Objectives:The antitumor effects of pyropheophorbide-αmethyl ester-mediated photodynamic therapy(MPPa-PDT)were observed in several cancers.The objective of this investigation was to examine the antineoplastic efficacy...Objectives:The antitumor effects of pyropheophorbide-αmethyl ester-mediated photodynamic therapy(MPPa-PDT)were observed in several cancers.The objective of this investigation was to examine the antineoplastic efficacy of MPPa-PDT acting on lung carcinoma A549 cells and further elaborate mechanisms.Methods:The viability of A549 cells was examined with cell counting kit-8 after MPPa-PDT disposal.Hoechst 33342 staining,monodansylcadaverine(MDC)staining,and transmission electron microscopy were employed to observe apoptotic bodies and autophagic vesicles.Flow cytometry with Annexin V/propidium iodide(PI)labeling objectively assessed cell death.The expression of associated proteins,including Caspase-3,Beclin-1,LC-3II,and mitogen-activated protein kinase(MAPK)families concluding c-jun NH2-terminal kinase(JNK),p38 MAPK,and extracellular signal-regulated kinase 1/2(ERK)were identified by Western blotting.Results:Prolonged exposure to MPPa-PDT gradually decreased lung cancer A549 cell viability.Apoptosis and autophagy activity were higher in the MPPa-PDT cohort in comparison to the control group.Meanwhile,autophagy inhibition enhanced cell-killing efficacy apparently.Besides,the JNK and p38 MAPK pathways were implicated in MPPa-PDT-triggered apoptosis and autophagy.Conclusions:This study demonstrated that JNK and p38 MAPK were engaged in MPPa-PDT-mediated apoptosis and autophagy in lung carcinoma A549 cells.展开更多
BACKGROUND The prevalence of metabolic dysfunction-associated fatty liver disease(MAFLD)is rapidly increasing,currently affecting approximately 25%of the global population.Liver fibrosis represents a crucial stage in ...BACKGROUND The prevalence of metabolic dysfunction-associated fatty liver disease(MAFLD)is rapidly increasing,currently affecting approximately 25%of the global population.Liver fibrosis represents a crucial stage in the development of MAFLD,with advanced liver fibrosis elevating the risks of cirrhosis and hepatocellular carcinoma.Simple serum markers are less effective in diagnosing liver fibrosis compared to more complex markers.However,imaging techniques like transient elastography face limitations in clinical application due to equipment and technical constraints.Consequently,it is imperative to identify a straightforward yet effective method for assessing MAFLD-associated liver fibrosis.AIM To investigate the predictive value of angiopoietin-like protein 8(ANGPTL8)in MAFLD and its progression.METHODS We analyzed 160 patients who underwent abdominal ultrasonography in the Endocrinology Department,Xiaogan Central Hospital affiliated to Wuhan University of Science and Technology,during September 2021-July 2022.Using abdominal ultrasonography and MAFLD diagnostic criteria,among the 160 patients,80 patients(50%)were diagnosed with MAFLD.The MAFLD group was divided into the liver fibrosis group(n=23)and non-liver fibrosis group(n=57)by using a cut-off fibrosis-4 index≥1.45.Logistical regression was used to analyze the risk of MAFLD and the risk factors for its progression.Receiver operating characteristic curves were used to evaluate the predictive value of serum ANGPTL8 in MAFLD and its progression.RESULTS Compared with non-MAFLD patients,MAFLD patients had higher serum ANGPTL8 and triglyceride-glucose(TyG)index(both P<0.05).Serum ANGPTL8(r=0.576,P<0.001)and TyG index(r=0.473,P<0.001)were positively correlated with MAFLD.Serum ANGPTL8 was a risk factor for MAFLD[odds ratio(OR):1.123,95%confidence interval(CI):1.066-1.184,P<0.001).Serum ANGPTL8 and ANGPTL8+TyG index predicted MAFLD[area under the curve(AUC):0.832 and 0.886,respectively;both P<0.05].Compared with MAFLD patients without fibrosis,those with fibrosis had higher serum ANGPTL8 and TyG index(both P<0.05),and both parameters were positively correlated with MAFLD-associated fibrosis.Elevated serum ANGPTL8(OR:1.093,95%CI:1.044-1.144,P<0.001)and TyG index(OR:2.383,95%CI:1.199-4.736,P<0.013)were risk factors for MAFLD-associated fibrosis.Serum ANGPTL8 and ANGPTL8+TyG index predicted MAFLD-associated fibrosis(AUC:0.812 and 0.835,respectively;both P<0.05).CONCLUSION The serum levels of ANGPTL8 are elevated and positively correlated with MAFLD.They can serve as predictors for the risk of MAFLD and liver fibrosis,with the ANGPTL8+TyG index potentially exhibiting even higher predictive value.展开更多

