It became a major species for aff orestation in Western Europe after WWII,currently grows in 35 countries on over 0.83 million ha and is one of the most widespread nonnative tree species across the continent.A lower sensitivity to drought makes Douglas-fi r a potential alternative to the more drought-sensitive Norway spruce so its importance in Europe is expected to increase in the future. It is one of the fastest growing conifer species cultivated in Europe, with the largest reported dimensions of 2.3 m in diameter and 67.5 m in height.Pure stands have high productivity(up to 20 m 3 ha−1 a−1)and production(over 1000 m 3 ha−1).The species is generally regenerated by planting(initial stocking density from less than 1000 seedlings ha−1 to more than 4000 ha−1),using seedlings of European provenance derived from seed orchards or certifi ed seed stands. As the range of end-uses of its wood is very wide, the rotation period of Douglas-fi r is highly variable and ranges between 40 and 120 years. When the production of large-sized,knotfree timber is targeted, thinnings are always coupled with pruning up to 6 m.There is an increasing interest in growing Douglas-fi r in mixtures and managing stands through close-to-nature silviculture, but the species’intermediate shade tolerance means that it is best managed through group selection or shelterwood systems.
文摘BACKGROUND Abernethy syndrome is a congenital vascular anomaly in which the portal blood completely or partially bypasses the liver through a congenital portosystemic shunt.Although the number of recognized and reported cases is gradually increasing,Abernethy syndrome is still a rare disease entity, with an estimated prevalence between 1 per 30000 to 1 per 50000 cases. With this case series,we aimed to contribute to the growing knowledge of potential clinical presentations, course and complications of congenital portosystemic shunts(CPSS)in children.CASE SUMMARY Five children are presented in this case series:One female and four males, two with an intrahepatic CPSS and three with an extrahepatic CPSS.The first patient, who was diagnosed with an intrahepatic CPSS,presented with gastrointestinal bleeding, abdominal pain and hyperammonaemia at six years of age.He underwent a percutaneous embolization of his shunt and has remained asymptomatic ever since. The second patient presented with direct hyperbilirubinemia in the neonatal period and his intrahepatic CPSS later spontaneously regressed. The third patient had pulmonary hypertension and hyperammonaemia due to complete portal vein agenesis and underwent liver transplantation at five years of age.The fourth patient was diagnosed immediately after birth, when evaluated due to another congenital vascular anomaly, and the last patient presented as a teenager with recurrent bone fractures associated with severe osteoporosis. In addition, the last two patients are characterised by benign liver nodules;however, they are clinically stable on symptomatic therapy.CONCLUSION Abernethy syndrome is a rare anomaly with diverse clinical features, affecting almost all organ systems and presenting at any age.

文摘Establishment of embryonic stem cell(ESC) lines has been successful in mouse and human, but not in farm animals. Literature mining revealed 32 studies, describing i PSC generation in pig(13 studies), cattle(5),horse(5), sheep(4), goat(3), and rabbit(2) that are summarized in a concise, tabular format.
文摘Gastric cancer is in decline in most developed countries;however, it still accounts for a notable fraction of global mortality and morbidity related to cancer. The focus of this work is the identification of relevant molecular changes that could be detected non-invasively.

文摘AIM:To search or and validate di erentially expressed proteins in patients with gastric adenocarcinoma.METHODS:We used two-dimensional gel electrophoresis and mass spectrometry to search or di erentially expressed proteins in patients with gastric adenocarcinoma.A set o proteins was validated with immunoblotting.RESULTS:We identified 30 di erent proteins involved in various biological processes:metabolism, development, death, response to stress, cell cycle, cell communication, transport, and cell motility.Eight proteins were chosen or urther validation by immunoblotting. Our results show that gastrokine-1,39S ribosomal protein L12(mitochondrial precursor),plasma cell-induced resident endoplasmic reticulum protein, and glutathione S-trans erase mu 3 were signi icantly underexpressed in gastric adenocarcinoma relative to adjacent non-tumor tissue samples. On the other hand,septin-2,ubiquitin-conjugating enzyme E2 N,and transaldolase were significantly overexpressed.Translationally controlled tumor protein was shown to be di erentially expressed only in patients with cancer o the gastric cardia/esophageal border.CONCLUSION:This work presents a set o possible diagnostic biomarkers, validated or the first time.It might contribute to the e orts o understanding gastric cancer carcinogenesis.
