Mechanism of Danggui Sini Decoction(当归四逆汤)for Atherosclerosis Obliterans Based on Network Pharmacology and Molecular Docking

Objective:Based on network pharmacology and molecular docking,we want to explore the pharmacological effects and mechanism of Danggui Sini Decoction in the treatment of lower extremity arteriosclerosis obliterans.Methods:Using TCMSP database to search the main active components of Danggui Sini Decoction(当归四逆汤,DSD)and their related targets,Cytoscape3.8.1 software was used to construct a"component-disease-target"interaction network.Meanwhile,DAVID database was used to enrich the key target genes with GO and KEGG functions.And we used Auto Dock Tools 1.5.6 for molecular docking.Results:A total of 45 candidate active molecules and 250 potential target proteins related to ASO were screened.Key genes such as TNF,IL6,VEGFA,MMP9,IL1B,CCL2,CXCL8,ICAM1,VCAM1,and IL10 are mainly through TNF signaling pathway,HIF-1 signaling pathway,cytokine-cytokine receptor interaction,Toll-like receptor signaling pathway,NF-kappa B signaling pathway,MAPK signaling pathway and other biological pathways exert their effects.The results of molecular docking showed that 5UUI-sesamin,5UUI-paeoniflorin,4XCT-sesamin,and 4XCT-sesamin have extremely strong binding ability.Conclusion:Danggui Sini Decoction(当归四逆汤,DSD)can synergistically exert pharmacological effects through multiple components,multiple targets,and multiple pathways.On the basis of regulating immunity and inhibiting smooth muscle proliferation,it can prevent the occurrence and development of ASO.

A Meta-Analysis of Treating Radiation Proctitis by Retention Enema with Integrated Traditional Chinese and Western Medicine

Objective:To systematically and objectively evaluate clinical efficacy of retention enema with integrated traditional Chinese and western medicine on radiation proctitis(RP).Methods:CNKI,WANFANG,VIP,CMB and foreign language databases like Embase,Central and Medline were searched.Search dates were from establishment of the databases until May 2020.Randomized controlled trials(RCTs)of retention enema with integrated traditional Chinese and western medicine for RP were retrieved.Two system reviewers selected literature materials,extracted literature data,and evaluated the quality of the literatures.RevMan 5.3 software was used for data analysis.Results:A total of 12 studies were included,including 941 patients.Meta-analysis results showed:Clinical efficacy in the retention enema group with integrated traditional Chinese and western medicine was better than that in the simple western medicine enema or blank enema group(OR=5.96,95%CI[5.50,14.29],P<0.00001).Improvement of abdominal pain in the retention enema group with integrated traditional Chinese and western medicine was better than that in the control group(MD=-0.57,95%CI[-0.72,-0.42],P<0.00001).Treatment for hematochezia in the retention enema group with integrated traditional Chinese and western medicine was better than that in the control group(MD=-0.55,95%CI[-0.69,-0.42],P<0.00001).Improvement of diarrhea in the retention enema group with integrated traditional Chinese and western medicine was better than that in the control group(MD=-0.69,95%CI[-0.91,-0.48],P<0.00001).KPS scores in the retention enema group with integrated traditional Chinese and western medicine were better than those in the control group(MD=11.91,95%CI[3.81,20.01],P<0.00001).Conclusion:Clinical efficacy of retention enema with integrated traditional Chinese and western medicine on RP is better than simple western medicine enema or blank enema.Due to the low quality of the included clinical literatures,it is necessary to carry out more deliberate,detailed,high-quality,randomized,double-blind,and multi-center RCTs in order to give more powerful demonstration on its clinical efficacy on RP.

Observation on Therapeutic Effects of Electroacupuncture on Polycystic Ovary Syndrome of Kidney Deficiency and Phlegm Dampness Type and Oxidative Stress Level

Objective:To observe the clinical effect of electroacupuncture on improving polycystic ovary syndrome(PCOS)of kidney deficiency and phlegm dampness type and serum oxidative stress level.Methods:There were 68 patients with polycystic ovary syndrome were randomly divided into observation group(34 cases)and the control group(34 cases),The observation group was treated by electroacupuncture,while the control group was treated by placebo electroacupuncture.Combined with the evaluation of menstrual cycle,menstrual color,quality,quantity and TCM symptom integral index,the difference of curative effect between the two groups was observed,and the levels of peripheral blood,sex hormones,insulin resistance and oxidative stress were detected before and after treatment for 3 months.Results:The total effective rate in the observation group was better than that in the control group(P<0.01);After intervention,serum LH,FSH,E_(2) and LH/FSH in the observation group were significantly different from those before intervention.The D values of serum LH,FSH,LH/FSH and E_(2) before and after intervention were significantly different between the two groups(P<0.01);Comparing the level of androgen,the serum A4 and T in the observation group were significantly different from those before intervention(P<0.01);After intervention,there was a significant difference in serum A4 and T between the two groups(P<0.01),and there was a significant difference in D value between the two groups before and after intervention;There was a difference in insulin resistance in the observation group before and after intervention(P<0.05),and there was a significant difference in D value between the two groups before and after intervention(P<0.01);After the intervention,the serum SOD,MDA,ROS in the observation group were significantly different from those before the intervention;After intervention,the results of serum SOD and ROS in the observation group were significantly different from those in the control group;There was no significant difference in serum MDA results.There were significant differences in SOD,MDA,ROS between the two groups before and after intervention.Conclusion:Electroacupuncture can improve oxidative stress injury,insulin resistance and hyperandrogenism in PCOS patients.