Twenty-eight...The purpose of this study was to evaluate the roles of different housing environments in neurological function, cerebral metabolism, cerebral infarction and neuron apoptosis after focal cerebral ischemia. Twenty-eight Sprague-Dawley rats were divided into control group (CG) and cerebral ischemia group, and the latter was further divided into subgroups of different housing conditions: standard environment (SE) subgroup, individual living environment (IE) subgroup, and enriched environment (EE) subgroup. Focal cerebral ischemia was induced by the middle cerebral artery occlusion (MCAO). Beam walking test was used to quantify the changes of overall motor function. Cerebral infarction and cerebral metabolism were studied by in vivo magnetic resonance imaging and 1H-magnetic resonance spectra, respectively. Neuron necrosis and apoptosis were detected by hematoxylin-eosin and TUNEL staining methods, respectively. The results showed that performance on the beam-walk test was improved in EE subgroup when compared to SE subgroup and IE subgroup. Cerebral infarct volume in IE subgroup was significantly larger than that in SE subgroup (P〈0.05) and EE subgroup (P〈0.05) on day 14 after MCAO. NAA/Cr and Cho/Cr ratios were lower in MCAO groups under different housing conditions as compared to those in CG (P〈0.05). NAA/Cr ratio was lower in IE subgroup (P〈0.05) and higher in EE subgroup (P〈0.05) than that in SE subgroup. NAA/ Cr ratio in EE was significantly higher than that in IE subgroup (P〈0.05). Cho/Cr ratio was decreased in MCAO groups as compared to that in CG (P〈0.05). A significant decrease in normal neurons in cerebral cortex was observed in MCAO groups as compared to CG (P〈0.05). The amount of normal neurons was less in IE subgroup (P〈0.05), and more in EE subgroup (P〈0.05) than that in SE subgroup after MCAO. The amotmt of normal neurons in EE subgroup was significantly more than that in IE subgroup after MCAO (P〈0.05). The ratio of TUNEL-positive neurons in EE was significantly lower than that in SE subgroup (P〈0.05) and IE subgroup (P〈0.05). Correlation analysis showed that the beam walking test was negatively correlated with NAA/Cr ratio (P〈0.05). Cerebral infarct volume was negatively correlated with both NAA/Cr ratio (P〈0.01) and Cho/Cr ratio (P〈0.01). The amount of normal cortical neurons was positively correlated with both NAA/Cr ratio (P〈0.0I) and Cho/Cr ratio (P〈0.05). The TUNEL-positive neurons showed a negative correlation with both NAA/Cr ratio (P〈0.01) and Cho/Cr ratio (P〈0.01). This study goes further to show that EE may improve neurological functional deficit and cerebral metabolism, decrease cerebral infarct volume, neuron necrosis and apoptosis, while IE may aggravate brain damage after MCAO.展开更多

The ischemia/ reperfusion heart mo del was established by ligating left anterior descending branch of coronary arte ry in Wistar rats. The apoptotic cardiomyocytes were examined under transmission electron microscopy and counted by in situ nick end labeling (TUNEL) method and light microscopy. Results showed that (1) The apoptotic cardiomyocytes were found in ischemic regions in the ischemia/reperfusion group, but not in the sh am oper ating group under transmission electron microscopy; (2) The number of apoptotic cells were 134.45±45.61/field in the ischemia/reperfusion group, 0/field in the sham operating group and 51.65±13.71/field in the ginsenoside Rb 1 treated group. The differences were significant among the three groups ( P <0.01). It was concluded that myocardial ischemia reperfusion could induce cardiomyocyte a poptosis, and ginsenoside Rb 1 could significantly inhibit cardiomyocyte apopto sis induced by ischemia reperfusion in rats, indicating that ginsenoside Rb 1 could inhibit cardiomyocyte apoptosis induced by ischemia reperfusion, thus alleviating ischemia reperfusion injury.展开更多

The survival time of patients with colorectal cancer and liver metastasis (CRLM) is short;not all patients can get radical resection of liver metastasis. For this part of patients, microwave ablation technology has been proved to be one of the effective methods for the treatment of liver metastasis. Laparoscopic B-ultrasound ablation also highlights a lot of minimally invasive advantages;this paper reviews the relevant literature of PubMed database, Wanfang database and CNKI database, in order to provide the treatment basis for clinical application of microwave ablation technology under laparoscopic B-ultrasound in the treatment of CRLM. The results showed that the safety and effectiveness of microwave ablation for liver metastases under the location of B-ultrasonic laparoscopy were confirmed, and patients with liver metastases of colorectal cancer who could not be resected could choose this treatment.展开更多