文摘Objective: To assess the antioxidant content, antimicrobial and antioxidant activities of various propolis samples. Propolis samples with low wax and high resin content might be more suitable to be used in future preclinical or clinical investigations.

文摘The curing process of two biobased adhesives:pine tanninhexamine(TH)and organosolv lignin non-isocyanate polyurethane(NIPU),suitable for interior nonstructural use, were compared with commercial urea-formaldehyde(UF)adhesive.Changes in chemical structure before and after the curing process were observed with Fouriertransform infrared spectroscopy(FTIR).The process of adhesive curing was monitored with differential scanning calorimetry(DSC)and the automated bonding evaluation system(ABES).Both DSC and ABES measurements confirmed UF as the fastest and NIPU as the slowest curing adhesive observed.Taking into account the ABES results, the optimal pressing parameters for the TH adhesive would be 4 min at 175℃,for the NIPU adhesive 7 min at 200℃and for the UF 1.5 min at 100℃.Strong linear correlation was observed between mechanical and chemical curing for the UF and NIPU adhesives, whereas lower correlation was observed for the TH adhesive. At all observed adhesives, the DSC measurements were underestimating the curing process determined by ABES in the first part and overestimating it at the end.The underestimation was the most evident with the TH adhesive and the less with the UF adhesive. When comparing the uncured and cured FTIR spectra of all three types of adhesives,a drastic decrease in the characteristic band of-OH groups at 3330–3400 cm^(−1)and an increase in the signal intensity at 2920 cm^(−1)of aliphatic-CH2-groups were observed. For the UF adhesive, the C=O stretching frequency has shifted from 1632 cm^(−1)for uncured to three different bands at 1766,1701,and 1655 cm^(−1)for cured UF.The sharp band for phenolic alcohols at 1236 cm^(−1)of C–O stretch and hydroxyl O–H functional group at 1009 cm^(−1)and at 684 cm^(−1)of uncured TH adhesive diminished during curing, which indicates that a crosslinking reaction occurs via-OH groups. The peak of the C=O group of urethane bridges at 1697 cm^(−1)for uncured NIPU shifted to lower wavenumber at 1633 cm^(−1)for cured NIPU.
文摘BACKGROUND With improved survival in gastric cancer patients,health-related quality of life has become an important clinical endpoint alongside primary oncological outcomes.AIM To investigate health-related quality of life after various surgical procedures for gastric cancer treatment.METHODS The validated Slovenian version of the European Organization for Research and Treatment of Cancer Quality of Life Core Questionnaire(QLQ-C30)and its gastric cancer-specific module(QLQ STO-22)was sent for self-completion to patients that underwent curative resection for gastric adenocarcinoma between January 2014 and December 2018 at our centre. In total,116 patients responded.Scores were compared between patients after subtotal distal vs total gastrectomy and patients after subtotal distal gastrectomy with Billroth II vs Roux-en-Y reconstruction.RESULTS Interestingly, the extent of resection did not influence daily functioning;however, more dysphagia and eating restrictions were reported in patients after total gastrectomy when compared to patients after subtotal distal gastrectomy. Moreover, patients with Billroth II reconstruction after subtotal distal resection experienced worse physical and role functioning and reported more pain, fatigue and reflux compared to Roux-en-Y reconstruction.CONCLUSION Based on our results,Roux-en-Y reconstruction after subtotal distal gastrectomy should be preferred over Billroth II reconstruction. The data obtained from this study will help surgeons when preoperatively informing their patients about expected functional outcomes after gastrectomy and enable them to ensure proper supportive care of their patients in the postoperative period.