Systematic Evaluation and Meta-analysis of Efficacy and Safety of Zhichuanling Oral Liquid (止喘灵口服液) in the Treatment of Bronchial Asthma

Objective:To systematically evaluate the effectiveness and safety of Zhichuanling Oral Liquid(止喘灵口服液)in the treatment of bronchitis asthma.Methods:A systematic search of CNKI,Wanfang Data,VIP,CBM,Cochrane Library,PubMed,Embase and Web of Science was conducted to screen out randomized controlled trials of Zhichuanling Oral Liquid in the treatment of bronchial asthma.Meta-analysis and systematic evaluation of the included studies were conducted to finally included researches using the Cochrane Handbook 5.1 evaluation standard tool and RevMan 5.3 software.Results:Five studies were included,with a total sample size of 354 cases,176 in experimental group and 178 in control group.Meta-analysis showed that the total effective rate of Zhichuanling Oral Liquid combined with conventional treatment was better than that of conventional treatment alone(RR=1.27,95%CI[1.13-1.42],P<0.0001).Conclusion:Based on the existing data and methods,Zhichuanling Oral Liquid combined with conventional Western medicine in the treatment of bronchial asthma can improve the total effective rate.However,the quality of the included studies is not high and the number is small,so it is necessary to adopt clinical trial protocols with large samples,rigorous design and in line with international standards to improve the quality of evidence in the future.

Meta-analysis about Western Medicine combined with activating blood drugs on modulating blood glucose and lipids in diabetic patients

OBJECTIVE:To assess the effect of activating blood drugs(AB drugs) on blood glucose and lipid levels of diabetic patients.METHODS:Systematic searches were conducted on 6 electronic databases and the search time was cut off in May 2016.Randomized controlled trials(RCTs) were included if they investigated diabetic patients who received Western medicine combined with AB drugs in the experimental groups and solitary usage of corresponding Western medicine or placebo in the control groups.6 outcome measures,which were considered as inclusion,were changes in fasting blood glucose(FBG),glycosylated haemoglobin A1 c(HbA1c),total triglyceride(TG),total cholesterol(TC),low density lipoprotein cholesterol(LDL-c) and high density lipoprotein cholesterol(HDL-c).The literature quality was assessed by Cochrane score for risk of bias in Review Manager 5.2,as well as meta-analysis was performed.RESULTS:7 RCTs comprising 999 patients met all inclusion criteria.Meta-analysis showed beneficial effects of Westernmedicine combined with AB drugs on modulating FBG,TC,LDL-c and HDL-c compared with Western medicine or placebo(P<0.05),while no difference was observed in HbA1c,and TG(P>0.05).To some extent,the therapeutic effects of Western medicine combined with AB drugs were better than that of solitary usage of Western medicine or placebo.Besides,AB drugs were safe and reliable with little toxic and side-effects.CONCLUSIONS:The meta-analysis suggests beneficial effects of Western medicine combined with AB drugs on modulating blood glucose and lipids in diabetic patients.

Academician Shen Ziyin(1928-),ethnic Han,born in Zhenhai,Zhejiang Province,graduated from Shanghai Medical Schoolas a medical undergraduate in 1952 and startedle a rning traditional Chinese medicine with Prof.Jiang Chunhua since 1955.He was rewarded the gold medal of"Carry forward Chinese Medical Heritage"with his tutor in 1959 and became academician of Chinese Academy of Sciences in 1997.Academician Shen was rated as Shanghai famous doctors in 1995 and won the honorary title of"National Famous Doctor of Traditional Chinese Medicine"in 2017.Academician Shen served as professor and doctoral supervisor of Shanghai Medical School,the director of traditional Chinese medicine department,the head of the Institutes of Integrative Chinese and Western Medicine,the head of the department of Integrative Chinese and Western Medicine of Huashan Hospital affiliated to Fudan University,the member of degree and academic committee in the university and the hospital,the member of medical evaluation by the academic degree committee of state council,the chairman of Chinese medicine evaluation committee of ministry of health,the vice president of Chinese Association of Integrative Medicine,president of Shanghai Association of Integrated Chinese and Western Medicine.At present,he is the honorary dean of the Institutes of Integrative Chinese and Western Medicine of Fudan University and lifetime professor of Huashan Hospital Affiliated to Fudan University.Academician Shen's study is mainly based on the exploration of integrated Chinese and Western Medicine,the nature of kidney and traditional geriatric medicine.