Conclusion: Skilled surgical procedures are performed in patients with local anesthesia, and some of the absorbable repair materials can be used. Methods: From August 2017 to June 2019, 42 patients with low lumbar single segment lumbar disc herniation were treated with percutaneous transforaminal endoscopic discectomy surgery in our hospital. The operation time, incision size, bleeding volume and hospitalization time were recorded respectively. The patients were evaluated before operation, 1 month and 6 months after operation. Visual analogue scale (VAS) and assessment were used to evaluate the lumbocrural pain. The JOA score and the Oswestry disability index (ODI) were used to evaluate the lumbar function, and the modified macnab score was used to evaluate the clinical effect in the last follow-up. Results: All the 42 patients successfully completed the operation without any other operation. There were no severe complications such as dural injury and nerve root injury. The operation time was (76.98 ± 8.58) min, the incision size was (8.45 ± 1.2) mm, the bleeding volume was (20.14 ± 2.93) ml, and the hospitalization time was (4.55 ± 1.13) d. One month and six months after the operation, the visual analogue scale (VAS), the evaluation of lumbar function (Oswestry) and the disability index (ODI) were significantly improved compared with those before the operation (P Conclusion: The treatment of low lumbar but segmental lumbar disc herniation with percutaneous intervertebral foramen, with small incision, less bleeding and quick recovery, can improve the pain and dysfunction of patients.展开更多
The applied value of serum hepcidin in differential diagnosis of infection fevers versus tumor fevers was explored.A total of 432 fever patients were selected according to the domestic fever of unknown origin(FUO) d...The applied value of serum hepcidin in differential diagnosis of infection fevers versus tumor fevers was explored.A total of 432 fever patients were selected according to the domestic fever of unknown origin(FUO) diagnostic criteria from our hospital between June 2010 and November 2013.Venous blood samples were taken on the day 1,5,10 after admission.The infection group(98 cases) and the tumor group(50 cases) were set up based on the clinical and laboratory findings.ELISA was used to determine the serum hepcidin and IL-6 levels.SPSS 13.0 was used for statistical analysis.Hepcidin showed obvious descending trend on the 10 th day in both the bacterial infection group(66 cases) and the virus infection group(32 cases),and the descending trend was similar to that of inflammatory indexes such as procalcitonin(PCT),hypersensitive C-reactive protein(h-CRP),erythrocyte sedimentation rate(ESR),white blood cell(WBC),and ferritin.Serum hepcidin showed no obvious differences in the tumor group on the day 1,5,10 after admission.In the infection groups,serum hepcidin was positively correlated with IL-6(r=0.687,P=0.000) and CRP(r=0.487,P=0.026),but had a poor correlation with blood sedimentation,ferritin,PCT and WBC(P〉0.05).Monitoring dynamic changes of hepcidin and related inflammatory factors in patients with fever is expected to be used for clinical identification of infection fever and tumor fever.展开更多

Methods: From September 2016 to September 2018, 28 patients with complex tibial plateau fractures were treated in our hospital. According to the odevity of hospitalized order, the patients were divided into two groups. Group A used 3D reconstruction, virtually reduction, 3D printing and demonstration of individual fracture model before operation while group B only received conventional process by use X-rays or CT image. Comparison between the two groups was made in operation time, operative blood loss, radiation frequency, surgery instrument cost and knee function score. Results: The follow-up was 14.4 months on average (ranged 6 to 22 months). There was no statistical difference of the surgery instrument cost between the 2 groups (P > 0.05). The operation time of group A was significantly shorter than that of group B (P χ2 = 0.373, P = 0.54). Conclusion: 3D printing techniques can improve surgery effect in complex tibial plateau fractures.展开更多

Methods: The clinical data of 146 patients with acute stanford type B aortic dissection treated with transseptal stent for aortic endovascular repair (TEVAR) from January 2012 to October 2017 in Xiaogan Central Hospital were analyzed. The time was divided into 3 groups, including the onset to TEVAR time ≤48 h group (41 cases in group A), the onset to TEVAR time 48 h - 7 d group (56 cases in group B), the onset to TEVAR time 7 d - 14 d group (49 cases in group C)). The clinical baseline data, the incidence of different complications during perioperative period, and the mortality rate at 30 days were compared between the three groups. Results: There were no significant differences in age, gender and comorbidities between the three groups (all P > 0.05). Group A had a clearer indication of immediate intervention compared with group B and group C (P P P > 0.05). Conclusion: Early intervention of acute TBAD may increase the risk of serious complications after surgery, and the incidence of serious complications will gradually decrease over time;the reduction of severe complications after early grouping is not accompanied by Early mortality and reoperation rates were significantly reduced, and TEVAR treatment in some patients with dissection did not prevent dissection progression and rupture.展开更多

With the recent transformation of the target of tumor therapy from tumor cells to tumor cell immune microenvironment, immunotherapy has emerged quietly. It is a new strategy for the treatment of hepatocellular carcinoma to improve the immune attack on tumor cells by changing the immunosuppressive environment of hepatocellular carcinoma cells. Immune checkpoint is the main mechanism by which liver cancer cells escape the host immune system. PD-1/PDL-1 and CTLA-4 are targeted immunocheckpoint inhibitors, which have shown good therapeutic effects and application prospects in the clinical treatment of HCC. Methods: The data of 128 patients with acute pancreatitis in the Xiaogan Central Hospital of Hubei from October 2016 to October 2017 were retrospectively analyzed. Clinical data of patients with fatty liver and non-fatty liver were statistically analyzed, the CT value of the liver and spleen was measure, and fatty liver was defined as mean L/SCT value Results: Among 128 patients, fatty liver was found in 56 (43.8%) and nonfatty liver in 72 (56.2%). Compared with patients with non fatty liver disease, the severity of pancreatitis, blood triglyceride and C reactive protein levels were higher in fatty liver patients, higher proportion of patients with local complications and persistent organ failure. Conclusion: Fatty liver has a greater impact on the severity of acute pancreatitis. It is possible to be an indicator of the severity of acute pancreatitis.展开更多

