Schistosomiasis is an infection with one of a series of related trematode parasites that are endemic to at least 76 tropical and subtropical countries.Schistosomiasis affects over 200 million people in tropical and su...Schistosomiasis is an infection with one of a series of related trematode parasites that are endemic to at least 76 tropical and subtropical countries.Schistosomiasis affects over 200 million people in tropical and subtropical regions of South America,Africa and Asia.This paper reported more than 1500 soldiers of US troops and 560 men of Australian Air Force were affected schistosomiasis japonicum in the Leyte of the Philippines during the liberation of the Philippines from Octorber 1944 to the early months of 1945.Many troops there were affected by contaminated water with snail hosts.At first,the medical corps knew little about the situations of the schistosomiasis japonica in the Leyte,no preventive measure was adopted.On December 23,1944 CPT morris Goldberg discovered schistosomes first from the hospitalized soldiers while conducting a liver biopsy.Some soldiers with markedly signs and symptoms were admitted to hospital for treatment.Preventive measure was taken since.The US sent a lot of experts to Philippines for the research of schistosomiasis japonicum,including epidemiology, prevention,diagnosis,treatment and so on.Nineteen chemicals were used to control snail hosts. In clinical,schistosomiasis has acute and chronic stages,the manifestations are plenty and various,the liver, intestine,veins,spleen and skin etc are common sites of impingement.An extensive educational program was carried out in the troops through different means,which yielded remarkable effect,the new incidence of schistosomiasis dropped.Though hospitalized soldiers were a few,but there were scare and some morale and psychological problems among them and the other affected victims,they worried about their prognoses or being infected. Some patients were evacuated to United States for treatment after diagnosis,some patients were treated in Leyte.Before World WarⅡ,schistosomiasis was treated generally with trivalent antimony compounds,Fuadin and tartar emetic.In this event,no case died,relapses were rare.It was a successful campaign against schistosomiasis in the army.展开更多

Study Design: We conducted a survey of OB/GYNs (n = 220) and HEMs (n = 30) to describe the characteristics of current treatment strategies for postpartum hemorrhage. Surveys were administered via a structured questionnaire on a secure internet website from 5 - 12 October 2009. Results: The majority of OB/GYN and HEM respondents were practicing in a community hospital environment (77%). Of the OB/GYNs, the majority practiced at hospitals with over 2000 deliveries per year (77%). A majority (58%) of OB/GYNs were affiliated with hospitals that lacked a massive transfusion protocol to treat severe postpartum hemorrhage. Subsequent to uterine massage and additional oxytocin, the majority of OB/GYNs (73%), preferred the administration of Methergine? as the next level of intervention for postpartum hemorrhage. There was considerable variability in response to specific treatment strategies for several hypothetical case scenarios;however, the large majority of OB/GYNs favored obstetrical procedures over interventional radiology or administration of rFVIIa. A large majority (77%) of physicians who are familiar with rRVIIa as treatment for postpartum hemorrhage reported being very satisfied with the agent for this indication. Conclusions: An established, systematic treatment strategy among OB/GYNs emerged only in the case of mild postpartum hemorrhage.展开更多

During fifteen years of conflict in both operations US troops have incurred a total of 52,022 wounded and 6857 deaths. The aim of this analysis is to describe the characteristics of US military nurses that deployed to these combat theatres. Data for this retrospective analysis were provided by the US Department of Defense, Defense Manpower Data Center and describes characteristics military nurses that deployed to Afghanistan, Iraq, or both between September 1, 2001 and July 31, 2015. A total of 6326 nurses deployed during this period;3296 nurses deployed to Afghanistan and 2683 nurses deployed to Iraq. Fifty-five percent of deployed nurses were male. The majority of deployed nurses were white (32.9%) followed by Hispanic (22.9%) and black nurses (14.5%). Most deployed nurses had a baccalaureate nursing degree (88.9%) and 9% deployed nurses had an advanced degree. Most deployed nurses were classified as general nurses (66.6%). Further research is needed to examine in detail the impacts of deployments upon nurses.展开更多

Thalidomide is currently used world wide, including the United States, to treat erythema nodosum leprosum, multiple myeloma, refractory Crohn’s disease, aphthous stomatitis and HIV wasting syndrome. New cases of thalidomide phocomelia are being reported as well. We report a case of the anesthetic challenges of a 23 year-old parturient with thalidomide phocomelia and review the important anesthetic challenges it presents. Spontaneous vaginal delivery under continuous lumbar epidural was achieved in this challenging patient. However, it required careful planning for reliable intravenous access and the use of magnetic resonance imaging (MRI) of her pelvis and lumbar spine.展开更多