文摘BACKGROUND For locally advanced rectal cancer(LARC),standard therapy[consisting of neoadjuvant chemoradiotherapy(CRT),surgery, and adjuvant chemotherapy(ChT)]achieves excellent local control.Unfortunately, survival is still poor due to distant metastases, which remains the leading cause of death among these patients. In recent years, the concept of total neoadjuvant treatment(TNT)has been developed, whereby all systemic ChT-mainly affecting micrometastases-is applied prior to surgery.AIM To compare standard therapy and total neoadjuvant therapy for LARC patients with high-risk factors for failure.METHODS In a retrospective study,we compared LARC patients with high-risk factors for failure who were treated with standard therapy or with TNT.High-risk for failure was defined according to the presence of at least one of the following factors:T4 stage;N2 stage;positive mesorectal fascia;extramural vascular invasion;positive lateral lymph node.TNT consisted of 12 wk of induction ChT with capecitabine and oxaliplatin or folinic acid, fluorouracil and oxaliplatin,CRT with capecitabine, and 6-8 wk of consolidation ChT with capecitabine and oxaliplatin or folinic acid, fluorouracil and oxaliplatin prior to surgery. The primary endpoint was pathological complete response(pCR).In total,72 patients treated with standard therapy and 89 patients treated with TNT were included in the analysis.RESULTS Compared to standard therapy,TNT showed a higher proportion of pCR(23%vs 7%;P=0.01),a lower neoadjuvant rectal score(median:8.43 vs 14.98;P<0.05),higher T-and N-downstaging(70%and 94%vs 51%and 86%),equivalent R0 resection(95%vs 93%),shorter time to stoma closure(mean:20 vs 33 wk;P<0.05),higher compliance during systemic ChT(completed all cycles 87%vs 76%;P<0.05),lower proportion of acute toxicity grade≥3 during ChT(3%vs 14%,P<0.05),and equivalent acute toxicity and compliance during CRT and in the postoperative period. The pCR rate in patients treated with TNT was significantly higher in patients irradiated with intensity-modulated radiotherapy/volumetricmodulated arc radiotherapy than with 3D conformal radiotherapy(32%vs 9%;P<0.05).CONCLUSION Compared to standard therapy,TNT provides better outcome for LARC patients with high-risk factors for failure,in terms of pCR and neoadjuvant rectal score.
文摘This work studies large deflections of slender,non-prismatic cantilever beams subjected to a combined loading which consists of a non-uniformly distributed continuous load and a concentrated load at the free end of the beam. The material of the cantilever is assumed to be nonlinearly elastic.Different nonlinear relations between stress and strain in tensile and compressive domain are considered. The accuracy of numerical solutions is evaluated by comparing them with results from previous studies and with a laboratory experiment.

文摘Landslides influence the capacity for safe and sustainable development of mountainous environments. This study explores the spatial distribution of and the interactions between landslides that are mapped using global positioning system(GPS) and extensive field surveys in Mazandaran Province,Iran.Point-pattern assessment is undertaken using several univariate summary statistical functions, including pair correlation,spherical-contact distribution,nearest-neighbor analysis, and O-ring analysis,as well as bivariate summary statistics, and a markcorrelation function. The maximum entropy method was applied to prioritize the factors controlling the incidence of landslides and the landslides susceptibility map.The validation processes were considered for separated 30%data applying the ROC curves, fourfold plot, and Cohen’s kappa index. The results show that pair correlation and O-ring analyses satisfactorily predicted landslides at scales from 1 to 150 m.At smaller scales, from 150 to 400 m,landslides were randomly distributed. The nearest-neighbor distribution function show that the highest distance to the nearest landslide occurred in the 355 m.The spherical-contact distribution revealed that the patterns were random up to a spatial scale of 80 m.The bivariate correlation functions revealed that landslides were positively linked to several linear features(including faults, roads, and rivers) at all spatial scales. The mark-correlation function showed that aggregated fields of landslides were positively correlated with measures of land use, lithology, drainage density, plan curvature, and aspect, when the numbers of landslides in the groups were greater than the overall average aggregation. The results of analysis of factor importance have showed that elevation(topography map scale:1:25,000),distance to roads, and distance to rivers are the most important factors in the occurrence of landslides. The susceptibility model of landslides indicates an excellent accuracy,i.e.,the AUC value of landslides was 0.860.The susceptibility map of landslides analyzed has shown that 35% of the area is low susceptible to landslides.
文摘This paper investigates the structural properties of a model fluid dictated by an effective inter-particle oscillatory potential by grand canonical ensemble Monte Carlo (GCEMC) simulation and classical liquid state theories. The chosen oscillatory potential incorporates basic interaction terms used in modeling of various complex fluids which is composed of mesoscopic particles dispersed in a solvent bath, the studied structural properties include radial distribution function in bulk and inhomogeneous density distribution profile due to influence of several external fields. The GCEMC results are employed to test the validity of two recently proposed theoretical approaches in the field of atomic fluids. One is an Ornstein-Zernike integral equation theory approach;the other is a third order + second order perturbation density functional theory.Satisfactory agreement between the GCEMC simulation and the pure theories fully indicates the ready adaptability of the atomic fluid theories to effective model potentials in complex fluids, and classifies the proposed theoretical approaches as convenient tools for the investigation of complex fluids under the single component macro-fluid approximation.