This study aimed to investigate the efficacy of early lifestyle intervention on high-risk groups of metabolic syndrome. Methods In a two-arm randomized controlled 1-year trial, we compared the effectiveness of a general recommendation-based program of lifestyle intervention carded out by wained professionals versus standard unstructured information given by physicians at reducing the prevalence of multiple metabolic and inflammatory abnormalities in 306 adults aged 45-64 years in Xiaogan city, China. Results At baseline, clinical/ anthropometric/laboratory and lifestyle characteristics of the intervention (n= 153 ) and control (n= 153) groups were not significantly different. The former significantly reduced total/saturated fat intake and increased polyunsaturated fat/fiber intake and exercise level compared to the controls. Weight, waist circumference, high-sensitivity C-reactive protein, and most of the metabolic syndrome components decreased in the intervention group and increased in the controls after 12 months. Lifestyle intervention significantly reduced metabolic syndrome (OR=0.28; 95% CI 0.18-0.44), with a 31% (95% C121-41) absolute risk reduction, corresponding to 3.2 (95% CI, 2-5) patients needing to be treated to prevent 1 case after 12 months. The intervention significantly reduced the prevalence of central obesity (OR=0.33; 95% CI, 0.20-0.56), and hypertriglyceridemia (OR=0.48; 95% CI, 0.31-0.75) and the incidence of diabetes (OR=0.23; 95% CI, 0.06-0.85). Conclusions A lifestyle intervention based on general recommendations was effective in reducing multiple metabolic/inflammatory abnormalities. The usual care by clinical physicians was ineffective at modifying progressive metabolic deterioration in high-risk individuals (J Geriatr Cardio12010; 7:10-16).展开更多
Objective To explore the association between lipid profiles and left ventricular hypertrophy in a Chinese general population.Methods We conducted a retrospective observational study to investigate the relationship bet...Objective To explore the association between lipid profiles and left ventricular hypertrophy in a Chinese general population.Methods We conducted a retrospective observational study to investigate the relationship between lipid markers[including triglycerides,total cholesterol,low-density lipoprotein cholesterol,high-density lipoprotein(HDL)cholesterol,non-HDL-cholesterol,apolipoprotein A-I,apolipoprotein B,lipoprotein[a],and composite lipid profiles]and left ventricular hypertrophy.A total of 309,400 participants of two populations(one from Beijing and another from nationwide)who underwent physical examinations at different health management centers between 2009 and 2018 in China were included in the cross-sectional study.7,475 participants who had multiple physical examinations and initially did not have left ventricular hypertrophy constituted a longitudinal cohort to analyze the association between lipid markers and the new-onset of left ventricular hypertrophy.Left ventricular hypertrophy was measured by echocardiography and defined as an end-diastolic thickness of the mterventricular septum or left ventricle posterior wall>11 mm.The Logistic regression model was used in the cross-sectional study.Cox model and Cox model with restricted cubic splines were used in the longitudinal cohort.Results In the cross-sectional study for participants in the highest tertile of each lipid marker compared to the respective lowest,triglycerides[odds ratio(OR):1.2S0,95%CI:1.060 to 1.474],HDL-cholesterol(OR:0.780,95%CI:0.662 to 0.918),and lipoprotein(a)(OR:1.311,95%C7:1.115 to 1.541)had an association with left ventricular hypertrophy.In the longitudinal cohort,for participants in the highest tertile of each lipid marker at the baseline compared to the respective lowest,triglycerides[hazard ratio(HR):3.277,95%C/:1.720 to 6.244],HDL-cholesterol(HR:0.516,95%C7:0.283 to 0.940),non-HDL-cholesterol(HR:2.309,95%C/:1.296 to 4.112),apolipoprotein B(HR:2.244,95%CI:1.251 to 4.032)showed an association with new-onset left ventricular hypertrophy.In the Cox model with forward stepwise selection,triglycerides were the only lipid markers entered into the final model.Conclusion Lipids levels,especially triglycerides,are associated with left ventricular hypertrophy.Controlling triglycerides level potentiate to be a strategy in harnessing cardiac remodeling but deserve to be furdier investigated.展开更多

In recent years, HSP90 inhibitors have made some achievements in molecular targeted therapy for malignant tumors, but further research is needed in clinical application. 文摘Objectives:The antitumor effects of pyropheophorbide-αmethyl ester-mediated photodynamic therapy(MPPa-PDT)were observed in several cancers.The objective of this investigation was to examine the antineoplastic efficacy of MPPa-PDT acting on lung carcinoma A549 cells and further elaborate mechanisms.Methods:The viability of A549 cells was examined with cell counting kit-8 after MPPa-PDT disposal.Hoechst 33342 staining,monodansylcadaverine(MDC)staining,and transmission electron microscopy were employed to observe apoptotic bodies and autophagic vesicles.Flow cytometry with Annexin V/propidium iodide(PI)labeling objectively assessed cell death.The expression of associated proteins,including Caspase-3,Beclin-1,LC-3II,and mitogen-activated protein kinase(MAPK)families concluding c-jun NH2-terminal kinase(JNK),p38 MAPK,and extracellular signal-regulated kinase 1/2(ERK)were identified by Western blotting.Results:Prolonged exposure to MPPa-PDT gradually decreased lung cancer A549 cell viability.Apoptosis and autophagy activity were higher in the MPPa-PDT cohort in comparison to the control group.Meanwhile,autophagy inhibition enhanced cell-killing efficacy apparently.Besides,the JNK and p38 MAPK pathways were implicated in MPPa-PDT-triggered apoptosis and autophagy.Conclusions:This study demonstrated that JNK and p38 MAPK were engaged in MPPa-PDT-mediated apoptosis and autophagy in lung carcinoma A549 cells.

