极低出生体重早产儿纯母乳喂养的价值研究 被引量:30

目的探讨极低出生体重早产儿(VLBWI)开展纯母乳喂养的临床价值。方法选取2016年7月-2017年12月在我院新生儿重症监护室(NICU)住院的118例VLBWI为研究对象,按入院时间顺序随机分为观察组60例和对照组58例。观察组实施纯母乳喂养,对照组... 目的探讨极低出生体重早产儿(VLBWI)开展纯母乳喂养的临床价值。方法选取2016年7月-2017年12月在我院新生儿重症监护室(NICU)住院的118例VLBWI为研究对象,按入院时间顺序随机分为观察组60例和对照组58例。观察组实施纯母乳喂养,对照组实施早产儿院内液态配方奶喂养。观察两组患儿营养情况及并发症发生率,并进行成本-效益分析。结果观察组恢复至出生体质量时间与持续静脉营养时间均长于对照组,差异均有统计学意义(P<0.05),但体质量增长速率与对照组差异无统计学意义(P>0.05);观察组喂养不耐受、NEC、败血症、BPD、早产儿视网膜病的发生率均低于对照组(P<0.05);观察组住院时间、总住院费用均低于对照组(P<0.05),效益成本比率为86.12∶1。结论对VLBWI采取纯母乳喂养,可以促进生长发育,降低相关并发症的发生,同时可有效节约医疗费用,值得推广。 展开更多

目的对ICU患者家属迁移应激量表(Family relocation stress scale,FRSS)进行汉化及其信度和效度评价。方法按照Brislin翻译模型的两人直译-回译法,进行翻译、回译和文化调适,并对20例ICU患者家属进行预调查,形成中文版的FRSS量表;采用... 目的对ICU患者家属迁移应激量表(Family relocation stress scale,FRSS)进行汉化及其信度和效度评价。方法按照Brislin翻译模型的两人直译-回译法,进行翻译、回译和文化调适,并对20例ICU患者家属进行预调查,形成中文版的FRSS量表;采用方便抽样法选取浙江省某三级甲等医院105例ICU患者家属进行调查,以评价中文版FRSS量表的信度和效度。结果中文版FRSS量表共有14个条目。项目分析各条目临界比值均有统计学意义(P<0.001),且各条目与量表总分的相关系数达到0.4以上;量表的单条目内容效度指数(Item-level CVI,I-CVI)值均达到0.833以上,总条目CVI(scale-level CVI,S-CVI)为0.952。量表共提取4个公因子,累计解释变异量为71.576%,且各条目在相应的公因子的负荷量>0.4;量表的Cronbach’sα系数为0.857,折半系数为0.743。结论中文版FRSS量表具有良好的信度和效度,可作为ICU患者家属迁移应激的测评工具。 展开更多

外源H_(2)S缓解小麦苗期干旱胁迫的生理生化基础 被引量:3

小麦是人类重要的粮食作物之一,而其生长发育过程常受干旱胁迫,致使减产降质.随着全球气候变化,极端天气频发,小麦受旱面积与旱灾程度正逐年加剧.探究小麦抗旱调控机制,增强小麦抗旱能力,对于缓解小麦干旱胁迫造成的减产降质具有重要意... 小麦是人类重要的粮食作物之一,而其生长发育过程常受干旱胁迫,致使减产降质.随着全球气候变化,极端天气频发,小麦受旱面积与旱灾程度正逐年加剧.探究小麦抗旱调控机制,增强小麦抗旱能力,对于缓解小麦干旱胁迫造成的减产降质具有重要意义.前期研究发现低浓度的H_(2)S可显著缓解小麦幼苗干旱胁迫.笔者以20%聚乙二醇(PEG-6000)模拟中度干旱胁迫处理水培百农207小麦幼苗,以1 mmol·L^(-1)NaHS为H_(2)S供体对小麦幼苗进行叶面喷施处理,研究结果表明,干旱胁迫条件下,与未喷施NaHS的干旱处理组相比,外源喷施NaHS 7 d时可显著缓解干旱胁迫对小麦幼苗生长的抑制效应,主要表现在:株高和根长分别显著增加9.63%和19.7%;地上部分及地下部分干重分别显著增加17.63%和12.58%;内源H_(2)S显著增加了43.76%;光合效率显著增加了3.02%;超氧化物歧化酶、过氧化物酶和抗坏血酸过氧化物酶的活性分别显著增加了13.83%,11.57%,41.87%;丙二醛和过氧化氢的含量显著降低了36.1%和65.72%;脯氨酸显著增加了48.22%;酸性磷酸酶及碱性磷酸酶活性分别显著增加了62.87%及22.03%.因此,干旱胁迫下叶面外源喷施NaHS缓解了小麦幼苗的形态学指标及光合生理性能的下降,增强了抗氧化能力及渗透调节能力,提升了小麦应对干旱胁迫的能力,与此同时,干旱胁迫下外源喷施NaHS提升了小麦幼苗酸性磷酸酶及碱性磷酸酶的活性水平.以上结果为进一步探究外源H_(2)S缓解小麦干旱胁迫的磷酸酶分子调控机制奠定基础. 展开更多