文摘BACKGROUND The prevalence of metabolic dysfunction-associated fatty liver disease(MAFLD)is rapidly increasing,currently affecting approximately 25%of the global population.Liver fibrosis represents a crucial stage in the development of MAFLD,with advanced liver fibrosis elevating the risks of cirrhosis and hepatocellular carcinoma.Simple serum markers are less effective in diagnosing liver fibrosis compared to more complex markers.However,imaging techniques like transient elastography face limitations in clinical application due to equipment and technical constraints.Consequently,it is imperative to identify a straightforward yet effective method for assessing MAFLD-associated liver fibrosis.AIM To investigate the predictive value of angiopoietin-like protein 8(ANGPTL8)in MAFLD and its progression.METHODS We analyzed 160 patients who underwent abdominal ultrasonography in the Endocrinology Department,Xiaogan Central Hospital affiliated to Wuhan University of Science and Technology,during September 2021-July 2022.Using abdominal ultrasonography and MAFLD diagnostic criteria,among the 160 patients,80 patients(50%)were diagnosed with MAFLD.The MAFLD group was divided into the liver fibrosis group(n=23)and non-liver fibrosis group(n=57)by using a cut-off fibrosis-4 index≥1.45.Logistical regression was used to analyze the risk of MAFLD and the risk factors for its progression.Receiver operating characteristic curves were used to evaluate the predictive value of serum ANGPTL8 in MAFLD and its progression.RESULTS Compared with non-MAFLD patients,MAFLD patients had higher serum ANGPTL8 and triglyceride-glucose(TyG)index(both P<0.05).Serum ANGPTL8(r=0.576,P<0.001)and TyG index(r=0.473,P<0.001)were positively correlated with MAFLD.Serum ANGPTL8 was a risk factor for MAFLD[odds ratio(OR):1.123,95%confidence interval(CI):1.066-1.184,P<0.001).Serum ANGPTL8 and ANGPTL8+TyG index predicted MAFLD[area under the curve(AUC):0.832 and 0.886,respectively;both P<0.05].Compared with MAFLD patients without fibrosis,those with fibrosis had higher serum ANGPTL8 and TyG index(both P<0.05),and both parameters were positively correlated with MAFLD-associated fibrosis.Elevated serum ANGPTL8(OR:1.093,95%CI:1.044-1.144,P<0.001)and TyG index(OR:2.383,95%CI:1.199-4.736,P<0.013)were risk factors for MAFLD-associated fibrosis.Serum ANGPTL8 and ANGPTL8+TyG index predicted MAFLD-associated fibrosis(AUC:0.812 and 0.835,respectively;both P<0.05).CONCLUSION The serum levels of ANGPTL8 are elevated and positively correlated with MAFLD.They can serve as predictors for the risk of MAFLD and liver fibrosis,with the ANGPTL8+TyG index potentially exhibiting even higher predictive value.
文摘BACKGROUND Clinically amyopathic deramatomyositis was manifested as the various cutaneous dermatomyositis(DM)manifestations without muscle weakness.Anti-melanoma differentiation-associated gene 5(anti-MDA5)and anti-Ro52 antibody-dual positive clinically amyopathic DM patients are at a high risk of developing rapidly progressive interstitial lung disease,and they exhibit an immensely high half-year mortality.CASE SUMMARY We presented three patients with anti-MDA5 and anti-Ro52 antibody-dual positive DM patients and we reviewed the previous studies on the link between anti-MDA5 and anti-Ro52 antibody-dual positive DM.Although we aggressively treated these patients similarly,but they all exhibited different prognoses.We reviewed the importance of clinical cutaneous rashes as well as the pathogenesis and treatment in the dual positive anti-MDA5 and anti-Ro52 associated DM.CONCLUSION Patients with anti-MDA5 anti-Ro52 antibody-dual positive DM should be accurately diagnosed at an early stage and should be treated aggressively,thus,the patient’s prognosis can be significantly modified.
文摘The purpose of this study was to evaluate the roles of different housing environments in neurological function, cerebral metabolism, cerebral infarction and neuron apoptosis after focal cerebral ischemia. Twenty-eight Sprague-Dawley rats were divided into control group (CG) and cerebral ischemia group, and the latter was further divided into subgroups of different housing conditions: standard environment (SE) subgroup, individual living environment (IE) subgroup, and enriched environment (EE) subgroup. Focal cerebral ischemia was induced by the middle cerebral artery occlusion (MCAO). Beam walking test was used to quantify the changes of overall motor function. Cerebral infarction and cerebral metabolism were studied by in vivo magnetic resonance imaging and 1H-magnetic resonance spectra, respectively. Neuron necrosis and apoptosis were detected by hematoxylin-eosin and TUNEL staining methods, respectively. The results showed that performance on the beam-walk test was improved in EE subgroup when compared to SE subgroup and IE subgroup. Cerebral infarct volume in IE subgroup was significantly larger than that in SE subgroup (P〈0.05) and EE subgroup (P〈0.05) on day 14 after MCAO. NAA/Cr and Cho/Cr ratios were lower in MCAO groups under different housing conditions as compared to those in CG (P〈0.05). NAA/Cr ratio was lower in IE subgroup (P〈0.05) and higher in EE subgroup (P〈0.05) than that in SE subgroup. NAA/ Cr ratio in EE was significantly higher than that in IE subgroup (P〈0.05). Cho/Cr ratio was decreased in MCAO groups as compared to that in CG (P〈0.05). A significant decrease in normal neurons in cerebral cortex was observed in MCAO groups as compared to CG (P〈0.05). The amount of normal neurons was less in IE subgroup (P〈0.05), and more in EE subgroup (P〈0.05) than that in SE subgroup after MCAO. The amotmt of normal neurons in EE subgroup was significantly more than that in IE subgroup after MCAO (P〈0.05). The ratio of TUNEL-positive neurons in EE was significantly lower than that in SE subgroup (P〈0.05) and IE subgroup (P〈0.05). Correlation analysis showed that the beam walking test was negatively correlated with NAA/Cr ratio (P〈0.05). Cerebral infarct volume was negatively correlated with both NAA/Cr ratio (P〈0.01) and Cho/Cr ratio (P〈0.01). The amount of normal cortical neurons was positively correlated with both NAA/Cr ratio (P〈0.0I) and Cho/Cr ratio (P〈0.05). The TUNEL-positive neurons showed a negative correlation with both NAA/Cr ratio (P〈0.01) and Cho/Cr ratio (P〈0.01). This study goes further to show that EE may improve neurological functional deficit and cerebral metabolism, decrease cerebral infarct volume, neuron necrosis and apoptosis, while IE may aggravate brain damage after MCAO.

文摘The effect of ginsenoside Rb 1 on cardiomyocyte apotosis after ischemia (30 min ) and reperfusion (6 h) in rats was observed. The ischemia/ reperfusion heart mo del was established by ligating left anterior descending branch of coronary arte ry in Wistar rats. The apoptotic cardiomyocytes were examined under transmission electron microscopy and counted by in situ nick end labeling (TUNEL) method and light microscopy. Results showed that (1) The apoptotic cardiomyocytes were found in ischemic regions in the ischemia/reperfusion group, but not in the sh am oper ating group under transmission electron microscopy; (2) The number of apoptotic cells were 134.45±45.61/field in the ischemia/reperfusion group, 0/field in the sham operating group and 51.65±13.71/field in the ginsenoside Rb 1 treated group. The differences were significant among the three groups ( P <0.01). It was concluded that myocardial ischemia reperfusion could induce cardiomyocyte a poptosis, and ginsenoside Rb 1 could significantly inhibit cardiomyocyte apopto sis induced by ischemia reperfusion in rats, indicating that ginsenoside Rb 1 could inhibit cardiomyocyte apoptosis induced by ischemia reperfusion, thus alleviating ischemia reperfusion injury.
文摘Liver is the most common metastasis target organ in the late stage of colorectal cancer. More than 50% of colorectal cancer patients will have simultaneous or heterochronous liver metastasis. The survival time of patients with colorectal cancer and liver metastasis (CRLM) is short;not all patients can get radical resection of liver metastasis. For this part of patients, microwave ablation technology has been proved to be one of the effective methods for the treatment of liver metastasis. Laparoscopic B-ultrasound ablation also highlights a lot of minimally invasive advantages;this paper reviews the relevant literature of PubMed database, Wanfang database and CNKI database, in order to provide the treatment basis for clinical application of microwave ablation technology under laparoscopic B-ultrasound in the treatment of CRLM. The results showed that the safety and effectiveness of microwave ablation for liver metastases under the location of B-ultrasonic laparoscopy were confirmed, and patients with liver metastases of colorectal cancer who could not be resected could choose this treatment.