Purpose:To investigate the fundus and fluorescein angiographic features in the patients with radiation retinopathy.Clinical Materials:Color fundus photography and/or fluorescein angiography from 13 patients with nasopharyngeal carcinomas received external beam radiation were retrospectively analyzed.Reslts:In this study,26 damaged eyes of 13 patients eveloped some degree of radiation retinopathy.The earliest and most common finding was macular microvascular changes (microaneurysms and/or telangiectasia),which was observed in 100%(26/26)of the eyes.Intraretinal hemorrhages,macular capillary nonperfusion,and macular edema were noted in 84%,50%,and 42% of the eyes,respectively.Conclusions:Radiation retinopathy is common after external beam radiation of nasopharyngeal carcinomas.The prominent changes include maular microvascular changes,intraretinal hemorrhages and macular capillary nonperfusion. 展开更多

Objective:To observe the clinical efficacy of mind-refreshing and balance-restoring needling method combined with Frenkel exercises in treating ataxia after cerebral stroke.Methods:The recruited 120 patients were randomized into an observation group and a control group,with 60 cases in each group.The control group was intervened by mind-refreshing and balance-restoring needling method,while the observation group was given additional lower-limb Frenkel exercises.Before and after treatment and at the follow-up,the ataxic lower-limb function was scored using Berg balance scale(BBS)and international cooperative ataxia rating scale(ICARS),and Barthel index(BI)was adopted to score the activities of daily living(ADL).Results:After treatment,the markedly effective rate was 70.2%and the total effective rate was 96.5%in the observation group,versus 39.7%and 87.9%in the control group,and the differences in the markedly effective rate and the total effective rate were statistically significant(P<0.01,P<0.05).The intra-group comparisons showed that the BBS,ICARS and BI scores after treatment and at the follow-up were significantly different from those before treatment in both groups(all P<0.01).There were significant differences in the BBS score between the two groups after treatment and at the follow-up(P<0.05,P<0.01);the between-group differences in the ICARS and BI scores were statistically insignificant after treatment(both P>0.05),while the between-group differences in the ICARS and BI scores were statistically significant at the follow-up(both P<0.05).The interaction effects between the scoring time of BBS and BI and the group factor were statistically significant(P<0.01,P<0.05).Conclusion:Mind-refreshing and balance-restoring needling can effectively improve the lower-limb ataxic symptoms and ADL after stroke;when combined with Fenkel exercises,this needling method can produce more significant efficacy. 展开更多

To improve the light environment and welfare of the turtle cultured indoors,the effects of lighting mode on growth performance,cortisol level,and oxidative stress of juvenile Chinese three-keeled pond turtle,Chinemys reevesii,were investigated in this study.The experimental turtles with an initial weight of 5.61±0.09 g were reared in tanks under four different lighting modes:three groups with light(lighting the basking area and water area,LBW;lighting the water area only,LW;lighting the basking area only,LB)and control group(no light,NL).The experiment was conducted for more than six months,with each group having three replicates.After 203 d of the experiment,the turtle in the LW group exhibited higher weight gain rate(WGR)and a specific growth rate(SGR,%/d)compared to other treatments.Also,results showed that the final body weight of the turtle exposed to LW was higher than that exposed to other treatments.On the physiological level,serum cortisol level in turtles exposed to LW was significantly lower than that in other treatments.Regarding oxidative stress,the level of catalase(CAT)in turtles exposed to LW and LB was significantly lower than that exposed to LBW and NL.The malonaldehyde(MDA)activity in turtles exposed to LW was significantly lower than other treatments.Based on the growth performance and health status,it is suggested that lighting the water area only is the optimal lighting mode for the juvenile threekeeled pond turtle cultured indoors. 展开更多

Chiglitazar(Carfloglitazar)is a novel peroxisome proliferator-activated receptor(PPAR)pan-agonist that has shown promising effects on glycemic control and lipid regulation in patients with type 2 diabetes.In this randomized phase 3 trial,we compared the efficacy and safety of chiglitazar with sitagliptin in patients with type 2 diabetes who had insufficient glycemic control despite a strict diet and exercise regimen.Eligible patients were randomized(1:1:1)to receive chiglitazar 32 mg(n=245),chiglitazar 48 mg(n=246),or sitagliptin 100 mg(n=248)once daily for 24 weeks.The primary endpoint was the change in glycosylated hemoglobin A_(1C)(HbA_(1c))from baseline at week 24 with the non-inferiority of chiglitazar over sitagliptin.Both chiglitazar and sitagliptin significantly reduced HbA1c at week 24 with values of-1.40%,-1.47%,and-1.39%for chiglitazar 32 mg,chiglitazar 48 mg,and sitagliptin 100 mg,respectively.Chiglitazar 32 and 48 mg were both non-inferior to sitagliptin 100 mg,with mean differences of-0.04%(95%confidential interval(Cl)-0.22 to 0.15)and-0.08%(95%Cl-0.27 to 0.10),respectively.Compared with sitagliptin,greater reduction in fasting and 2-h postprandial plasma glucose and fasting insulin was observed with chiglitazar.Overall adverse event rates were similar between the groups.A small increase in mild edema in the chiglitazar 48 mg group and slight weight gain in both chiglitazar groups were reported.The overall results demonstrated that chiglitazar possesses good efficacy and safety profile in patients with type 2 diabetes inadequately controlled with lifestyle interventions,thereby providing adequate supporting evidence for using this PPAR pan-agonist as a treatment option for type 2 diabetes. 展开更多