This concept not only helps patients speed up postoperative recovery, reduce the incidence of related complications and shorten hospital stays, but also has been proved to be effective and safe in the perioperative application of gastric cancer. This article reviews the clinical application and research progress of enhanced recovery after surgery in the perioperative period of advanced gastric cancer in the elderly.
文摘Purpose: Analyzing the causes of pain after tension-free repair in the inguinal hernia, and propose corresponding treatment strategies. Results: The patients in this group were followed up for 3 - 12 months. A total of 5 patients still had pain relief within 3 months after surgery. Further treatment was needed, and the incidence rate was 5%. Conclusion: Skilled surgical procedures are performed in patients with local anesthesia, and some of the absorbable repair materials can be used. The improved patch fixation and extra peritoneal repair can reduce the pain after inguinal hernia repair without tension.
文摘Objective: To observe the clinical effect percutaneous transforaminal endoscopic discectomy (PTED) in the treatment of single lumbar disc herniation. Methods: From August 2017 to June 2019, 42 patients with low lumbar single segment lumbar disc herniation were treated with percutaneous transforaminal endoscopic discectomy surgery in our hospital. The operation time, incision size, bleeding volume and hospitalization time were recorded respectively. The patients were evaluated before operation, 1 month and 6 months after operation. Visual analogue scale (VAS) and assessment were used to evaluate the lumbocrural pain. The JOA score and the Oswestry disability index (ODI) were used to evaluate the lumbar function, and the modified macnab score was used to evaluate the clinical effect in the last follow-up. Results: All the 42 patients successfully completed the operation without any other operation. There were no severe complications such as dural injury and nerve root injury. The operation time was (76.98 ± 8.58) min, the incision size was (8.45 ± 1.2) mm, the bleeding volume was (20.14 ± 2.93) ml, and the hospitalization time was (4.55 ± 1.13) d. One month and six months after the operation, the visual analogue scale (VAS), the evaluation of lumbar function (Oswestry) and the disability index (ODI) were significantly improved compared with those before the operation (P Conclusion: The treatment of low lumbar but segmental lumbar disc herniation with percutaneous intervertebral foramen, with small incision, less bleeding and quick recovery, can improve the pain and dysfunction of patients.
文摘The applied value of serum hepcidin in differential diagnosis of infection fevers versus tumor fevers was explored.A total of 432 fever patients were selected according to the domestic fever of unknown origin(FUO) diagnostic criteria from our hospital between June 2010 and November 2013.Venous blood samples were taken on the day 1,5,10 after admission.The infection group(98 cases) and the tumor group(50 cases) were set up based on the clinical and laboratory findings.ELISA was used to determine the serum hepcidin and IL-6 levels.SPSS 13.0 was used for statistical analysis.Hepcidin showed obvious descending trend on the 10 th day in both the bacterial infection group(66 cases) and the virus infection group(32 cases),and the descending trend was similar to that of inflammatory indexes such as procalcitonin(PCT),hypersensitive C-reactive protein(h-CRP),erythrocyte sedimentation rate(ESR),white blood cell(WBC),and ferritin.Serum hepcidin showed no obvious differences in the tumor group on the day 1,5,10 after admission.In the infection groups,serum hepcidin was positively correlated with IL-6(r=0.687,P=0.000) and CRP(r=0.487,P=0.026),but had a poor correlation with blood sedimentation,ferritin,PCT and WBC(P〉0.05).Monitoring dynamic changes of hepcidin and related inflammatory factors in patients with fever is expected to be used for clinical identification of infection fever and tumor fever.
文摘Objective: To investigate the value of 3D printing techniques in the treatment of complex tibial plateau fractures. Methods: From September 2016 to September 2018, 28 patients with complex tibial plateau fractures were treated in our hospital. According to the odevity of hospitalized order, the patients were divided into two groups. Group A used 3D reconstruction, virtually reduction, 3D printing and demonstration of individual fracture model before operation while group B only received conventional process by use X-rays or CT image. Comparison between the two groups was made in operation time, operative blood loss, radiation frequency, surgery instrument cost and knee function score. Results: The follow-up was 14.4 months on average (ranged 6 to 22 months). There was no statistical difference of the surgery instrument cost between the 2 groups (P > 0.05). The operation time of group A was significantly shorter than that of group B (P χ2 = 0.373, P = 0.54). Conclusion: 3D printing techniques can improve surgery effect in complex tibial plateau fractures.
文摘Objective: To investigate the relationship between early intervention timing and complications of acute Stanford type B aortic dissection. Methods: The clinical data of 146 patients with acute stanford type B aortic dissection treated with transseptal stent for aortic endovascular repair (TEVAR) from January 2012 to October 2017 in Xiaogan Central Hospital were analyzed. The time was divided into 3 groups, including the onset to TEVAR time ≤48 h group (41 cases in group A), the onset to TEVAR time 48 h - 7 d group (56 cases in group B), the onset to TEVAR time 7 d - 14 d group (49 cases in group C)). The clinical baseline data, the incidence of different complications during perioperative period, and the mortality rate at 30 days were compared between the three groups. Results: There were no significant differences in age, gender and comorbidities between the three groups (all P > 0.05). Group A had a clearer indication of immediate intervention compared with group B and group C (P P P > 0.05). Conclusion: Early intervention of acute TBAD may increase the risk of serious complications after surgery, and the incidence of serious complications will gradually decrease over time;the reduction of severe complications after early grouping is not accompanied by Early mortality and reoperation rates were significantly reduced, and TEVAR treatment in some patients with dissection did not prevent dissection progression and rupture.
文摘The current treatments of liver cancer in China are mainly comprehensive treatment and systematic treatment, with poor therapeutic effect and high recurrence rate and metastasis rate. The existence of immunosuppressive microenvironment is an important reason for liver tumor cells to escape from the host immune system, and also an important basis for the occurrence and development of liver cancer. With the recent transformation of the target of tumor therapy from tumor cells to tumor cell immune microenvironment, immunotherapy has emerged quietly. It is a new strategy for the treatment of hepatocellular carcinoma to improve the immune attack on tumor cells by changing the immunosuppressive environment of hepatocellular carcinoma cells. Immune checkpoint is the main mechanism by which liver cancer cells escape the host immune system. PD-1/PDL-1 and CTLA-4 are targeted immunocheckpoint inhibitors, which have shown good therapeutic effects and application prospects in the clinical treatment of HCC. This article reviews the latest advances in immunocheckpoint inhibitors in the immunotherapy of hepatocellular carcinoma.

文摘Objective: We aim to explore the relationship between fatty liver and the severity of acute pancreatitis. Methods: The data of 128 patients with acute pancreatitis in the Xiaogan Central Hospital of Hubei from October 2016 to October 2017 were retrospectively analyzed. Clinical data of patients with fatty liver and non-fatty liver were statistically analyzed, the CT value of the liver and spleen was measure, and fatty liver was defined as mean L/SCT value Results: Among 128 patients, fatty liver was found in 56 (43.8%) and nonfatty liver in 72 (56.2%). Compared with patients with non fatty liver disease, the severity of pancreatitis, blood triglyceride and C reactive protein levels were higher in fatty liver patients, higher proportion of patients with local complications and persistent organ failure. Conclusion: Fatty liver has a greater impact on the severity of acute pancreatitis. It is possible to be an indicator of the severity of acute pancreatitis.

文摘Objective Intensive lifestyle intervention significantly reduces the progression to diabetes in high-risk individuals. This study aimed to investigate the efficacy of early lifestyle intervention on high-risk groups of metabolic syndrome. Methods In a two-arm randomized controlled 1-year trial, we compared the effectiveness of a general recommendation-based program of lifestyle intervention carded out by wained professionals versus standard unstructured information given by physicians at reducing the prevalence of multiple metabolic and inflammatory abnormalities in 306 adults aged 45-64 years in Xiaogan city, China. Results At baseline, clinical/ anthropometric/laboratory and lifestyle characteristics of the intervention (n= 153 ) and control (n= 153) groups were not significantly different. The former significantly reduced total/saturated fat intake and increased polyunsaturated fat/fiber intake and exercise level compared to the controls. Weight, waist circumference, high-sensitivity C-reactive protein, and most of the metabolic syndrome components decreased in the intervention group and increased in the controls after 12 months. Lifestyle intervention significantly reduced metabolic syndrome (OR=0.28; 95% CI 0.18-0.44), with a 31% (95% C121-41) absolute risk reduction, corresponding to 3.2 (95% CI, 2-5) patients needing to be treated to prevent 1 case after 12 months. The intervention significantly reduced the prevalence of central obesity (OR=0.33; 95% CI, 0.20-0.56), and hypertriglyceridemia (OR=0.48; 95% CI, 0.31-0.75) and the incidence of diabetes (OR=0.23; 95% CI, 0.06-0.85). Conclusions A lifestyle intervention based on general recommendations was effective in reducing multiple metabolic/inflammatory abnormalities. The usual care by clinical physicians was ineffective at modifying progressive metabolic deterioration in high-risk individuals (J Geriatr Cardio12010; 7:10-16).

文摘Objective To explore the association between lipid profiles and left ventricular hypertrophy in a Chinese general population.Methods We conducted a retrospective observational study to investigate the relationship between lipid markers[including triglycerides,total cholesterol,low-density lipoprotein cholesterol,high-density lipoprotein(HDL)cholesterol,non-HDL-cholesterol,apolipoprotein A-I,apolipoprotein B,lipoprotein[a],and composite lipid profiles]and left ventricular hypertrophy.A total of 309,400 participants of two populations(one from Beijing and another from nationwide)who underwent physical examinations at different health management centers between 2009 and 2018 in China were included in the cross-sectional study.7,475 participants who had multiple physical examinations and initially did not have left ventricular hypertrophy constituted a longitudinal cohort to analyze the association between lipid markers and the new-onset of left ventricular hypertrophy.Left ventricular hypertrophy was measured by echocardiography and defined as an end-diastolic thickness of the mterventricular septum or left ventricle posterior wall>11 mm.The Logistic regression model was used in the cross-sectional study.Cox model and Cox model with restricted cubic splines were used in the longitudinal cohort.Results In the cross-sectional study for participants in the highest tertile of each lipid marker compared to the respective lowest,triglycerides[odds ratio(OR):1.2S0,95%CI:1.060 to 1.474],HDL-cholesterol(OR:0.780,95%CI:0.662 to 0.918),and lipoprotein(a)(OR:1.311,95%C7:1.115 to 1.541)had an association with left ventricular hypertrophy.In the longitudinal cohort,for participants in the highest tertile of each lipid marker at the baseline compared to the respective lowest,triglycerides[hazard ratio(HR):3.277,95%C/:1.720 to 6.244],HDL-cholesterol(HR:0.516,95%C7:0.283 to 0.940),non-HDL-cholesterol(HR:2.309,95%C/:1.296 to 4.112),apolipoprotein B(HR:2.244,95%CI:1.251 to 4.032)showed an association with new-onset left ventricular hypertrophy.In the Cox model with forward stepwise selection,triglycerides were the only lipid markers entered into the final model.Conclusion Lipids levels,especially triglycerides,are associated with left ventricular hypertrophy.Controlling triglycerides level potentiate to be a strategy in harnessing cardiac remodeling but deserve to be furdier investigated.
文摘Heat shock protein (HSP) is a kind of protein that mainly acts as a molecular chaperone to participate in the synthesis and folding of proteins, maintain the spatial conformation of proteins and protect cells from damage and other important biological functions. HSP90 plays an important role in maintaining molecular chaperone structure, regulating cell cycle and apoptosis, coordinating hormone signal transduction and promoting wound healing. And HSP90 also plays an important role in the occurrence and progression of tumors. In recent years, HSP90 inhibitors have made some achievements in molecular targeted therapy for malignant tumors, but further research is needed in clinical application. In this paper, the research status of the relationship between hepatocellular carcinoma targeted by heat shock protein 90 was reviewed.
